It was written by Jim Steinman, Meat Loaf's long-time musical partner. Nobody else could do pure, overblown bombast like Steinman. He's the only one who could get away with writing songs like this. He once described his music as, "Like Disney, but with an R-rating".

Her voice is so scuffed in this live version. Sounds like she was trying to sing either incredibly high on something hard, or like she went on stage to sing while being on the verge of losing her voice. If you listen to the studio version her voice is much cleaner and doesn't sound nearly as forced.
