It looks like multi color mode with 4 shades of gray. Larger sprites ( cars and scenery) seem to set a color. The street is only possible in graphics mode, but I do wonder if the sky is text mode? So only one color attribute to set. For small cars without their own color: vertical pixel precise movement is not so expensive. Even scale down may be possible. Horizontal movement is probably in 8px steps. I Wonder if the larger cars with their color could be updated at a higher framerate. I should check the C64 version. On plus4 the exact frame when a car should move by 8px could be calculated. So all other stuff is drawn in the back buffer, but up to two cars are added on the fly in the front buffer with backup of the overwritten background into a third buffer? Event based redraw.

Check the game preview CarTED by Mad, it might give you a better idea of what could be possible from a driving game on Plus/4. It's a bit like Lotus 2.
