The ashes from the heart attack restaurant weren't of an employee, they were of a friend of the crazy doctor guy who owns the restaurant. The guy died of a heart attack or something related to him being super fat, so that whole thing was basically the villain backstory that gave birth to this fucked up restaurant idea and it's why he designed the restaurant to be some kind of cautionary tale on fast food to the extreme. the doctor guy legit acts like one of those movie villains who think they're in the right even though their plan doesn't make much sense upon critical inspection like Thanos.
