This turned out to be an interview for one person. She got free doctors advice and took up all the time. I am 73 and have used the patch for 25 years after a complete hysterectomy. I my doctor is trying to get me off the patch but I feel I still need it. I went from 0.05 to .0375 and now I don’t sleep more than 4 hours a night have headaches, vaginal dryness and depression. She refuses to keep me on my dosage because I had a stroke 15 years ago. I think it should be my choice. I have my yearly mammogram and have my heart checks.

I agree patients should be deciding what risk level they are prepared to accept (within limits). I wonder if you could phone in yourself to get her opinion on your situation.

And this dosage will not protect you from Alzheimer's, Heart Disease and Osteoporosis. Time to find another, far more educated, doctor!

Great video! I love the estradiol patch. I’m still surprised at how many gynecologists will not support HRT. Ladies, find a doctor who knows about HRT and will prescribe them. Don’t suffer needlessly.

I’m so thankful for all the doctors talking about hormone replacement online and on KZbin especially you Dr. Susan you are helping so many women I suffered for so long with so many symptoms my worst symptom was not being able to sleep for so long so many years I am seven years postmenopausal and my doctor never mentioned it to me even though I had these symptoms it is so heartbreaking that doctors don’t advise women about Hormone replacement my mother is now suffering with dementia because she never took was told by anyone to do hormonal replacement and it’s so sad so I’m thankful now for the knowledge and I’m helping my sisters now and I will tell all women that I come in contact with about this wonderful treatment they are gonna think I’m crazy but they will thank me later. Many women still have breakthrough bleeding when taking 100 or even 200 mg progesterone gel caps, I’m one of those. Many have to change to using the gel caps as a suppository for better absorption in order to stop the bleeding. If taking progesterone orally works for you, you’re lucky!

No clue how I missed your segment. I'm a fellow Houstonian and a patient at your office! I had a consultation at your office with your nurse about a year ago. Y'all did my labs and it was extremely useful. I am ready for my follow-up labs and sense I may need to increase my estrogen. Will see what the labs show. Thank you and your team💖

Thank you for all of this extremely helpful information that has been long, long overdue to be made available to women. I'm learning a lot from you and I hope I can "teach" (haha) my doctor. I'm currently in a search for a new OBGYN since mine recently retired, and in the meantime, I saw my internist for GYN needs. That visit upset me so much because although I adore this doctor in general, she is not a specialist and that is what I need. I would speculate that ALL women would benefit from seeing specialists. PS: in case it is of interest to you, I went through what they used to call premature ovarian failure (premature menopause) for no reason at age 37. I've been on estradiol and progesterone orally for 21 years and have had no troublesome menopause symptoms. Also, my bone density, cardiac health, and as far as I know my brain health are all fine. I also do not suffer from any genitourinary tract problems. HRT is not just for "symptoms" -- it is for lifelong, postmenopausal health benefits!!!

As an update, I did find a new (younger than me!) OBGYN who is completely on board with my goals for managing this stage of my life.

I wouldn't mind just having a period once a month, but I haven't stopped bleeding since I started HRT. I use a patch and 100mg of progesterone daily. Was thinking to just cyclically dose myself by taking 2 pills for half the time and see if that fixes it. But idk what days of the month to hold the progesterone. How do you take it?

Dr. Susan, if a person doesn’t respond to their hormone treatment and/or combinations an Endocrinologist may be able to crack the case. They test the entire glandular system and they have a series of markers including enzymes in their tests. They love their science. I see an Endocrinologist for Adrenal Insufficiency. I’m stuck in a rabbit hole here. I have a cervical cord injury that causes spasticity. The signal blocker Gabapentin I take for it and others like it were studied before Covid by the FDA for their affect on the pituitary. Turns out, my weaning down on Gabapentin we discovered I’m in a rare group that suffers with adrenal insufficiency. A third of my pituitary suppressed, after 3 years of having a grave case thanks to the FDA swinging that door open to study Gabapentin I started scoring normal cortisol. My cortisol went from 0.9 to double digits highest is 17 proved it was a side affect to the drug needed to treat my cord condition. I’m not cured. 800mg of Gabapentin a day is not enough to deal with spasticity so I get stuck having a mini crisis. I use injectable hydrocortisone. I can rescue without going to a hospital. My Endro learned from my OBGYN daughter in law to try the patch, never occurred to him to try it AND for post menopause my daughter in law learned low DHEA affects libido in both men and women Per my Endro. I checked, the info is out there. Unfortunately we don’t have a baseline on me before the pituitary suppression but I took Premarin as my HRT during menopause. I take DHEA supplement We’re hoping the patch will help the DHEA come up. Anyways, I’m excited to get some estrogen back. You’re right, without it, everything goes ka-put.

I've heard widely varying recommended levels in blood work and that's why it's so controversial. In menopause the level that we should have is 0 so figuring out what we want to have should just be based on what was normal in our lives before menopause, but that varied daily in our cycle. I think 40 to 80 is quite arbitrary and I have heard 180 on other channels, so to each his own. I also think we should be very careful when referencing patches that we express the dose correctly; specifically, that it's not 0.5, it is 0.05; that it is not 0.25, it is 0.025; that it is not 0.75, it is 0.075; and that it is not 1.5, it is 0.15. Hugely important.

I think blood work combined with the symptoms we are experiencing is important. You are right. We are all different, and one approach does not relieve all symptoms.

I gained weight on the patch! Creams torches never but they quit working. Patch worked great but now wright gain! Perhaps to high of a dose

No, there is a .0375 dose as well. In the U.S. anyway. Maybe you are not in the U.S.

@tonettejackson7899 body identical estrodiol patches and progesterone pills are available at very reasonable prices across the US, and are typically covered by insurance. Bioidentical pellets are not covered by insurance, are not necessary for everyone, and they are quite pricey. Personally, I'll probably stay away from the pellet and just go with body identical. Best wishes.

I started on Combipatch 0.05/0.14 and even after 3 weeks I can tell a difference. I am sleeping more soundly, mood has evened out a bit and vaginal dryness/integrity has gotten better. They say it takes 3 months to see full benefits so I am hopeful that it can only get better yet with time.

I had a total hysterectomy in 2012, took estradiol tablets for about 2 years. Took myself off of them as I was afraid of getting cancer from hrt. I’m now 53, have been on Climara .325 mg, for about 6 months and now I’m losing hair!!! It’s not on my pillow but when I comb or brush it, I’m losing A Lot! I have long, fine, dark brown hair and have a few gray hairs. I’m taking Biotin with Collagen gummies plus 50+ women’s multi vitamins daily and have been for about 8 months. I’ve taken tenormin since I was 23 for mitral valve prolapse, Gabapentin, low doses of pain meds due to a rare congenital severe arthritis condition plus my ra level is 54. Yes!!! I hurt every day and have since I was a child due to my congenital condition but this hair loss is frightening! Do you think I should stop the vitamins? I was on a normal multi vitamin and the vitamin A was lower than this 50+ vitamin. I’ve noticed hair loss since the change of vitamin but could the Climara patch be responsible for my hair loss? I don’t have ovaries or anything and I was put into post surgical menopause and was on my cycle when I had my total hysterectomy so that was horrible! I have no hair on my arms now and that just happened within the last month. Climara was a miracle for collagen that I lost and seems to have helped my arthritis symptoms. Can’t take arthritis treatment cuz the Dr said my condition is congenital.😢 I also have had lower left leg and ankle swelling the past year and I only weigh 115. Lost from 125-113 in a month this April and think that was stress related 🤷🏻‍♀️. Hopefully I’ll be divorced soon due to domestic abuse. Mental abuse. Yes I’m miserable but can’t find a place to rent that I can afford and my family is 3hrs away. Lost my only son in 2014, my husband at the age of 35(he accidentally shot himself in the head), and the husband before that, abused me for 13yrs(kids dad). My life is stressful so could that be the reason for hair loss, could it be poor circulation from my leg, my vitamins, or the patch? I’m praying you can help me and sorry for all the info but I’m trying to touch base on things to help.

Hi, I’m not a doctor, but I wanted to mention that I was prescribed Gabapentin, and it made me lose my hair like crazy! My doctor had me reduce the dosage, and the hair loss lessened a bit, but then it got bad again when I tried going to the full dose again (I wasn’t even taking very much, compared to most people). You didn’t mention how long you’ve been on Gabapentin, but I would suggest trying to stop that if it’s possible, to see if your hair loss improves. Gabapentin was terrible for me and my hair. The hair loss stopped after I stopped taking it. 🙏🏻

I turned 25 less than 3 months ago. Shortly after, finally booked in with a gyno for always having struggled with irregular and painful (nearly vomiting painful) periods. She did some hormonal testing, and while nothing indicated PCOS or thyroid issues, my estradiol was 15.4. Believe it or not, she thought it was a fluke! I knew it wasn’t, and thankfully she wanted me to test again about a month later. It was 17.4. Now knowing the symptoms of low estrogen, I have a feeling I’ve had it for years. Should someone like me inquire about patches? If I go from 20 years old onward with low estrogen, I hate to think of my risks.

Danika, that low number is what menopausal women have. And anything below 60 means you are losing bone!! Only getting your estrogen at or above 60 will stop the bone loss. And the estradiol patch in a higher dose, can help. Also your doctor should have recommended a DEXA scan to see how much bone you’ve already lost. Or at the very least, Have you stood in your bare feet and had someone checked your height, the old fashioned way with a pencil mark on the wall at the top of your head? Also what do you think is the cause of this low estrogen? How is your diet? Are getting enough protein? Some people who don’t eat meat, have serious health issues. But you might consider finding another doctor because if you’ve been losing bone, you’re at risk for a fracture.

@@celeste-236I was able to get a second opinion, but was informed that their policy isn’t to look into HRT until someone is down to 4 periods a year 🙄 HOWEVER, I actually work in radiology and perform DEXAs as part of my job, and can neither confirm nor deny that I may or may not have had one done as “training.” I’ve since just had to take manners into my own hands and get in extra supplementation and focus on strength training. It’s not a fun thing to have on my radar, but at least I know about it now and have an idea of how to work on it.

@@celeste-236I thankfully work in radiology and am aware of the role of estrogen on bone health (I actually perform DEXAs!) and may or may not have had a coworker possibly or not possibly perform one on me on the down low. I’ve had to do a looot of my own research to work on prevention. The stupidest thing is that I did get a second opinion last month on the estrogen level, and I was told that “standard” protocol is to not worry about low estrogen until I’m only getting 4 periods a year or am unable to get pregnant 🙄 Absolutely pathetic.

@@celeste-236also! I had mentioned to my doctor that I have what’s called Ehlers Danlos Syndrome. One of the maaaany things it can affect is how well your digestive system absorbs nutrients, unfortunately. But, there’s no way of knowing if that’s the cause 🤷🏼‍♀️

@@danikacbass9438 I learned about retina issues from a surgeon when my nephew needed eye surgery due to an accident. I also recently saw a female GYN on YT say there are retina issues concerning lack of estrogen.

I was told by my doctor that the patch DID NOT treat dryness. You have to use the cream for that

Yes I've been on the Estradiol 0.025 going on 6 months and I have seen a huge difference the belly bloating the feeling full symptoms along with gas hot flash are gone I also recommend to my older sister she was having the same symptoms she's 54 She seen a huge difference as well I highly recommend I don't have any side effects from using them they are once a week patch they don't interfere with my workout or bathing I highly recommend this patch I hope this helps anyone who's interested in using them

@@babyluv1879In the first comment you said 0.025 wasn’t helping so you started taking the 0.05 and would update us. Then in your update you said you were still on 0.025 and loving it. I’m a little confused, did you bump up to the 0.05 and then go back down to the 0.025 and stayed there? Was there a reason? I’m only asking because I was just prescribed this today and I am TERRIFIED so I’m trying to learn and understand everything. 😭 Also my patches are twice a week and that stresses me out too. Plus I have to take 100 mg of progesterone on it and I’m so afraid I’ll gain weight because I’m only 5’1” and already weigh 184… I hate getting older so much. 😭

I am happy to report that all sexual drive has vanished after using the patch since 2009 (?), I think. It is a blessing to be free of that torment. I am 77. My Doctor is reluctant.

I take the .05 mg estradiol patch and 100 mg mircronized progesterone. Before starting hormone replacement i hadn't had a period for 6 months and now I have one every month again. So i'm puzzled why i'm getting regular periods again when my doctors are doing all the right things why I am still getting a period

Are you taking your Micronized Progesterone nightly or cyclical? You could be peri-menopausal instead of menopausal, which is defined as 12 months since your LAST period.

Since starting the estradiol patch I gained 12lbs!! I am a diabetic I take novalog twice a day and ozempic 1 mg every Sunday. I eat organic foods drink water every day and I walk 3 to 4 times a week...why Am I gaining weight and not losing weight???😮 also I had a complete hysterectomy when I was 30 I'm 53 now and I just started estradiol 2 months ago.

I have been on an Estradiol patch for yrs and mine is at 0.1 MG and was told I could not get it any higher, I had a complete Hysto in 2018 still on the patch and feel it's not doing anything. I finally made an appointment with a hormone clinic and they took my blood and I'm getting the pellet.

Me too! Years on the highest patch. I don't sleep well, dry skin, anxiety, inflammation, hair fall. I had a blood test and found i have less estrogen in my system now than the last year! I've just ordered cream. Hoping I can get levels up and feel better.

I am 51 and I want to start HRT. I had a heart attack 8 years ago and have 2 stints. I was at 125 lbs and in perfect health. It happened out of no where. I am still working out and feel great. Do you think I am a candidate for HRT?

Im not post menopausal at 51 just missed a few over the past year so im on the progesterone for 14 days on and 14 off off and oestrogen gel every day. I felt better within the week less joint pain anxiety much less brain fog and slept better first night i too progesterone. My night sweats are pretty much gone and the weird sweat smell has gone im amazed.

Thanks. I’ve had menopause four years ago and started some HRT for two years or so and now observing that my tendency to obsess and binge on KZbin videos about skin care may be related to my HRT (100 mg progesterone pill and 0.05% estriol patch)--I might need to cut things in half. I’m so obsessed about things! Eek!

Thank you for your helpful videos. Can you please clarify… I’m 63, in estradiol patch .05, my blood work range for estradiol for the last 2 years has been 21,5,32 15!! That is lower than the range of 40-80 you recommend. My concern is that I asked for the .05 dose to prevent the Alzheimer’s, heart disease and osteoporosis but if the labs show such a low dose of estradiol, I am getting the full benefit or do I need to take a higher dose to raise the blood level? Thank you for your help.

Hi, it's not the dose of E that is preventative it's the E levels in your body (everyone absorbs E differently). So if you want to achieve the preventative levels you need a higher E dose. Hope this helps!

You don’t need the progesterone to prevent uterine cancer in your case, but you may still need it for the benefits that estrogen doesn’t provide.

I'm 67 started estradiol and medroxypro in 2007, 16 yrs now. .05 dose. My Np says I don't need patch, I would like it I think. Also menopause is long gone, do I still need medroxypro? I have tried to stop HRT a few times over the years ( because they said it was dangerous you know) and it was nothing but a nightmare each time so I'm not going to quit anymore.

If your on the patch and go off will the symptoms start all over again from the newly reduced estrogen when you’re weaning off? I’m afraid to start. I’m 10 level estrogen (so low) and 54 but I’ve had night sweated and flashes for 2 years. If I start psych and go off in 5 yrs will the symptoms start all over again for years to adjust?? Help.

It's not just the body, my brain is also malfunctioning. Sometimes I stop and pose and think what I am going to say, or I stop and think what i will be taking from the cabinet. I kinda have this blank moments. I hope I can go early tomorrow and get ultrasound, I will ask for patch if it helps my dryness. All sorts of things happening to me when I entered this 40's. Even spider veins are all over, maybe it is also hormonal. saggy cheeks, laugh lines, stretchmarks due to gaining fats on hips and thighs and legs, Pores are also huges, acne scars got deeper, My acne is getting hyper again probably due to androgen. My estrogen is declining. A few pieces of gray hair on my head. My cycle is shortening and dryng up. It's sad to grow old single and alone. I'm 46 now going 47 I feel like I have a death sentence.

Yes true for many women. I suffered for an entire year with no sex drive, loss of muscle strength and tons of mood swings/weepiness. Figured out my testosterone was in the 5th percentile. Started testosterone therapy and within 4 days I started to feel myself again- it was amazing! I’m now feeling normal with a great sex drive.

@@Lifee4224 It’s given via injection every three months by a doctor who specializes in Bioidentical HRT- the dose will be determined by your weight and a couple other factors and should start out conservatively in order to avoid unwanted side effects. I’m very happy with it so far- I’m about 4 months into being on it.

MANY MANY Male OBGyn’s are huge proponents of Estrogen/Testosterone supplementation for your healthy but jumping to a conclusion that her MD is a Male is just wrong and this is one Specialty that Men take a huge hit in, from Women. So there is no risk of clotting with the patch? My understanding is there is risk of clots with both just a bit less with the patch. Oral estrogen is 1 out 1,000 for clots and 1 out of 2,000 for the patch. Is this correct or am I misinformed?

The combi patch has Progestin in it, which is a synthetic form of progesterone. I would stick with oral Micronized Progesterone and the Estradiol patch, both of which are bio-identical.

My experience with doctors is you cannot say you want to be in a certain range. You must say you are symptomatic. They are told to prescribe ONLY to alleviate symptoms. My gyno told me I am at risk for heart disease & stroke by taking HRT. I told her I have heard the opposite. She shook her head, “no”

I won’t be taking it anymore. I was put on estradiol 0.5 dose 4 months ago, it’s a gel I had to put on my thighs. Within 3 months I got cystic acne, a very bad side effect of this stuff. I now have 2 abscess on my private parts, and 2 very painful cyst in same area. I’m on 2nd lot of flucloxacillian to clear them up. If they don’t go this time my g.p said they have to be drained. I also got facial acne, so much white stuff oozing out my skin. I felt sick. My doctors agree it was the estradiol that caused it, and has taken me off it. The patches made me itch insanely, and they wouldn’t stay stuck to my skin. I have to find natural foods and vitamins supplements to replace this hormone. I’ve had no estrogen in my body since having a hysterectomy in 2014, we are now in 2023.
