Theory: I'm pretty sure what Billy has is a literal brain worm. He might have had a tumor from the Temp V, but when he took Compound V 4 months ago he superpowered a parasite that he may have picked up years ago. The worm has been living in Butcher, eating his brain. When Ezekiel was killing Butcher the worm came out to protect it's host, before slithering back in his ear. At some point Billy's worm will find a new host in Sister Sage. A supe who will constantly regenerate brain matter for it to feast on.

I think that Billy will try to get rid of it using the conservatives favorite drug "Ivermectin", But it won't work. He'll only lose the worm when it jumps ship and finds a better host, Sister Sage. A supe who's brain constantly produces new brain cells for it to feast on. Leaving Sage useless to Homelander, but likely happier in the end. She'll be able to enjoy trash tv and junk food full time now

In the comics, after Black Noir kills Homelander and then Billy Butcher kills Black Noir, Butcher still isn't satisfied with his vengeance and wants more. He wants to kill all supes and coerces the rest of The Boys to help him, but they all think he's gone too far. Butcher realizes his plan is going to fail, so he lies to Hugie telling him that he killed his parents forcing Hugie to kill him out of spite. Hugie then looks at Butcher's smiling corpse knowing that he corrupted the initially innocent Hugie into becoming a murderer. So, if this Joe-Fight Club theory turns out to be true, it'll be a better ending to the series than what the comics had.

More likely that when Billy took the V, the tumor is actually what mutated and got powers...not Billy himself (per Ryan's explanation). Therefore Billy wouldn't be durable or have super strength, but rather the tumor would. So Billy is basically now a vessel for the super up mutant tumor...symbiotic relationship. The tumor may need Billy to survive and vice versa. I.e. very much like Venom just with some deranged context inserted.

The scene seemed to say that he changed his mind was preparing himself to say goodbye. But somehow V got in his IV anyway. I think Hughie’s mom is or was one of Vought’s “Scouts” for expecting parents to have a Supe baby.

She has powers. She used her X-ray vision to see that Hughie had the Compound V in his pocket. How else would she have known? Unless she can telepathically read someone's mind. But I doubt that. The way she looked at Hughie was like she was looking right through him like Superman does. Then she gave his dad the Compound V when Hughie went to the coffee machine. That's my theory. We shall see what happens in episode 5. I can't wait.

To me, this can be explained pretty similarly to the situation where hughie’s mom (who could potentially be a hallucination) seemingly injects the V into hughie’s dad. These actions could be performed by the characters who are hallucinating but their mind covers it up to keep the delusion alive. For example, Butcher could have gotten the stuff for the cookies and then repressed the memory subconsciously to keep his delusion of Joe helping him active.

On board with that theory especially after episode 5 where (spoiler alert) In the final scene, Butcher and Kessler are alone in Stan's barn with Vicky's husband who they amputated to convince that he died while actually keeping him to remake the virus that they used up to survive in the episode. The fact that Kessler is unknown to anyone else but Butcher really drives home the idea that it's the cancer taking over and if he keeps letting it, Butcher will meet the same fate as the comics, where he kills the entire team except for Hughie who kills him in the end.

Slight correction: gunpowder did have super strength, at least applied more than a peak human with a crowbar, or a speeding armored truck. As in, gunpowder made butcher actually move with every single hit, where as weaker supes (Maeve is debatable actually) could tank a speeding armored truck without moving, and stormfront (whose definitely weaker than V24 butcher) could casually take full crowbar swings from a peak human butcher without even flinching or slightly moving, Maeve herself also called the entire payback team very strong. And I mean even mindstorm could tank several full hits from soldier boy, and could slightly knock back soldier boy with a punch (though obviously weaker than him).

I just realized that the last episode of diabolical was explained in S4/The Boys. They told Highlander to wait for Noir and to follow the script prior to entering the facility. This would explain a lot of the dialog and Highlander being confused about how things were playing out. The actors(faculty/terrorists)were trying to stay in character, but lost it towards the end.

As his condition worsened, his personality manifested itself to him while he was still "awake" and he saw him as a completely different person who could physically and mentally coerce him into doing things. Later on he learns from his love interests that that different person was actually himself and they show that everything he did, he did himself

In the scene with Billy and Ezekiel, the parasite seems to travel to his eye, and when Victoria neuman activates her head explody powers her eyes are affected (turn hazy/white). In the scene when Billy comes back it looks like Ezekiel has exploded. I think the parasite somehow gave him powers similar to hers for a limited time.

But now he again killed someone, rage is building up in him, maybe now the V would buff the rage and give him new powers that were related to the state from the moment he took the V. I don't think that parasite comes out of his body & goes back in after kill. I think it will be more likely Venom type which may bring back his season 3 powers (Laser eyes, Super strength, Durability) also might add up additional powers like flight, super speed. Thus Season 5 grand finale could end with Superman vs Superman type climax fight

It looks to me like his parasite is keeping him alive! It's probably the parasite that absorbed all the Compound-V. Thanks for pointing that out to us. It looks to me like this parasite (cancer) has Victoria Neuman's power to pop heads, except this power can pop the whole body. What was the signifigance of that whale? My guess is that Homelander was once their "whale" in terms of the information they got from testing him. I just love how they reminded us about the whale from Season 1. 🤣
