As a former EMS worker, we have known of NY ambulance issues for decades. Nothing new here, except higher call volume. There's continually too many people living there, since the 1920s. Buildings too tall, too much corruption, and the list goes on.

I was visiting NYC and saw an ambulance with lights and siren going and no one even tried to move over. It was around Times Square. I live in British Columbia where we do move over when we can. I felt bad for the patient inside.

Many of these places send Police ahead of the Rescue Ambulances so the selfish ones will get out of the way. It's a failure of our education system.

A solution from rural Germany, which are sometimes too far away from Hospitals: There is a local EMS worker in a private car on stand by (can pop up a light on his roof), they will drive to the emergency quickly. They make basic evaluation of the patient and have everything ready for when the real ambulance arrives.

that's not a solution, that's a damn poor try to compensate the lack of EMS coverage. Should be at least some municipal or volunteer service backed up first responder system, not EMS workers running their POV with pop up lights. ALso that's not an option for a place like NYC. They have more people working in a single high-rise than there are residents in an entire village in rural Germany

One could argue, for high rise, that there should be a trained person in first aid (deeper than just the basics!) in every building with XXX amount of people or more. At least the first responder would be quick on the scene and the Ambulance coming a little bit later is than not such a big issue. Also: The bicycle lanes they shown: They should have something like an entrance that an Ambulance (bull bar? Retractable (by remote) bollards) could be getting through and use the space there to find a space to stop. Even better: Make the bike lanes wide enough for an ambulance to get trough and it just so might offer a shortcut on congested roads?

@@marwerno there are often trained people for first aid, fire alarms and which take care of the evacuation, they totally have that in their mind; However a first aid course some time ago and no medical equipment both won't really help in actual emergencies. Not sure where in Germany that's supposed to be at, but it's not good, it really isn't. Apart from that EMS has to meet its response time limits to a certain percentage and the early provided help of first responders cannot be counted in that statistic as they just aren't actual EMS The bike lanes can easily be used by emergency vehicles in NYC, at least police cars and ambulances do so on quite a regular basis. Some examples he showed was like a diverted bike lane around a construction zone and ones which were narrower for the same reason - however later in the video you also see perfectly to use bike lanes, also with an ambulance making use of it

@@EnjoyFirefighting The region I am talking about is only 40km away from Stuttgart... so a wealthy region. But just in 20km surrounds, they closed 2 Hospitals and "Specialized" others (I could have not driven my kids to the nearest one in emergency...). During Covid, they found this to be really bad, but still they continue this road. At least now they convert the old Hospitals (well the one I know about) not to building land, but to doctors centres, where you can find somebody to help you during normal business hours. Smaller Hospitals did contain the outbreaks much better. They are "saving" (money, not lives) this way in the health system over here (which is sort of free for most people). The thing to keep in mind for the local EMS in the private car is: This is usually a trained nurse, former hospital worker, doctor or fireman with more extensive training than just First Aid. I.e. I was needing a compulsory First Aid training for car licence, but I decided to go for the comprehensive training, that was also attended by the fireman for a refresher. We did (from memory, it was a long time ago..) something like 8 times 4 hours for the whole course including some simulated motorcycle accident with fake blood and even "broken bone" sticking out where we had to recover the driver of the accident car and make sure we handle the motorbike rider correctly. Was cool to do as a 17 year old 🙂. The red cross that did this training, did really well (consider I still remember and this was 33 years ago). The first Aid training I did at my company for the first responder 2 years ago, which I signed up for, was just one afternoon of 3 hours... quite a difference.

They they might be trained, but without adequate equipment it's not much worth. A cardiologist on a plane can still barely help a patient with cardiac issues, despite all his knowledge.

If they were to start paying EMTs more than two dollars above minimum wage, and actually FAIRLY compensated them for their work, we wouldn’t be having this issue. One of the biggest problems with EMS is that nobody wants to do it because it pays so poorly compared to the amount of time, training, cost to enter the field, and physical and emotional toll it takes on you. The average EMT class takes up to four months to complete, and cost anywhere between $1,000 and $3,000 to attend. It’s a couple hundred of hours of class work, practical skills, an ambulance, and clinical rotations. Plus after all of that, you still need to complete and pass the NREMT, which is not an easy exam by any means. And after all that, a brand new EMT will likely only see an hourly paycheck that is between $2 and $3 above minimum wage. If the state were to sponsor EMT programs to remove the cost burden to become an EMT, and if they were to raise the minimum wage for the profession to $30 to $40 an hour to fairly compensate EMTs for their training and skills, you would see this issue very quickly disappear.

I turned down a job in downtown Philadelphia because it’s miserable to drive there. Just as Cash says, with delivery vehicles parking along the streets, traffic is snarled. And Philadelphia wasn’t taking car lanes out of use to make pedestrian and bike lanes, further narrowing the roads.

To put into perspective how ridiculous this is, 6000+ calls was something we experienced during COVID. We experienced this pretty much everyday in 2024 4500 calls would be slow day in 2024 it’s closer to 6000 a day. It’s insane to get sent to a high priority job that’s 40 minutes away on the other side of the Borough.

@ we have 8 hour shifts and 12 hour shifts, if we rushed our calls we could realistically do a call every hour (from being dispatched to back in service) but anyone who works like that is insane. Most people take their time and do the full call in about an hour and a half. The day is normally a mix of transports and on scene dispositions (Refusals of medical aid, non EMS related 911 calls, unfounded patients etc…) if I were to say an average day in 2024 is probably 10-14 calls you’re dispatched a day with 60% transports and 40% of the other things I mentioned. In terms of how long we’re in service without calls I’d say if you don’t get an assignment the second you go in service you’ll probably get hit within 10 minutes of being in service. Hope that answers your question.

@@bigmood8370 I live and work right outside our state capital city, covering anything from the outskirts to extremly rural areas. Normal call takes us 1.5 to 2 hrs to respond to the scene, treat the patient but not spending too much time on scene, transport him to the hospital and head back to the station. Also because it's at least 25 miles for us to go to the hospital and back to the station. We still have 12 hour shifts on our emergency doctor unit, but we had to go from 12 to 8 hrs shifts on the ALS Emergency Ambulance due to the call volume Sure, same here ... mix of transports, on scene assistance, refusals, calls which turn out not to be an emergency and none of our business, to canceled calls. We do an average of maybe 5 calls in 8 hrs on a normal to busy day, but that's due to the rather rural setting. Back when I was a trainee in the city and had 12 hrs shifts we usually did 10 to 12 calls

NYC 911 EMT here, thanks for covering the subject of EMS. My opinion on delayed response time and EMS? -Increased congestion -Crappy road planning -Bad drivers -Increased calls (we do roughly 4500-6000 calls a day) -Dealing with system abusers. Individuals that aren’t having real medical emergencies or calling 911 unnecessarily -understaffed, lots of people steering away from EMS -increased burnout of crews due to various reasons, overworked, UNDERPAID, high turnover rate -Increased violence in the homeless population and shelter system -Politics between FDNY and Voluntary Hospital (look at the history of NYC EMS & FDNY merger, kinda shaped the way EMS is now) I’m glad EMS is getting a little bit of coverage. We need more awareness of how the field really is. We all wish we got paid more and appreciated more. 🤧

- many cities have crazy traffic despite having not nearly as wide streets as NYC has ... just check out cities like Rome or Paris - sure road planning might be poor at some point, but not overall bad in NYC in my opinion - every place has its fair share of bad drivers - the total number of calls doesn't really matter, it would be more interesting to know how many calls a single ambulance does in a shift of ... idk 8 / 9 / 12 / 24 hrs - system abusers and non emergencies are a thing in other cities and even in the countryside as well - understaffed: got to ask the question WHY they're understaffed. It most likely is about work conditions, backup or lack thereof from the station boss in case of personal issues / work situation / shift planning etc - underpaid ... Idk how much you do in a year tbh

I'm a retired N.Y.C. Paramedic and I also worked a great deal with MetroCare out of Jamacia/Brookdale/Flushing. We have had always been short on equipment and competent crews. There are also operational protocols to consider when looking at response time such as certain areas require EMS to wait till PD can come and provide support to the Bus crews. I have not been home to NYC in 16 years so I can imagine how difficult it has gotten for the citizens to get the help they need in a timely fashion and the Medic to have the equipment they need. I remember my fist night as a Medic working in the 188 and Hollis are, ran stabbings, shootings and assaults ALL night not to mention one 19-year-old that was thrown off a rooftop. I really miss it. As a former NYS EMT, I could attest it all began when hospitals began to close, not enough Rigs to cover each borough let alone town so rate of time ppl saw ambulance help increased. Pay cuts and high crime then came along and now, not enough em's.

Cause the Bronx is a shit show. 50% of NYC 2million 911 calls are in the Bronx alone. Also no hospital wants to help the FDNY in the Bronx you never see NYU or Mount Sinai or Presbyterian or Northwell ambulances you only see FDNY why? Cause nobody has insurance. These hospitals want money but they know only poor people live in the Bronx.

I feel sorry for innocent people who have to wait so long for EMS response. But let’s be honest-the People of NYC voted for the policies that are causing these problems. They are just reaping the results of their actions at the polls. Ultimately, until the people of NYC come to their senses, break out of their Stockholm Syndrome and stup voting for Woke Democrat politicians and policies, I have précisely ZERO sympathy for the People of NYC.

Almost no streets with 3 lanes here and the emergency services still reach their destination. Emergency vehicles are mostly smaller and EMS services are split into ambulances and emergency doctors that arrive in smaller vehicles on demand. Even with gridlock, there is still space to drive to the sides and make a lane in the middle.

Reviving a bunch of drug addicts that do not pay taxes and overuse the services is the real issue. These drug addicts light fires, harm each other and other innocent people. You should analyze the ambulance call types and who paid for the ambulance support. I think you will find very few are emergency calls for taxpayers. Same with fires.

Like any other major city (and small towns all over), NYC EMS is overwhelmed due to excessive inappropriate calls to 911. A sneeze is usually not ambulance-worthy.
