It was just before the Duggan vs Gordy UWF Tournament Finals for the UWF Championship. OMG entered the ring to challenge the winner. That would lead to a wild Duggan vs OMG brawl and that's when Duggan caught the bolt on the ring post. After the brawl, the Gordy would beat Duggan to win the UWF title.

No. About two weeks later they did a loser leave match. I could be wrong, but I think they had had a few loser leaves matches, in the area, with Duggan losing each.

No there was a loser leave town match where Duggan lost to OMG that was on a UWF tv show which if this date here is correct, was probably just days later.
