Excellent review! I caught this episode last week after not watching ”Grey’s Anatomy” for years, and this happens to be an episode you recently reviewed! Still can’t believe the coincidence. Even with my limited knowledge, I knew the neuro consult was such a stretch. If I remember correctly, Meredith first met Derek in that same bar, and they were having a “will they or won’t they” situation going on during this season. So bringing Derek into the mix, definitely entertaining for the drama aspect, but not realistic medically. Nor was the way the residents spoke to the patient. Poor guy! The lack of a urology consult also irked me. Thank you for breaking down how a real case of priapism typically goes. Can’t wait to watch more of your videos!

the urologist of this show kinda looks like an older version of you actually! she firstly appears on episodes 8x05 (Love, Loss and Legacy), 8x17 (One Step Too Far), 9x04 (I Saw Her Standing There) and 11x19 (Crazy Love). You definitely need to react to these!!! PLEASE!!!!

This is very interesting. my late elderly dad had an ischemic priapism event and was in extreme pain. An Ambulance took him to the hospital. At the hospital everything medically the happens too fast for me to understand completely. Nothing funny about it as this episode of grey's makes it seem. Also my Dad was in so much pain he could not carry on a conversation. Sadly his was due to cancer. Your reaction video was very educational and interesting. Thank for doing this.

Just a note - bailey's bedside manner is *because* she keeps having (at that point) to intervene or deal with meredith's sex life & meredith circumvented normal procedure (triage, er) to have bailey check him out as a favor to try to keep the whole incident on the downlow so derik wouldnt find out - so there's that. Her character is blunt but usually has good bedside manner - this one was more dramatic effect for comedy for the show. One of those times reactions go a bit awry for lack of context lol otherwise good

Yep, because is costumarry in this kind of series to make fun of male patients. So still pretty bad. If we take into account that men are less likely to seek medical care is not convenient to teach boys and young men that the ER is a place were women mistreat and laught at patients.

I love how you take dramatic and fun shows but break it down and explain so much. You are extremely informative. We all know these shows are “fake” they have to actually make people watch them but it’s nice to hear what doctors actually would do. I love this!! ♥️♥️♥️♥️

I'm very sad that the problem was treated so lightly and as a joke. Shouldn't that be a big red flag? What about people who have had this problem before and are watching the episode? Or maybe someone who has the same problem in the future but is too ashamed to look for help?

@Jenn That's very reassuring that you realize that.. This entire channel thrives off this TV show basically laughing at Men's urologic conditions and Rena "explaining" them.. There is NO way that would happen if the genders were reversed, with a TV show laughing at Female patients, being attended to, and made fun of my male attendants.. Sad Rena doesn't see that - Especially because she's actually TREATING males..! Our Fathers, brothers, sons, friends, etc.. Sad..

I realize that I am commenting several months after this video was posted; however, I only learned of your commentaries a couple of weeks ago and have been going through them as they are both well done and interesting to boot! This episode does compel me to ask if the American Urological Association or the Women’s Urological Association has written to the producers of this (and other shows cut from the same cloth) as it seems that the male patients are presented in a (comical) urological predicament and being predominantly tended to by female care givers who coerce, bully and intimidate the patients into evaluation and/or treatments. As many viewers do not distinguish between fiction and non-fiction storylines, this approach creates a negative stereotype of both patient and physician. The continuation of such presentations seems to reinforce a narrative that men should not seek medical attention and if they do, avoid women care givers. I realize that you do clearly and unequivocally set the record straight in your review and comments, but the show itself reaches millions and plays into the stereotypes fashioned overwhelming your thoughts and positions. Just wondering if the Associations larger megaphone has said anything on this issue. My thanks and keep up the great videos!

@@mikebressler8951 I appreciate your comments and do not disagree; but note my inquiry is not whether Dr. Malik would address these points but whether or not the American Urological Association or Women's Urological Association has adopted any position regarding these story lines or would be doing so. An individual may rely upon empirical impressions and offer an opinion; however, an association such as the AUA or WUA when adopting a position gains credibility if based upon studies conducted independently of the association offering it. I think both men and women are presented more for what passes for entertainment with little thought given to the real life ramifications inaccurate portrayals of each leave in their wake. Relying on such shows, men do not seek medical help from capable providers willing to respect their modesty and individual needs because they operate under the impression of fear of being ridiculed and demeaned by those very providers. Becoming a urologist means a 4 year medical degree and 5-6 years additional advanced training to become board certified. If any specialty is aware of opposite gender exam dynamics it would be Urology----but given the show's portrays --- who would know this to be the reality?

I’m a retired Firefighter/Paramedic, I responded to two cases of Falls, one from a Bridge deck 18’ to muddy ground and 14’ to Concrete that resulted in a host of injuries that included Priapism. Both patient had Spinal Injuries that, thankfully, later resolved on its own.

2:38 The very fact that we call it "performance" is fucked up. "Performance issues". It's expected of a man to "perform". If you don't take yourself out of that mental state that society puts on you, you'll end up obsessing over it and maybe even precipitating it. Sex is fun. It's communication between people. No one has to perform, you just enjoy each other and slow down if you get overexcited. If you're not performing in conversation, you don't need to perform in bed. You're just talking. With your bodies.

Yes. I feel like I am just a machine to perform. I always avoid sex because of this. I have to have big penis, and I have to perform for long hours. The first thing is just not in our control and this is also body shaming. I found myself planning to have penis enlargement surgery or mast cell injection. I still hope one day these two medical Technics will be mastered and safe

Having only watched this video and Dr. Malik's video about the broken penis episode of the same tv show, it occurs to me that serious health problems that are specific to men is considered humorous to the people who make that show. I've never watched that show so I don't know more about it than these two videos relate. Reminds me of the ladies on the View laughing about a man getting his penis cut off. Is that funny? Is it because tv shows don't think men's lives and health matters?

Grumpy Wx weirdly I had wild fluctuations due to a blood thinner because a partial lumpectomy from a motorcycle accident. It was terrible but in my case easily manageable. It was mostly intermittent and at worst uncomfortable but at best...... well I did take some advantage but I wouldn’t want to repeat it.

I laughed when you mentioned Trazodone can cause it. I knew I was familiar with what priapism is, but I couldn't remember why, but you reminded me! My dog had to be on Trazodone, so one of the side effects we needed to watch for, since he's a male (and especially because he was still intact at the time), was priapism. Dogs tend to be on a much larger dose than humans, because they metabolize it differently, but there haven't been a ton of studies on how common priapism is as a side effect in dogs. It's basically a case of "Well, it can happen in humans, so keep a close watch on your intact male dogs, just in case." So for 3 straight months (x2 = 6 months... 2 different surgeries, and it's used to aid crate confinement during recovery) I had to constantly make sure that whenever he "lipstick"ed that it would get re-sheathed without issues.

Hi I’m new to your channel. This show drives me nuts. I’m an RN and used to work in the hospital and it’s so not like this. I’m going to find other videos of yours and watch them. I have Interstitial Cystitis, diagnosed in 1996. What kinds of things do you do for that? I’ve about done it all. I know everyone is different just curious. It’s a awful and painful disease. OK. Funny story. I had a girlfriend in college. Total genius, and my best friend. The thing is, I was a closeted gay guy at the time. (I'm still gay. Always have been. It's just that the closeted thing changed.) As it turns out, one of the reasons we got along so well is that she was a closeted lesbian and so neither one of us wanted to have sex. At least not with each other. So, I go off to law school, she goes off to medical school. We got together for lunch a few years later and came out to each other. She told me that, on her first day in Anatomy class, she and her lab partners, all guys, were told to transect the penis of the cadaver they were working with. One guy fainted, the other three got nauseated, and she said, with a smile on her face and in her brightest and happiest voice, "I'll do it!" Which she did, and two of the nauseated guys ran out of the room. She's now a tenured professor at an Ivy League medical school. Her specialty? Psychiatry. (I do hope that made you laugh. Lord knows I did. LOL!)

These were the earlier seasons of Grey's, which aren't as accurate as the newer ones. This was also suppost to be a funny episode and eveything was played on and over exaggerated for the humor and storyline. But, I enjoy hearing your input and talking about actual correct urology treatments and steps with these kinds of issues. ❤

Couldn't you also use phenylephrine injection for ischemic priapism? When would that be done in the management of this?

Yes you can if aspiration and irrigation doesn't work you can then inject phenylephrine.

@@jessicasuchy7850 I think it's tough because the symptoms can be so variable for different people and fertility is so hard to handle emotionally. I'm not an expert in these areas so I would be only able to tell you what's in the literature or textbooks

I very rarely watch TV except for the news and from the rare glimpses you show us of programmes such as Grey's Anatomy, thank heaven I am not that desperate for entertainment. I am of the opinion that most of the programmes shown on TV do not warrant the paying of licensing or viewing fees.

Only time I saw a Priapism was when I was a medic in the Australian Army. Following an explosion the casualty had spinal shock. He asked me "Is everything there doc?" and I replied "Well it's certainly telling us It's still there" It was turgid semi erect which really had me worried, 30 minutes later the Surgeons and Resus was on the case. Apparently from talking to one of the Nurses she said he had a wicked sense of humour about it, saying when he had his uniformed cut off "For the record Ma'am, it's cold in here" 😂before getting prepped for surgery. He did recover so "Happy ending" 😐 I guess.

so if i understood correctly, if you do not have any illness and do not overuse any ED drug .... your chances of getting priapism are very low to non existant. Right ?

I remember the first time I learned about priapism. My professor went on about how the blood flowing through the penis can start rotting and then it gangrenes or the already poisoned blood could reenter the bloodstream and becomes lethal... Anyhow, sorry for the picture but needless to say I hope I never end up going through something like that.

This is just a show. Of course it's not real. It's a comedy-drama. Im an ICU RN and I find that nit picking on all the incorrectness of the show is neither wanted or needed. Im sure the writers took liberties. If you're a real doctor, what a waste of your time

These shows never make any kind of medical sense. I’ve never even been to med school and I know it’s bullshit. It’s hilarious. My mom is a medical coder and she can’t watch medical shows anymore because they’re ridiculous😂

Why isn't a phenylephrine injection used first for priapisms instead of the more invasive procedure of draining the blood? Can't the needle aspiration cause ED as well?

The treatments do not cause ed its the prolonged erection that causes ED. Phenylephrine requires monitoring and has the potential for dangerous side effects.

I'm really glad you call out the unprofessional comments on this show the African American doctor on this show reminds me of someone I worked with who acted this way and I had a horrible time with her and one time I was told by her this was how African American women are supposed to be. I disagree her character on this show is poor example of how doctors should behave.

I appreciate Dr. Malik's commenting on this episode / topic. But the show's handling of the medical issue as a punchline is insulting. Hollywood needs to produce WAY better TV than this crap. It's so ridiculous what passes for "entertainment" in out culture/country these days. :-(

So four hours without blood flow is bad. I would have thought there would be issues with ischemic priapism long before that. How is the tissue able to last so long without oxygenated blood being supplied?

Typically the damage done before 12 hours is minor with little damage to smooth muscle. After that ,the cells start to show damage and after 24 hours the risk of irreversible damage increases dramatically.

Women laugh at men with health problems, especially if he's suffering erectile dysfunction , I know I kinda suffer from it and im 34,, men feel ashamed/humiliated and hate themselves and don't want to live anymore, that's how I feel nearly every day, i feel uncomfortable to get it treated,but it's okay fr women to laugh and mock at men's health problem, imagine if a man was laughing n joking at women's health problems, plus women can't b trusted, they love to talk, backbiting and gossip, just like the guy said don't say anything, but she's here talking, this is why men don't open up to anyone especially women.

I know somebody who had an erection that lasted 3 days (a friend husband). He also was starting to get dementia at the time so never mentioned how long it lasted, his wife just thought that he was up and ready each time separately they had sex. It eventually started to cause pain and his wife rushed him to the ER and they had to do emergency surgery right there in the ER. It turned out to be a blood clot and he did lose sexual function with his penis.

Honestly it was kind of a hidden blessing considering that as his dementia progressed he became more crude with sexual remarks to any woman he had contact with. (he's not dead just in a full time care facility now)
