TG actually went down, as would be expected. It's nuanced but here's the explanation: - Over just 16 days of eating +12 Oreos/day (short time frame), I did not become insulin resistant. - However, the extra Oreos did increase my insulin levels. Insulin stimulates Lipoprotein Lipase which converts VLDL to LDL (also generating HDL-C) and lowering TG. - This is why LMHR have high LDL-C, high HDL-C and low TG - So, upon increasing my fasting insulin relative to baseline - but still being insulin sensitive - when I got my lipids tested in the fasted state, the TG in chylomicrons from meals had cleared but there was a relative increase in LPL action on VLDL in the fasted state relative to the baseline ketogenic (Oreo-free) diet, yielding a slight drop in TG to the high 30s. Make sense? To be clear, I won't not expect this pattern to continue were I to eat Oreos for a year+ since the development of insulin resistance would eventually become more powerful. A key point here is that acute changes don't necessarily translate into chronic changes. But, in the case of Oreo vs Statin, the dynamic/acute changes were the outcome of interest since I/we were demonstrating the dynamic nature of Human Lipid Metabolism. If you don’t have high cholesterol or follow a keto-ish diet, this might not interest you. However, as someone with high LDL who fits Nick’s ‘lean mass hyper-responder (LMHR) phenotype,’ I find it fascinating. Nick’s experiment highlights how much the medical community still misunderstands about lipids and cholesterol and the need for more rigorous studies. It resonates with me because after going keto-ish, all my health markers improved, I reached my ideal weight, gained muscle more easily, yet my LDL skyrocketed. I’m also uncomfortable with my doctor pushing statins, given their potential side effects and limited benefits.

Agree to an extent- of course just getting the point is more efficient for most. But hopefully you can appreciate the mechanisms/science about this. It’s not a short tik tok video; a lot of time goes into these experiments and there aren’t really black and white answers. Also, what he is doing “contradicts” science and medical practices that are being implemented on real people everyday, so it’s definitely something that isn’t easily explainable, hence the drawn out video. Maybe that’s part of why sugar is addictive- it takes several weeks for the lipids to go back up and that may in older people or heavier people be a time of extremely low energy. Most people I know who tried Keto said that they had no energy and that’s why they quit.

Interesting. Could I suggest those people should stick with Keto for longer. In my experience it takes about 12 weeks to become truly fat adapted. I have a fair bit of energy on Keto - I exercise in the morning 7 days a week.

Yep, I have seen Nick's metabolic phenotype as somewhat unique, actually what he refers to a Lean Mass Hyper Responder. I would love to see the rest of his blood panel. I would guess he has a metabolic panel that is closer to a type one diabetic that has super good control of his insulin intake. I would guess his fasting insulin is low and very responsive. He would have excellent mitochondrial metabolism. He has great metabolic diversity. Low BMI is very relevant to his results. Basically, what most keto people aspire to be like. The superman of metabolic health. He is still pretty young. It will be interesting to see how that changes with age. Oreo sales went up and statins drug sales went down but understand that was not the point of his study. Very good discussion Thomas. Love many of the interviews you have had in the last few years.😊

@@nicknorwitzPhD You misunderstand me - just be curious as you say. I didn't mention placebo or anything emotional. (I'm also an MD (ITU) with a MA in physiology (Oxon)) I'm a LMHR - been into this from the start. I was getting blood sugars of 1.9 (UK units, normal is 5) during exercise on keto - completely asymptomatic. The brain does appear to need glucose, it is the highest consumer of energy per gram. It probably sits at the top of the energy equation. If you're doing high intesity exercise on keto the brain needs some new strategies. I'm sure you're familiar wtih Noakes' work - he now thinks it's all about blood sugar and our obsession with muscle glycogen was misguided. If blood sugars are buffered robustly there is less need for high circulating lipids and their derivatives. No need to envoke placebo/nocebo (although like everything it will have a role too)

I also have high cholesterol over 400 concerned my doctor I am a carnivore. I don’t believe it’s come down yet. I haven’t been retested for a while. I believe this may be what my problem is. I’ve been leaner, but when I was Hunter gatherer and Paleo diet, I was leaner, and 20 pounds lighter muscle mass and had low cholesterol, but I did carnivore to increase my bones mass and muscle mass without TRT or steroids just high protein and 10,000 eggs/64 ounces a day for one year and 1 pound of meat day I was able to gain 20 pounds of lean muscle mass but my vascular fat went from 2 to 6 And my subcutaneous fat went from 9 to 14.

How does Niacin factor into metabolism, since it raises blood glucose and lowers ldl and increases lipolysis and ketones after 4 hrs. My theory is that it depletes the liver of glycogen and forces it to metabolize fat once the glycogen is depleted. Most papers only say that the cause of increase of blood glucose is from temporary insulin resistance. However that doesn't explain the increased ketones and reduction in visceral fat. Only the liver needs to be depleted of glycogen for the body to make ketones, because of the brain liver vagus axis, from my understanding. My theory is the liver is drained of glycogen which floods the blood with glucose (without a corresponding increase in insulin?).

My emotional response that absolutely nothing changed for me since high LDL (based on the latest standards that were lobbied down by the statin companies to sell more statins to actually cause more heart disease since they actually want to kill you because they believe we have an overpopulation problem) is not bad and oreos are still bad for you because they lower LDL. People with the old standard of "normal" levels of cholesterol have been studied to live longer and have the least amount of all cause mortality rates then people with lower cholesterol or even higher cholesterol since cholesterol levels have a sweet spot. It's not about high or low. Both extremes are deadly. It's a bell curve. Total cholesterol at 140 and within an acceptable ratio in between the two types is the safest place to be yet your doctors are trained to convince you that you need to be on a statin with those numbers. Wake up people.

A lot of what Nick says is misinterpreted and you can see that in the comment section of his channel. If you take the time to watch his videos they are informative although sometimes I feel they have a keto bias, but I also understand that is Nick’s passion. In the keto/carnivore space Nick provides some of the most interesting content and I enjoy that Nick doesn’t talk with the level of certainty that other content creators do but focuses more around sharing data and information regarding possible mechanisms for outcomes.

But I also do high intensity weightlifting. I try to keep my heart rate 160 to 187 for over an hour of heavyweights and constantly moving and sprinting, and it doesn’t seem like it made my cholesterol go lower because I was doing that when I got the test that made me over 400.

I use one of Thomas's favorites for 3 days to lower mine. I have about 2,000 calories a day of macadamia nuts. It works for me but I am not a lean mass hyper responder. Next time I am trying a combination of macadamia nuts and purple sweet potatoes.

So for those who are lean low carb eaters who are trying get their doctor or spouse off their back, because their doctor or spouse still subscribes to the misguided hypothesis that LDL causes heart disease, a few weeks before your lipid panel is taken just up your carbs by around 100 to 150 grams and you should be good. Yes, in an ideal world your physician or loved one should amend their understanding of how erroneous this hypothesis is, but for many people, the brainwashing is too strong that it is just easier to game the system.

In my head, if LDL's purpose is to transport fats, doesnt it make sense that your LDL goes up on a low carb diet since you get your energy from fats instead? And it goes down again when you switch back to carbs. And presumeably the body can switch more effectivley if it is healthy/fit and not obese, right?

Simple sugars are good fuel during hard activity. No question about that. Ask every athlete from cyclist to strongman. If you are sedentary your intake of simple sugars should be basicaly zero apart form some fruit in moderation.. Same eith complex carbs to refuel glycogen in muscles and the liver. The more active you are the mote you can eat and vise versa.. For sick person who needs to lower inflammation and is not active. Low carb is preferable..

Every time I eat sugary foods before a running exercise, it has had a negative impact on performance. In short, sugar is not very good for the heart. At least for mine, it wasn't. To be honest, I think sugar is the worst you can have before a running exercise.

Sure it might be a cool experiment but 2 weeks to a month is not nearly long enough to truly see long term effects of a highly processed sugar carb like an oreo cookie. If I may I am 45 years old. I can give you a 40 year study of my own body. I have eaten the highly processed American diet for my whole life and guess what? I now have stage 4 colorectal Cancer. I have eaten Doritos the Big Bag from Costco and The big pack of Oreos on a regular. It was always a regular part of my shopping. I can tell you without a doubt if you eat 12 oreos a day for years. I guarantee you will either get Cancer or heart disease or diabetes. Dont do it. If you are going to eat garbage at least exercise. The exercise will at least help to get it out of your system faster. I have since gone full organic Keto only healthy fats. Guess what my health turned around immediately from eliminating highly processed carbs. Dont do it people. The Cancer rate is 1 in 3 for men and 1 in 4 for woman. Guess what the screening doesnt start until you are 40. If you are younger it can go unnoticed until its too late. Your health insurance and Doctors dont even look for Cancer until your 40. Moderation people.

You might consider checking out some of Tony Robbins free content here on youtube to help your husband reprogram his thinking on nutrition. Tony teaches how to associate pain with the bad habit and pleasure with the new one. i.e. reversing diabetes gets your husband back to his former sexy and active body - what great things could happen? The actual transformation from doing Keto or Carnivore is fun, easy, and you'll get quick results.

He then use his domain knowledge to draw possible inference towards what might be the cause. The Oreo cookies is a way to draw attention to the problem. Maybe try the Keto diet to clear up some of the brain fog.

If you wanted him to eat pure sugar then you miss 2 points. 1) It would make less headlines. 2) There need to be mechanisms behind it that makes sense and further increasing the Saturated fat intake to lower the LDL doesn't make sense from a biological perspective. At this point in time of Keto research and lean mass hyper responder, getting attention to the problem is much more important than doing double-blind studies that isolates the compounds completely. The studies have completely different purposes, they are not the same.

I believe those test the hospitals have were always flawed to begin with and they’re always finding more ways to widen the deception. Their theory of testing is like saying I got into a car accident because I didn’t wear a seat belt.

@nicknorwitzPhD may be sugar is pro aging. I gave up sugar, gluten, alcohol , processed food and smoking all together in 2013 after being diagnosed with an autoimmune condition. I reduced carbs and added fat and protein to my diet, started to eat 2 meals a day, nothing in between. Now my autoimmune condition is in remission. Also, I am 43, look much younger than my friends with the same age, still no gray hair. Not sure its sugar or eating habit overall.

Pro cyclists eat 120 grams of sugar per hour during races plus a lot more pre and post. They burn all of it and have no issues.

When I go deer hunting for a full day in the mountains I can knock back 6000 calories, about 500 - 700 grams of carbs and about 200 grams of fat. And still lose weight

You can't burn sugar off. You can burn calories, but sugar have adverse affects on the body. Eating 120g of sugar would actually make more sense to make them groggy.

the body is a dynamic process, it does what it needs in the long term, we are all short term focused trying this or changing that and every time we do this behai vor, it must adjust this insanity, and it never gets into a flow. diets and this type of mindset is subverting those processes. oh look all these studies that just came out and this is what we should do... so now we go change something on some BS study in which you trust people you don't even know. Our reliance on science is killing the years of evolutionary wisdom of the past. It's time to stop! The wisdom if the past cane up with terms Like SNAKE OIL SALESMAN...... There is a reason we created that Idea. Wake Up People. Oh, and the idea of a Diet? Ya go tell a Lion or a Bear he should go on a diet or try dieting. The body mind hates man made abstract concepts. His n=1 experiments might be an authentic attempt to engage in a creative dialogue about diet. However in reality, when you read the people who follow him they just take his short term experiments as a green light to keep eating way too much meat, fat, eggs etc.

A few things I have noticed about the online carnivore community and my personal interactions with people doing carnivore is 1) They talk with so much certainty and you know what they say about people that talk with certainty…..2)They start to become conspiracy theorists and end up in the flat earth space. Would love to understand the psychology that drives this.
