The best way to insert the speculum through the introitus is to rotate it 45 degrees to avoid touching the sensitive urethral meatus and to benifit from the fact that the oblique diameter of the vagina is wider .. you insert it closed and then open it when it fit inside nicely .. the reason why the posterior lip is longer is that the posterior vaginal wall is longer than the anterior ..so it gives you acsess to the deep posterior fornix where you take your swab .. this speculum is designed for exposing the lateral vaginal wall and the cervix ... and it is my first time to see disposable one i have only seen the metal one :)

So I am a nurse. And I am currently doing pre-requs to apply to med school. I had no idea that Ob/gyns didn’t get paid if they didn’t deliver. Doctors don’t typically tell us how their pay works. So thanks for telling us that! It would be awesome if you could let us know more on pay regarding different specialties?

I am a mum to five kids ages 31 to 8. My first delivery was twins which was medicalised due to pre eclampsia; the first was a forceps delivery and her sister just then came out. My second pregnancy was known as a domino delivery which meant I laboured at home, delivered in the hospital and came home six hours later. My last two deliveries were home births which were the best, nothing like the comfort of being in your own home. Hospitals have their place but here in the UK if you are deemed low risk then home births are well supported and managed. I am also a grandma to a four year old and another daughter is expecting her first baby, I will have to show her this video regarding the stages of labour :-)

Ya. Since the health care system in Canada is a lot different from the States, with regards to the insurance system and I didn't know that I have to assist in delivering a baby to get full payment . Either way just wanted to let you know that I love your videos. loved it!! such a fun and informative description of labor !! :-) but I am a little confused about what you said at the end, that OBGYN docs don`t get paid unless they deliver the baby, I have a coworker who is pregnant and I asked her if her OBGYN would actually deliver her baby, and she said no, it would be a different person. Asked other women and they all said their OBGYN doc did not deliver their baby. Are all those people working for free? :-) or maybe their niche is clinic work? thanks for the awesome video!

Insurance pays you for the bulk of the work when baby is born. Physicians who work in a team figure it balances out when they're on call because they end up delivering babies that weren't originally their patients.

Hey Jenny - 7 kids here, 9 pregnancies altogether. I actually got asked to present my high risk pregnancy (last one) to med students. I felt so bad for them as I’m such a mess lol. I’m under Maternal Medicine (previous multiple sections), haematology (at e antibodies), endocrine (over active thyroid) and cardiac (congenital heart defect ASD repair and current residual VSD). Couldn’t believe how many students didn’t think the cardiac issues were less of an issue lol. Also during my last c section they were expecting haemorrhage but I only bled 300mls (too low to chart apparently). So I was really happy with that. Would love one more baby but I think I’m pushing it now lol, plus I’m nearly 36 with a family history of twins (probably would end up with twins 😂)

When I shadowed a cardiologist over the summer it was him and his wife's 20th anniversary (his wife is an OBGYN) and they were going out to celebrate and as soon as they left to go out for dinner she got a call about an emergency C-section and had to leave. Being constantly on call does not seem the most appealing lol. It seems like you made the best decision!

+Gracie Perkins many people like ER because it's shift work. Example on for 3 days off for 4 days. When you're on you're on. And when you're off you're off. There is plenty of time to have a family. How much you choose to work depends on you and your family.

+Melissa Alexander it's hard not to tear with your first. But usually the OBGYN will hold pressure on the perineum and you need to push the head through more slowly. That's also harder with an epidural.

No. As a family practice physician, it is within my realm of practice to deliver babies. I would only need to do a fellowship if I wish to deliver high risk OB patients or do c-sections

+Danielle Jack Lol no we were taught in medical school. We had standardize patients. But from what I can remember they just taught "insert the speculum and find the cervix." 😅 Everyone's technique is different.

+Forever Saskie one of by best friends is the same way! That's why she went into internal medicine. lol usually when you're on rotations, your preceptor will ask you what kind of doctor you want to be. If you express that you're not as interested in delivering babies they won't push you as hard.

Hi Jenny! I just have a quick question, why do transvaginal ultrasounds take 45 minutes to complete?

+Alicia Ramirez Depends on who's performing the exam. If they have a lot of experience then they will be faster. The technician also has to look at every thing in the pelvis like the ovaries, overall anatomy, and take measurements. I have never personally experienced a transvaginal US that lasted that long.

Jenny Le Thank you for the reply! My physician put in an order for me and I was told that they reserve 45 mins for every appointment because it could take that long. Hopefully it doesn't!

+Alicia Ramirez usually they have to check you in, set up, and explain the procedure so that always add in a 10 minute buffer.

+Adeeba Moam OBGYN stands for obstetrics and gynecology. Obstetrics for delivering babies, gynecology for women's health when it comes to their reproductive organs.

why wont a obgyn get paid unless they ''deliver the baby''? So no matter how much prental care and antepartum care the mom recieves, the doctor doesnt get paid unless the doctor delivers the baby? what kinda rule/law is that? Never heard of it
