There was a version of this mode just for nukes (It's called Realistic Nukes). With it you could basically set state population to 0 with like 3-4 nukes

For anyone looking for substitute mods for this I recommend "devastation overhaul" it's a part of the "better mechanics" collection. It adds devastation from combat and bombing as well as refugee mechanics. As devastation rises factories disappear construction and war support drop and civilians either flee to neutral countries as refugees causing stability drops in the neutral countries until assimilated. Devastation can also cause civilians to be "removed from the game" It also adds strategic utility to nukes as a single nuke can spike devastation by 60% deleting a few factories and six figures of civilians.
