The scene starts with Tony LITERALLY trying to fix a heart, 2 pieces of it to be exact. One fits, which is the part that represents his uncle, the discarded part is his mother. There is no way to repair that relationship, but with his uncle is possible.

Watched this episode last night. Yeah the person they're talking is never shown on screen and is never mentioned before this. You find out he was murdered a few scenes later after his parking space is shown empty and a cop finds an abandoned car and a hairpiece on the ground in the following scene. Janice mentions him to her mother later in the episode to persuade Livia, as part of attempted manipulation of her mother, that it would be safer if she moved home with her rather than stay in a place when the owner was murdered. But that's it really. Not sure what the point of mentioning him is. Maybe there's some unused footage that it relates to it.

Interesting that the opening scene is tony fiddling with a heart model. Junior says “You break that you’re paying for it”. Gandolfini broke his and he paid for it with his life. Interesting real life foreshadowing.

I loved the character of Uncle Junior but let's face it, he fucked it up. He fell for it, I mean he didn't see Tony's trap and became a figurehead boss. And on top of that and to save face, he tried to kill him. Junior should have stayed captain of his crew.
