I'd assess the tibial internal rotation range of motion like I showed in the video.

Is it ok to start this exercise with the foot in turn-out? - some of my clients can't get into this start position easily

Sure as long as they don't have any knee pain with the movement and you progressively move the foot inward. Doing some isolated tibial internal rotation work (like tibial CARS) or release work around the calf and shin can also be helpful.

I have had this exact issue since I fell in 2015. NO ONE, not a single MD or PT has figured it out. Thank you! May I ask, I also feel like my ankle ligaments are loose since I've been dealing with this for so long. Is there any hope of fixing that sensation? How many reps of the lunge should we be doing?
