Thrombi can indeed form spontaneously (for example due to 'bad genes' - also known as a 'genetic predisposition'), but they are often influenced by underlying conditions such as atherosclerosis, hypertension, hypercholesterolemia, obesitas, and diabetes. Hope this helps! :-)
