That ending makes a lot of sense when you understand the designers state of mind while making this game. Edmund made this game at a period in him game making career when he started to feel a little disenchanted with game design. He felt really shitty about making games, and this game is essentially a reflection of that sentiment. When the game released, he avoided talking about the game because it was very personal for him to publish a game about his depression with games. The ending with the toenail reflects how he felt about making games. Edmund was worried if he would ever be able to rise from this depressing game trench and make something new and amazing again; hence why ash was worried if his toenail would ever grow back in right.

The whole monologue segment is also a roundabout reference to time fcuk, another Edmund game that is closely linked to this one

I have this weird theory that a lot of Edmund games are tied together. When Dr. Fetus (Super Meat Boy) dropped the bombs onto Rapture, a fallout had broke out. Probably tying up with both of Time Fcuk and The End is Nigh. The END. One of the final levels in the story of Super Meat Boy. Rapture and the End are both in a ruined city, full of skyscrapers. Steven, who might've been irradiated by the bombs, was probably having trouble coming up to terms with his loss of everything. In this, he wanted to make his time count. The other versions of Steven, which I think it's safe to say that it's him hallucinating, urge him to 'Get in the box'. Presumably, 'the box' could refer to a fallout shelter. Though, the bombs had most likely already dropped. OR, this could be before, and then maybe Ouroboros could be after the bombs. The Binding of Isaac, could be an interesting one. Isaac and his mother live out of the city, as seen in depictions of pictures and cutscenes. I think safe to assume that Isaac's family lived out of the city, yet not too far to be effected by the radiation. This is most likely also be the reason why Isaac's mom is the way she is. Maybe there is no Christian broadcast? Maybe it's all recorded tapes, and that's all they have. And religion is the only thing they have. The radiation can most likely have an affect on Mom's brain. I would say this also may be another reason why Isaac may be bald, but Edmund himself stated that Isaac's Mom shaved his head. The basement could be filled with items, as maybe they were stockpiled for the fallout. Now, Spewer. This one is a bit weird. I like to think that Spewer takes a few years later after the bombs hit. Remember the hospital from Super Meat Boy? It's possible that the lab in Spewer, is that very hospital. It's most likely had renovation to look nicer and more repaired. The scientist may have been tasked and paid to create some kind of technology to remove the effects of radiation. Instead, he's far more interested in Spewer. This may also be the reason why he paces around and looks worried in the background. Oh boy, just Aether. Aether take place in the mind of a boy, I assume. I like to also assume that this is actually Isaac. Isaac is dead, that's easy to say. But what if this is just his afterlife? This is his heave. Wanting to explore space without a worry in the world. Aether does have a dreamy and calm vibe with it, something rare for an Edmund game. Gish is the only one I can't seem to figure out. That's mostly because I've never played it, and probably should.

I think spewer was before super mea boy, remember the hospital boss? He looked a lot like spewer right? My theory is That after spewer's ending more "spewers" were created, one of them was chad, but That chad destroyed the lab due to how massive he was, and when you beat him in smb he becomes smaller and looks a lot like spewer.... And gish is maybe before everything, because in the game nothing is "weird" so it maybe was just a common day.... Or maybe gish is a tar ball affected by the radiation you just said!

My personal theory about Isaac's placement in the "Edmund megaverse" is that all other games Ed has worked on are videogames Isaac has in his Gamekid. The warp portals in SMB could be some of the meat boy games Isaac used to play

Yes, and no. Ash escapes death, becoming immortal (one with mother) and never ascending, which might sound nice, it when you think about it, living forever is the worst possible thing. In the other ending, Ash accepts his fate and dies, much like the whole world did, and really, when you think about it, its not that bad of a thing, living in a post apocalypse world where everything is dead and there is nothing to live for is pointless, Ash was just afraid.

The decisions to make once this unlocks: suffer through the cart levels for more lives or suffer through 'nevermore' to avoid having to play more of the cart levels. I managed to beat it, barely, with only 20 tumors and I got so close so many times but thanks to my OCD for this game I managed to beat it with only a handful of lives remaining. 10, 12, and 15 were the most stressful but I think I lost the most lives on 13 even after figuring out the pattern.
