The rich man went through an extreme surgery to look like his servant and the servant also had a surgery to look like the rich man. The beggar's spirit got them mixed up and ended up killing the servant instead of the rich guy. Now both the beggar's spirit and the servant's spirit want vengeance.

That's just what it's called. When Araki wrote those one-shots, he gave them random episode numbers, probably to make it seem like it is just one of many stories that Rohan has heard over the years. "Episode 16: At a Confessional" is actually the first One-Shot in the series, for example.
