Could be but personally, I don’t think so. Because she has gone mad after (fake) Yu Ying Er ghost is haunting her. She doesn’t even know it was a fake ghost, let alone knowing it was the Trio’s plan. My personal analysis has two probabilities of who she was. One is Song Zhi, after Huafei has been demoted to Second Attendant, Song Zhi is also demoted to Palace Maid. After Huafei’s death, it is unknown for Song Zhi’s fate but she could possibly become concubine later and somehow is against Zhen Huan but the story wasn’t told. Another possibility is that she’s fifth prince’s mother. In Zhen Huan, fifth prince has been mentioned a few times but neither their him and his mother is shown in the series. It was only mentioned that he was raised in Yuanming yuan alongside the fourth prince (Qianglong) and that he was too unruly and unfitted to be the crown prince. But after Hongli has been crowned the next emperor, she wasn’t pleased perhaps of Hongli’s actual origin and turn against Zhen Huan.

Actually the empress did believe it was Ruyi's fault. Hailan cursed the empress' son by burning some paper thing (in addition to causing his death via allergies). Then she and her maid heard someone coming, so they put out the fire quickly and ran. The empress' maid found the burnt cursed paper things, showed it to the empress, and they assumed Ruyi did it since it was found close to her palace. That's why the empress thinks Ruyi killed her son. So technically it's Hailan's behavior that caused this particular grudge. Not saying I dislike Hailan or anything, just saying that every action has a consequence in this show. That's just a side effect of the plotting that goes along with living in the palace. It is true that they didn't bother looking for evidence, they just found a cursed thing and assumed Ruyi did it. The empress' hatred made it easy for her to believe. Too bad her own negative actions were the cause of all of it and she was too blind to see it until that night that Mei mentioned "measure for measure" and the empress freaked out lol. I couldn't believe that was the first time the empress considered that her own behavior was related to her struggles. She's so delusional.
