Common advice is to avoid high impact activities like running and jumping as well as prolonged kneeling and crawling after a total knee replacement.
But that advice doesn't always apply to everyone. There are a lot of factors to consider when determining what you are allowed to do after a total knee replacement.

Excellent advise. This information and advice to follow 21 years ago, while recovering in a small country town, 75 km from the city. Don't forget aqua therapy, it's brilliant. You can exercise without weight bearing until your strength returns. Probably through my knee revision, I will be using these videos to help with my recuperation. 21 years ago I did it all on my own, not once but twice.

The main thing to avoid is the clot shot, and do due diligence on picking your Surgeon. 2 knees done in last 26 months. All good. 😊

63, Planning to get back to wading trout streams at 6 plus months, currently at 2.5 months and recovery doing well. Can tell I’m not physically there yet, endurance, plus range of motion, but I’m near expected status and working at it daily. Good to have a goal. Did buy a wading staff if I find poor footing areas.

What's the earliest you've heard a tkr prosthesis wearing out? How many years can you expect with high activity: running, kneeling, climbing (ladders), etc.?

People really need to focus on good proper foot wear and addressing hip weakness / imbalances which is where knee pain / discomfort stems from in most cases. As a personal trainer I recommend useful and extremely beneficial strengthening exercise to my clients, friends and family, 99.9% never perform them and allow their knee issues to progress!!

Kneeling on my replaced knees hurts less than before they were replaced, but it's still super uncomfortable on a hard surface. That seems strange, since it's no longer my knee, but there must be some kind of nerve endings still hanging around on the kneecap. I bought a cushioned garden kneeler for outdoors, and also strap on the gel cushion knee pads to keep things doable.

Thank you so much for all your videos ☺️ extremely helpful. I had TKR on May 3, 2021. About the third week I started have big tor joint pain which I never had before. Feels like I now have a bunion on my surgical knee. Will this resolve once I am walking better. The evening pain is uncomfortable. Thank you.

Had TKR in May 2020. I’m fully recovered for all practical purposes. I’m in mid-sixties. Consultations with doctors excluded high impact sports, however, they cautiously considered Pickleball might be acceptable. It’s a doubles game, played on typical tennis court surfaces and inside surfaces like a community center, etc. The courts are smaller than a tennis court. It’s a racket style sport with lots of movement, some quick like racquetball or badminton, including lateral, but not constant or high impact. I’ve returned to play for a couple of hours 3X per week without any problems at all. I don’t feel I’m over using the TKR in any way that will shorten it’s utility. There are more like me, plus individuals with meniscus problems and other knee issues that play Pickleball recreationally without problem. So, as Tony says it may be a qualified answer to whether you can do some exercise and not necessarily the blanket prohibition. It may depend on the exercise and many factors, such as the stress on the joint plus frequency, intensity and duration.

I just had my TKR in sept. So I can play MORE pickleball and do coaching too. I’m hoping this helps as I totally love the game and played it probably more than my body wanted me too! Lol

As a member of the Islamic faith, kneeling (or prostration) is an essential component of my 5 required daily prayers. I hope to return to the ability to kneel (with forehead on the floor) and sit back on my heels (the greater challenge). I haven't had the knee replacement yet and i should add that my loss of ability to sit back on my heels mysteriously faded away years ago, significantly ahead of my osteoarthritis diagnosis. I am 67. I appreciate your videos. Thank you.

I had Total knee replacement 9 weeks ago as far I was told that the implant is designed for western style living not for sujod although I can make sujod on my forhead on the floor and my knee on a soft cusion but it aches and may be cause failure to the implant

Just to play devils advocate, lets say you quit all of things you like to do in an effort to make your TKR last longer only to die unexpectedly from a health issue? I think many people would like to enjoy what's left of their lives and should balance that in their longevity equation.

I live on a hill and have 34 steps outside my house to get up to the road and of course, down again. There’s lots of steps around the house and in the house too. I’m managing these but what is the maximum weight I should be carrying in this scenario? Two bags of shopping can weigh 15kg. As can a bag of logs or garden materials. I have had sciatica and hip pain since I’ve been lifting things but it is a necessary part of my life. I’m 68 female and had surgery 4 months ago.

Hi, Tony, your videos are so helpful to us! What are your thoughts on a Rebounder (indoor trampoline)? I had an LKR on Oct. 8th and ever since I have had a sharp pain in the bottom of my left heel - in general, both the bottoms of my feet ache on the pads but more so with the left would you know any reasons for this and how to solve it. It doesn't seem to be going away. I'd also like to get your thoughts on shoes... I have a high instep and wondered which types of runners are good for after this kind of surgery? I still do my strengthening exercises 3-4 times a week. Thanks! 😊

Let me record a video on shoes. I love the rebounder as long as you are safe on it. The heel pain is likely due to all the changes your lower body is experiencing. It is common and almost always gets better with time.

@@totaltherapysolutions A video would on shoes would be great! Right now I just do a gentle bounce not leaving mat on the rebounder. I don't know if it tones up up a lot, but bouncing up and down makes my brain feel happy if that makes sense. LOL

Enjoy the practical advice - would like to have more up to date info, as this one is over two years old. I'm at 10 weeks now. My surgeon cleared me for work at 6 weeks, okayed me for light running (treadmill) and kneeling after 8 weeks (using a soft pad). Definitly different feelings in the knee, and as I'm still working on full ROM (bending), I'm limited slightly - HOWEVER, beginning the kneeling helped me a couple weeks ago when I had a flat tire on the freeway in the evening - with no one around to help. Definitly found out where my limitations were. Fortunately it only took about 25 minutes to change the flat, with no ill effects, though now I've got some better kneeling pads which I carry with me everywhere!

I’m 6 weeks out of surgery and returning to moderately light warehouse work with no heavy lifting. Was wondering what do you do for work and was there a lot of painful work days when you returned?

@@mr.c8700 I own a safe & vault company, so most of my work is on the floor, on my knees. I'm at 6-1/2 months now and have very little problems and virtually no pain. I do use rubber padding to kneel on. The biggest problem I have is after being in the car for an extended period. It takes a couple minutes after I get out to get the leg working again, LOL. As long as I keep moving, I'm fine. two days after returning to work, I had a flat tire on my van, on the freeway. With no one there to help, I had to learn what I was capable off. Getting up off the floor was the most challenging. Changed tires and back on the road in 25 minutes. Stationary Bicycle and Ice are your friend. Continue the pain meds as prescribed to help you work through the rehab, but get off them as soon as possible - long term they aren't helpful. Good Luck.

10 weeks apart?!? Wow! I’m 3 weeks post op and doing pretty good. But the soonest I would consider doing the other knee is March or April 2024. I think it’s more mental. Plus my husband needs more time to recover from taking care of me. He lost five pounds that first week. So you are awesome to be able to do that. I tell everyone considering this to get the ice machine. I’m addicted to it. O H - -!

Yes I did mine 8 weeks apart due to my insurance changing next year, and extreme luck that they had a cancellation. My PT said not to tell anyone how my recovery went because it was unusually good progress. I thank my lucky stars, but I also was very consistent with the exercises. Left knee was more problematic ( probably because my right knee was only 8 wks post surgery. Very good results.)

There's a lot of contradiction about kneeling and (de)sensitization. i want to kneel, but I also want to get back skin sensation. I've heard that you can help advance the skin sensation by simulating the numb part (usually lateral) to help the skin nerves regenerate; but also I hear that massaging can damage the nerve regeneration, so don't do it. Of course, kneeling on the knee puts harder pressure than any manual massage would on that same skin. So what to do?? Which information is correct? Do i have to give up kneeling to regain sensation; or give up the idea at getting skin feeling back in order to train my kneeling?

I had a bilateral UKA (cement-less) 6 months ago and my surgeon mentioned a 20yrs. lifetime of the implants if everything goes according to plan. At that time I will be 83 yrs. of age and probably have reduced mobility anyway. Spoke to another doctor recently and he was more thinking of a 12yrs. lifetime for the Oxford type implant. :-( Reason given is that these implants have not been "improved" during the last 15yrs and it's not about the wear-out of the implant itself (the PE plastic...) but the fixture i the bone. A partial (Oxford) type implant is just smaller in size hence the mechanical stress in the junction between the implant and the bone is higher. If so, and if the implant will loosen the next step would be a TKA which I was able to avoid this time. Bottom line: Treat yourself with care and caution, avoid high impact and repetitive stress and enjoy your working implant as long as possible.

TKR right knee 11/18 have gained most of my strength and muscle back spinning, weight training etcetera. However, when in a bent position ROM is inhibited by what seems to be fluid that appears from out of nowhere but only in the bent position. Additionally, over the last couple of months I can feel and see grinding, gristle, clicking and popping on the inside (mcl) area of the knee. It has been suggested that it's probably just scar tissue. But after over 2 years...Can you address? Thanks for your videos. I wished I had found you 2 years ago.

I split firewood by hand with an 8 lb. maul. Approximately 3-4 logger cord per year. Will this repetitive activity shorten the life of my knee replacements? Both have been replaced in the last 10 months.

So im a 25 year old with medial meniscus removal in right knee and lateral in left. I have been told by my doctors i am more than likely to need a knee replacements later on. My surgeon said i can run. My physical therapist helped me in PT with footstrike and many other things to help me to do long fast paced walking with running mixed in. I'm 71.

Excellent. Im a tennis player who play recreational competitive matches. I have been active all my life. Had both my knees done last year. My therapist is fully aware of my intention returning to tennis and said that I can play. We continue to work on hip and leg strength. I will cut down the amount and frequency of matches I will play but with good rest on the knees in between, stretches, foam rolling, continue to do knees therapy and hip strengthening exercises on my own. Its a lot of work to maintain a healthy knee but its all worth it to be able to play the sport that i am passionate about. Im just pretending Im a pro tennis player. They do the same thing day in and day out and then some. I just wish my therapist can be my personal physio.

Of course you can run no one said you could not. What they are saying is that wear and tear will come in faster by doing so in other words your installed equipments will not last longer so it is up to you. For me I am just happy I can walk pain free period.

Tony, I have been diagnosed with a DVT (below knee) after 4 weeks in of my TKR. It has continued to be painful and stiff. What do you recommend?

That is a question for the surgeon. Often they will tell you to avoid certain things. They may have you on medication. Please keep us posted but also don't let your surgeon forget about you.

Tony, thank you so much for your helpful videos. I have had both knees replaced. Right knee replaced twice due to implant sizing issue. I am 53 years old, RN worked 2 jobs my entire career but now I have only one job 👏🏻. F course, I need to continue to work much longer. I am 10 weeks out from last knee replacement doing great but I considered investing in a lifePro rhythm vibration plate machine. Would that be something you would recommend if ok with my surgeon?

Hi I’m a 58 year old UK RM can I possibly ask how you manage CPR I’m going back to work in a few weeks time after TKR and I’m concerned about the kneeling, I can’t walk about with a foam pad incase someone arrests 😊 how did you manage. How inverted do you go? I would not recommend a full inversion after a total knee replacement.

Typically lock my unit out at about 60° . Still gives good decompression without total inversion.

Thanks for your great words, coming as out of your exposure and experience in your field......I am 56 yrs, and .have had my bi lateral TKR on Dec 20th and am into 4th week of recovery...My knee flexion of Rt Knee is 115 and left knee is 105 . My extension of left knee is not that good, howevr my doctor says I will have to.go through the grind to achieve the desired results...I hope so I achieve that... .I have been an avid golfer and wish to continue playing after 4 to 5 months time ...Can I play golf ...my love of life. Are there Any special exercises recommended for taking up.Golf.. Best Rgds, Singh( India)

I had a knee replacement on April 1st. Is cutting my lawn safe on the knee? I started cutting 2 weeks after surgery and my wife told the Doctor and he said it was safe as long I do not slip. But my brother still does most of the cutting. I just wonder will it will hurt the knee down the road?

@@florencepackiamuthu3456 were you able to find any good videos for bilateral TKR? I was hoping to see some bilateral video recoveries here, but so far I haven’t. I’m getting ready to have bilateral in a couple weeks. A little nervous looking around at the pain of some TKR patients. After my first shots I felt wonderful, it didn’t last long, but my brain & body couldn’t remember the motion of how to sit on the ground like a normal person because I haven’t been able to get to ground for years. The way I try to sit has been trying to turn and lean as low as possible using my arm and then flopping down on my hip which isn’t a good thing. My hip is moderate OA. Both knees are Bone on bone. Your question is very interesting as to how to sit and get up from the ground ,with two, new knees. I hope you are still on the road to recovery.

Thanks for your videos, they are excellent. I had TKR on 12/01/2020 and am really excited to get back to playing tennis (doubles). What are your thoughts? I’m nervous about damaging the device. I’m in my late 40’s and want it to last!

Wow, you are so young. Check out the video I posted today... about 20-minutes ago. We discuss different implants. Only you can make that decision but I can tell you that others are doing it.

Ok my orthopedic surgeon is not as informative and helpful as he is into rushing you in and out of your fourth week assessment after surgery. He may be a great surgeon but a total failure in communication. Before and after surgery. I have a 3D replacement but as far as why it is better or anything he never discussed it. I have only watched podcast plus I have type 2 diabetes for 20 plus years. When I had my operation my A1C was 6.6. After surgery I could not exercise to keep my blood sugar down and I hated that. I was very active. Gardening with no tiller only tools. Plus tons of clearing of brush line on the backyard and built a patio after a major clearing. So I was active. I had hardly any pain in my left knee but it was grossly curved to the outside with a bone protruding more as time went by. Leg was shorter and causing gait and balance problems snd hip and back as well. Plus that same knee had three major problems. First the Patella tendon tore off when I pushed off to dunk a basketball at the age of 40. I played basketball at a very high level got a long time. I knew my tendon mist likely had micro tears snd then more. It took me two years before I could actually run again but everything healed . Oh I was in the Army National Guard and also had been in earlier in my career. After healing up I went to ranger school and it was fine. Made it through. Also had on the same knee arthroscopic surgery on medial side and it was awesome. Also cut across my left knee superficially with a chain saw accident which was nasty but did not hit the knee cap. So you know my history and I was always in fantastic shape and even a power lifter as well way into my 50’s. Anyway today I need to know about my 3D replacement surgery and complications. Today is my fifth week and going be evaluated by physical therapist group 8-10-21. They have it all. The main problem do far is sleep, pain 5-7 at 2-3 am. I only take a pain pill when I go to bed. Every day on the OptiFlex-K1 when I wake up then the KINEX coolant machine. The wait a while to do all your exercises. So explain my 3D . Plus are they recalling them and how many different types are there. Please get back to me some how. Thanks

I just had my knee replaced in June and have been able to get back to weightlifting and using a stationary bike, elliptical, and walking on a treadmill. My question is as my strength has come back in my legs I have continued to gradually increase the weight on my exercises. Any concerns with me continuing with this as my strength increases?

Nope, it sounds like you have a solid history of training so you know what normal soreness is. Keep progressing slow and steady. Good for you!

@@JT-gq8wv My Surgeon as well as the therapist I worked with said it was ok so my plan was to just slowly increase the workload but I really haven’t much since the last post. I still have some discomfort, not acute pain, most of the time so I don’t take any chances with it. I was 52 last year when it was replaced so I would like it to last as long as long as possible. I stick to some lightweight weightlifting for my legs, cycling, treadmill, and hiking. I avoid anything with impact as Much as possible. All in all I’m pleased with the outcome, as it was I was in misery with bone on bone. I would have opted for amputation if replacement was off the table.

@@samuelfink4799 My situation parallels yours. Because I chose my parents carefully, I am a hyper-active 69 y.o. guy. Like you, the knee replacement helped the bone on bone pain, and that has tremendous value. Good to know what others are experiencing. Thanks for your feedback.

6 weeks is really early. Many of my clients are still using a cane and some a walker. Sounds like you are doing really great. Downhill puts a lot of shear force through the knee. A walking pole might help if you have one, but with time that will absolutely get better.

We usually have patients on the elliptical before we even do the bike. Bilateral knee replacements and singles. I find the elliptical is often easier because it involves less knee flexion than the bike. We usually start week 1 post op.

You know I was watching you for about 9 months finally got the nerve to ask the question about my knees being replaced and using the elliptical and I can’t thank you enough with out you knowing you have helped me succeed I’m now walking so much better you explain so much how I was feeling and why I’m walking heal in first and pushing off with my toes and I don’t have stiff knees nearly as much I still rock a bit but I’m getting there I watch my self walk on my out door cameras and listen too you very carefully and work it from there too all the others out there that are just reading ask away he got back to me in 2 days it really meant a lot that he or they responded thankful Canadian 😔🇨🇦

9 months a tkr, rode 3 wheeled bike 1st time for an hour. After, knee felt like a clamp is around it, some clicking is this normal?

Yep. Totally normal. The clicking is likely due to increased swelling. An hour is obviously a lot for the first time. You will recovery in a couple days and maybe next time start with 15-minutes.

18 days post double knee transplant. I can walk unaided pretty well, slowly but the speed's picking up. I, too, have noticed a clicking in the right knee, no pain or discomfort but I can feel it. After watching several of the videos I now realize this is very common.

A meaningful recovery plan should include a sportive mindset. Keeping your mind active, and focused on your future, will diminish the focusing on the present pain and discomfort. Recovery is a team effort and you are a player on the team. Act like it. I’ve yet to find a video on mindset. Where are they?

Had a TKR six months ago. Resumed running a month ago. Other things hamper my running more than replacement. Ortho surgeon said running is bad. Running will wear out your new knee faster. Does that mean don't run? We bargained. How about running a marathon! Answer No! Half marathon? No. 10 k? Well running will wear it out faster. 5 k? Yes, but it will wear out faster. Can you give me a time say 10 years if I run a lot verses 25 if I don't run? Running will wear it out faster. Ok, I just want to run a 5 k fast enough to beat other 70 year olds in a sprint triathlon with a strong swim and bike!

I had a tkr 6 weeks ago I was healing well down to one cane then I went to physio and pedelled a stationary bike for 10 minutes now I can't straighten it at all I can't bend it like I did and it is very swollen and painful . what should I do

As long as you were never told not to. Some patients have knee stability issues that limit their ability to cross legs.

Yes i agree. I just asked my doc about the cementless replacement and he said if i transition to total he would prob put a cementless in my knee.

@@totaltherapysolutions Cement less = bone grows into the porous ends of the prosthesis = take longer time to heal = newer so no data on longevity of device. Cement = older technology - lasts 10-20 years. Used if Osteoporosis. The cement can deteriorate and cause tissue damage + knee swells (inflammation) trying to get rid of the debris.
