First time I died was to Muffet, and since I've played the game through a few times, I can tell you this is consistently true. Probably the second hardest fight in the game. Luckily it doesn't last very long.

I think my first ever death was against Papyrus, but my friends say that doesn't count because he only captures you and you can continue on. Aside from that, my first ever death was against Woshua and Aaron combined. I just wanted to get back to/from (I don't remember which) Temmie Village and then they suffocate me with soap and sweat >.> But technically Papyrus was the one. He was the first to bring my HP to 0, so I think it counts.
