Hey team! In this video I react to an episode of Doctor Jeff (Rocky Mountain Vet) where a paralyzed dog is seen and medically managed by Dr. Jeff's associate Doctor Hutcheson. I thought she did a really good job explaining everything to these distraught owners and wanted to go over some of the finer details of paralysis in pets. Have you watched his video about the veterinary shortage crisis? We can't perform major medical procedures on pets for "cheap," and expect to pay staff living wages, and offer health insurance, and other standard benefits that other jobs easily offer. If they can make more money almost anywhere else with better benefits and less physical, emotional, and mental stress they will eventually all leave the field (as we see happening). And clients saying things like you are saying about "being cheaper" like Doc so and so (Dr Jeff gets his money from the show!) further worsens the situation as employees feel guilty for wanting to be paid (which is why EVERYONE works).

@@AnnmarieHillDVM The show didn't keep him in business before the show came along and it has been a long time since any new shows have come out and he is still going and expanding his clinics in other parts of the world.

​@@AnnmarieHillDVMHave YOU watched the news lately?? Have you seen the cost of living and the inflation problem?? I understand the vet shortage and people want to be paid. I get it. But when vets basically show poor people the door because they can't pay reflects badly on vets. A lot of vets won't even allow clients to pay over time and those clients can't get Care Credit or afford pet insurance. So people are supposed to let their loved pets die because they can't pay you???? Are you bitching because of the vet shortage or are you bitching because vets like Dr. Jeff are making you look bad?? He has been in business long before he got a show. He has been philanthropic for decades so if you're going to complain about vets helping out people when you won't could you at least know the situation before you speak?

​@AnnmarieHillDVM because Dr Jeff is so fair in what he charges, then that's why he has more patients than any other vets. Dr Jeff will always have thriving businesses because hes fair with what he charges, and he is the best of the best. Everyone loves him and people will always be loyal to him

Almost every time I watched Dr. Jeff's show, there was at least one person in the waiting room saying that they came to Dr. Jeff because the couldn't afford what the other vet quoted them. Some drove hours. I really love Dr. Jeff not for only helping the animals, but also being so mindful of the fact that most of us can't afford thousands of dollars for some procedures. Oh, and for going to poor/rural communities as well as to places in Mexico to spay/neuter, and just offer general vet care.

In my vet career, part of my duty was to listen to and respect my clients and their life situation. Not everyone can afford pet insurance or a $10k bill which often comes with no guarantee that things will go well. Showing compassion, thinking creatively to come up with ways to treat within the limits of a family’s reality-that part does not fit for a corporate run practice. Being a vet and not having “permission” to think outside the box, needing to comply with corporate rules, would be horrible.

Years ago now, I had a dog with a hard lump midway on her femur. Not lame, no pain. She was over 12 years old. The vet took her in the back, came back and said they'd done a biopsy on a bit of the lump. It was cancer. They'd keep her and completely remove her hind leg. I said no. At that age [age does matter] How would she cope with walking. With the lump she was very active, running, turning, being chased [ she was much faster] than my jack russell, who was no slowcoach either. Latter I actually met her original owners, they told me she was kicked by a horse and BROKE that back leg, the lump was calcification [no idea who or how treated her, just put it in plaster maybe?] Did these vets think they could fleese me, Biopsy? The lump never changed size, She lived to be at least 19 years old, I don't know how old she was when I got her. I had her 19 years.

I had a vet that tried to rip me off several times. I haven't been to him in years and I'll never go back to him. He's awful. I had another vet and he was the same, awful. I won't tolerate any type of abuse from any vets. I currently have a wonderful vet

I love what they fo at their clinic. Pets play such an important role in our lives, but the cost for care has become more than many can handle. To know that there are vets who make vet care possible for those who struggle to afford it is wonderful. I dont brgrudge vets money, but corporate buyouts are putting vet care in line with corporate human health care. It becomes a privilege, which puts it out of reach for many. You mentioned pet insurance would reimburse, many dont have the cash to lay out to be reimbursed. Its a sad state of affairs.

I would love to have a doctor Jeff on this sides of the states in New England. Well me and my mom did in my old state. He helped us with the payment and if the bill was way over. He would make sure to help bring it to where we could pay it. He needs to have a class for Vets now and furtue Vets how to treat the animals and their owners. We had one vet who would should of sued. He treated our poor girl like crap and us. She was having serious seizer. They told us we couldn't be seen until 3 in the afternoon. It was 8 in the morning. I called my husband back and told him there getting worse. He came flying home we got in his truck and took her there. The woman behind the desk told us she couldn't be seen until 3. My husband said ,this is a emergency. Everyone in the waiting room told her take the poor dog now. Because she was having another one and it was a really bad one. He came back in the room not even in a a nice voice. Like we bothered him. He goes will have to put her down. My husband goes may e if you took her earlier she would be good . The vet had nothing to say,he just goes what do you want to do. My husband goes we want to see her do we can say goodbye. The vet goes why because she's our dog. Well I already put her down. I blow up and I told him who just asked us what did we want to do and you already put her down with talking to us. We made sure to let others know . He doesn't care about your animals and can't be trusted. A few people in the waiting room told the woman behind the desk. We want our animals files now. We're leaving and not coming back. We heard others put the word out . One went all the way to our state board.

We had this happen to our part corgi (long backed) and did 6 weeks crate rest with steroids & pain management. She improved slowly but steadily and was walking on her own in 4 weeks. After 6 weeks she was given acupunture twice a week for 6 weeks and did really well. She lived 7 years after this injury and was happy and pain free. Best dog ever@

Love the extra info on the case. And yes, yes, yes to Pet insurance! Got our girls started on it earlier this year and I've already hit our deductible on the 5 year old. Totally worth it for accidents and illnesses. What about Bondi Vet in Australia? They have clinic, hospital and farm episodes, should be a good one to do. Keep it up! 👍

Thank you for posting this! Yogy (a beagle, of course) was deep pain negative and incontinent. He started walking with assistance just a few days after his hemilaminectomy. Now he runs and jumps and does everything a normal dog does. He has delayed proprioception and a wonky gate (sometimes he walks sideways 🤦🏻‍♀️) but I will take it. It’s all because of an amazing veterinary neurologist/neurosurgeon like you. Thank you for all you do, you guys are heroes ❤️

I used to live just north of Denver and Planned Pethood was my vet. My 2 dogs and a kitty got excellent care there. I never met with Dr Jeff but his team was wonderful.

I had insurance for my dog when he was young but the older he got, the more expensive it got, until I could no longer afford it. So far, so good. The dog is 13-1/2 and so far healthy except for some skin issues.

I think she said the part about age is because they couldnt afford the surgery and its a easy way to make people not beat themselves up about it.

I wish Dr Jeff was closer to me I’d drive to him in a minute. Living in Florida the drive to Colorado is out of the question for this senior. I paid $5,000 for my cat to be treated for a UTI. Now my senior dog is in bad shape with constant fast labored breathing. He is 13. My vet has seen him several times and all I was told is to give him Benadryl. Well that relaxes him at night but it kills me to see him struggling during the day. I had x rays done and it shows little tumors on his lungs. Guess I need to get a second opinion. I wish more vets had the common decency and values of Dr Jeff. 😊🙏🏼❤️🐕‍🦺

I have used speciality vets but Economic euthanasia is becoming a reality. It seemed like to me the vets knew my dog was going downhill and started chaging me a lot. It had cross my mind that I could no longer afford medical treatment but luckly I didn’t have to make that choice because my dog died June 22. I have decided never to rescue or own another pet because the vet bills these days are outrageous. I am not the only one who feels like this so Dr. Jeff is correct.

I am so sorry you will never have another pet. Isnt a shorter life with you, better than life in a shelter? I have been in the same situation you were. My dog had a stroke last year, I couldn't even afford the 1000$ test. Needed. This girl was a pit bull mutt I rescued ,whose owners were literally taking to a kill shelter the next day. Our shelters are so full of them, I knew she didnt have a shot at a life if I didnt take her in , at age 8 months. I had lots of experience with big dogs, Which you need if you are rescuing a pit bull! Or any large breed dog, Anyway, She had 9 good years with me until the stroke. She brought more to my life in those years than I can ever say! But look I get it . Even fleas and heartworm and food cost money. But I need a pet for me too. I need a good dog. So maybe YOU will be okay without one, I respect your decision However long I can afford to give them, and its a rescue I am going to do what I can with what I can. My Dad made the same decision as you did.

My dog was a rescue dog and had a heart condition. I put in two pacemakers but at the end of his life I feel that the really took economic advantage of me,@@primesspct2

2 vets that I have watched on here for quite awhile are Dr. Jeff & Bondi Vet. If more vets in the United States were like Dr. Jeff, more people could afford to take their pets/animal family members to a vet for things like shots & spay/neuter & even emergencies would go back to being affordable. There's absolutely NO REASON someone's animals should be taken away or euthanized because they can't afford $10 or $20,000 for tests or surgery!! Insurance will reimburse you?? (& what happens when you don't have the huge amount of money to begin with?!?)

Im 72, from FingerLakes, NY. I Could Only Hooe, Wish, Pray I could find a Vet like Dr Jeff💝. Not All about Himself, or Money. But, Care & Compassion, the Animals💝 & their Parents💝. I had 2 Vets like that for 30 Years, but They've retired and Sold Practice. So, Im going to a Vets with my 7 Year old Buddy, Connor💙, about 3-4 Years, Now. I'm not happy there, so I decided to look for Another Vet. Well, I've Discovered that BC of 'that covid', a Lot of Vets Left. 😕 Anyway, I have Called Many in a Wide Area around Me. NONE are taking Anyone New. Wow !!! What Terrible Days 'those people' have brought upon Us and are trying to do Again, with virus, masks & Lockdowns !!! Medical Drs for People the same. Never thought I'd see such Days. Plus, You mentioned Pet Insurance, I've looked into it, Several are approx., $100 per month. A Lot for me. Its just me and my Buddy, if he needed care $$$ And, I didnt have it !!! IDK what I'd do, I Love Him 💙😢 Sooo Very much, it's mostly Just me n Him. My income is limited I don't even have Savings or Any Life Insurance, will leave my Children💝in a mess😢. Thank You for Helping Folks here. I adopted an older tripod pound hound. He had been turned in as a stray. He was missing a rear leg and would drag his remaining rear leg, so I don't know how he would have wandered far. Plus, he was a perfect gentleman so had been well cared for, so someone should have been looking for him. I got him custom Eddie's Wheels. It did take him a bit to adapt to using his cart. Once he figured it out any time he heard me move his cart he would come running (his version!) expecting to go for a walk. He was a certified therapy dog and we did visits to nursing homes and library reading programs. The best visit was with a resident who had recently lost his leg. He was bitter and angry. He asked what was wrong with my dog that he needed a cart. Theres nothing wrong with him, he's just missing a leg. It was a lightbulb moment for the resident. So not really about this case, but about how dogs do well with a cart.

I came very close to getting a tripod rescue hound just before covid when they stopped flying. I watched him on the website and eventually he got to go to England. It warms my heart that yours did so well. Now I'm thinking about a friend for my covid foster fail, and it seems there are several more tripods in the rescue. Perhaps by the end of the summer. . .

Caring for a dachshund that was originally my son's dog. His sister was mine. The male has now had 2 back surgeries. Each time lost use of hind legs. Initially every thing. Diagnosed degenerative disc. Age 4 and 6. Now 8. Pray every day it doesn't happen again. Very costly, plus therapies. He recovered both times. Not able to do steps anymore.

OMG your comment on pet insurance hit hard. When i first got my dog to my care he was already suffering from parvovirus for the good part of two weeks with no treatment and had already dropped 30% of his body weight (18.5 kg - before he was 25 kg).. 6 month old malinois-GSD cross breed, he was not insured, was not vaccinated for anything except for rabies. Immediately had vets look at him, treat and stabalize him. The costs of over night hospitalization were too great for me to bare for more than 3 nights after that had to switch vets, had a 2nd vet offering me to bring him twice a day instead (morning and late evening) for him to get the meds by infusion... that reduced the costs from 600$\day to 200$\day. What saved him and had the most impact on him overcoming this horrible condition was a syringe of plasma... what a difference that made on terms of his apetite and behaviour. In the end it had cost me a little over 3,000$ for the entire ordeal, 6 weeks quarantine, and 6 months of special food was an additional 1000$. Its been almost 2 years since then, he is a big boi now (31 kgs), professionally trained, completely vaccinated and insured... AND the most important he is such a happy dog. Gdamn i luv that dog!

They did follow up and she regained some use of the back legs, owners were using a sling for support when walking

Great video again! Btw, as a neurologist you'd be the perfect person to do a video on post-amputation phantom pain/sensitivity in pets! I'd love to see that! I once had to make a decision on behalf of a pet, and finding out that it has been observed in both cats and dogs were very interesting. Have you ever observed it in your professional life? I'd love to know more about possible cases, treatment and outcomes, I find it very fascinating.

One of my labs made it to 12+ years while having allergies, DCM, TPLO surgery, and tie back surgery for GULP... with no quality of life issues.. He was the perfect patient for his vet/vets which added to his longevity. It was a pleasure to have him as long as I could. I DID get pet insurance for my present 2 labs!! ;)

Dr Pol would be interesting to see you react to. I am former exotic vet tech myself, so I can't claim massive expertise, but my opinion of him has always been somewhat mixed. I realize he's working a very rural and impoverished area, so he tends to go the cheapest route in care. He does plenty of amazing work, but sometimes his really old-fashioned methods stand out to me, and I definitely disagree with him even attempting most exotics, because even to my limited knowledge there, he lacks a lot of basics.

Vet Tech here: If you can afford pet insurance and do your research, than it could be for you. However, pet insurers can be very picky about what they will cover. Also, you have to pay out of pocket and then be reimbursed. Most people I come in contact with at the ER cannot afford $1,000 + let alone $10,000 nowadays. Heck, some even struggle with $300+. Care Credit might be easier for some if they are accepted (doesn't hurt to try, so PLEASE try to enroll if you have an emergency), plus it allows you to make payments, and interest is usually deferred for several months in most cases. Unfortunately, most vet offices don't do private payment plans anymore (and offer outside credit line businesses) because they were burned over and over again by nonpayment and collections.

It has always been obvious to me that Dr Jeff is an exceptional vet who has incredible instinct paired with solid scientific and medical knowledge and skill. I would not seek another opinion, and not sure your reasons for giving critique? Thanks anyway.

We have a Maltese at are clinic who has had two failed IVDD sx on top of a bladder stone surgery. The dog is always happy. O has a cart but said the dog doesn’t always like using it and will just pull herself with the front legs as this way she can get into more places

Pet insurance only works if the owner has the ability to pay $$$$ at the time of treatment and hope they get reimbursed eventually. People with low income or bad credit can't pay up front so pet insurance is useless. I don't think anyone should trust an insurance company to actually pay their share. I have read policies. One had a fee schedule based on the national average because obviously prices have no relation to the cost of living in a region. Other policies say they will cover "reasonable" fees. I have no idea what that means.

Great video Doc! Last course i took which included disc herniation showed alot of promise with surgery and even just using pain-medication and giving time (as long as deep pain is intact). Though the surgeon teaching the course, said that steroids are contra-indicated (even though i was also under the impression that that was the actual conservative treatment). As of now i cannot find any science behind this statement, but what do you think? EDIT: Found it (though this is only about dexamethason) Adverse effects and outcome associated with dexamethasone administration in dogs with acute thoracolumbar intervertebral disk herniation: 161 cases (2000-2006)

I wouldn’t say they’re contraindicated just that the side effects probably outweigh the benefits. Lots of GI issues seem to come up and that creates a whole bunch of problems in paralyzed pets.

MRI is superior in almost every way except for specifically bone imaging. Even then MRI still gets you a reliable answer most of the time.

@@DVMCellini I respect your opinion as a neurologist and am sure you are correct. I've worked (as a tech) in two specialty practices, one that had both MRI and CT, and the other only had CT. The cost difference to clients of MRI vs. CT was significant, so unless the suspected problem was one that could only be diagnosed by MRI, we performed CT most of the time at the first clinic. We could complete a CT in a couple of minutes whereas MRI took an hour or more with the pet on mechanical ventilation. At the second practice, our surgeons were comfortable with the accuracy of CT and did many successful hemis based on CT results. The nearest boarded neurologists are 2-1/2 hrs away, so in our area we have to rely on what we have availble locally most of the time.

Yeah most of the time CT does well enough when it comes to surgical planning for IVDH. Problem is everything outside of a mineralized disc herniation becomes very difficult to image properly.

Yes four to six week…my girl went through a neck injury, somehow . I kind of think the accident happened at the vets practice but they hid it from me. Also months later she was treated for a uti…but another vet in the same practice diagnosed her with incontinence. A few months later I took her off the meds..not incontinence

Dr. Jeff says that you shouldn't go into debt to take care of your pet. My mother almost went into debt when my little cat got bladder gravel from eating junk cat food. She had to ask my dad for half of the money it took to pay for my cat's vet care. It would have been nice to have pet insurance to pay for that stuff.

It's easy to say not to go into debt, but when your pet needs help, you do. I was lucky enough that when I rescued a dog that turned out to have cancer my vet let me pay for the surgery over time at a lower rate than my credit card, but a lot of vets don't do that.

Being that this dog is in Colorado, I would put my money on her being an Australian Shepherd and/or Australian Cattle dog mix. Both breeds are as common in Colorado as Pitbulls are in California.

I enjoy your videos, and I agree with everything you said except the no x-ray advice. I fully understand why you and other vets recommend skipping x-rays and going straight to MRI - it does save money, and if the owner's can afford an MRI and/or surgery then sure just do chest x-rays prior to surgery. However, I think age should play a role in whether to do x-rays before heading to a specialist for MRI or not. This dog is 13, so cancer should definitely have been a concern, and certain cancers can show up on x-rays. I'm only a pet parent, not a DVM or tech, but I recently lived this situation with my 14.5 year old Aussie. She had an acute onset of pelvic paresis, with an occasional front leg limp, my vet thought it was probably a herniated disc and said due to her age no one would probably recommend surgery (despite her being otherwise healthy) and here's where I love your mantra "Age is not a disease."! He also said it could be a tumor, but either way x-rays wouldn't be likely to show anything, so he did steroidal medical management. It worked for about 6 weeks and she was able to walk again- then only on/off for another month. We asked for x-rays and he still said it wouldn't show. After the 3rd month of her sickness I decided to take her to a neurologist because I really wanted a definitive diagnosis, and they had no reservations about doing an MRI and/or surgery if her bloodwork and chest x-rays came back good. Unfortunately, the chest x-rays showed bone cancer in her front right leg and T1 vertebrae (hence the paresis). I can't help but wonder, if our vet had done initial x-rays would he have found the cancer? If we'd found it originally, or even 4 weeks sooner, we could have pursued treatment.. but unfortunately she declined quickly after her neurologist appointment. I have great appreciation and respect for my vet, I understand he's human, I just wish things had been different. Maybe next time he will do x-rays before just starting medicine. Or maybe he will refer to a specialist sooner. Pet parents, always consult your vet, but if you really feel in your gut that you need to do something (like I did with the x-rays; and assuming your vet says it's safe) then push to have it done! I read somewhere, "It is your money. It is your dog. It is your decision." Thanks for the video, Dr. Cellini!

May not have been clear - I’m referring to spinal X-rays not thoracic X-rays to screen for cancer prior to advanced imaging. I always do thoracic rads prior to mri in older pets, but never spinal rads.

I'm sorry, I think it was my comment that wasn't clear. I know that thoracic x-rays are always done prior to MRI. I was talking about diagnostic (is that the right word?) x-rays alone, when MRI is not being considered at all. Also since you mentioned pet insurance, an idea for a future video would be doing an overview of pet insurance, and the top providers. :) unless you have one currently and I missed it? I think a lot of folks don't know how it works, and it could be a good resource.

@@DVMCellini The comment the Dr. made about "A strong course of steroids" I thought you would jump all over. As an ER Veterinarian for 16 yrs I got so tired of treating dogs with severe GI effects caused by high or even moderate dose steroids I could scream. Who else has seen dogs after high dose steroid therapy with perforating ulcers who had severe and even life threatening side effects from steroids. I am not saying she was advocating that aggressive of a steroid regime but I still see it way to often. I also disagree with the no rads policy. If they are not able to pursue a MRI or CT scan then I think rads are warranted You are of course correct that we will not definitively dx IVDD with rads but can rule out other causes. I thought the Veterinarian did a very good job especially on camera and she very likely did a reasonable anti inflammatory steroid regimen.

GI tract moves along without much spinal cord input. The “enteric nervous system” continues to do its job unaffected. But the 💩 will come out irregularly in frequency because they can’t posture to go like they usually would.

In all fairness re. not putting this senior girl through surgery, there may have been other factors not shown in the clip that made her too risky a candidate for anesthesia. Good review though! I miss Dr. Jeff so much, sure wish it hadn't been cancelled.

Another thing to think about if your dog suddenly has mobility issues is Lyme disease. A poultry farmer in Vermont with a KZbin channel has a Marimba guard dog named Toby who one morning was noticeably lame in his back leg. The vet diagnosed him with Lyme disease even though the dog had been fully vaccinated. The farm is Gold Shaw Farm.

in order to be reimbursed you need to have the money to pay in the first place, at 10,000, a large number of people don't have that kind of money laying in their bank account.

Loved it!! I think stepping into the realm of reacting to other vets' TV shows is a tricky thing to do and you did it splendidly! I saw your comment about reacting to Dr Jeff's surgical techniques and I can't wait! I couldn't stand him, not because of his surgical techniques but just his...lack of transparency about the level of care provided, and demonizing of vets that are providing a different level of care and charging appropriately for it. The rest of the TV vets don't seem to do that. Dr Pol may have outdated techniques but it's not like he's out there demonizing other vets. Dr. Oakley is super transparent about the fact that's she's the only vet for (?) miles and she gets expert consults all the time! Thank you again for everything you are doing for the profession!

I love Dr jeffs philosophy BUT in real life, he refuses to treat animals if they are not altered or if the owners wont alter the pet. I've seen it happen and i saw how he treated people that refused to alter their pet. now I believe in altering your pet unless you are a breeder that ethically breeds (what do I mean by that, I mean one who is financially stable enough, one that does OFA checks, and one that breeds for breed betterment) but he was downright awful to 2 pet owners that where seeking parvo treatment for their unaltered 8-month-old greyhound. he refused to treat them unless he could spay her. obviously spaying a patient that is parvo positive is not an ethical thing to do, he required her to be spayed before she left the hospital at the end of a being hospitalized for parvo... sorry but I agreed with the owner that spaying as she is getting over parvo isn't right, and she needs a significant amount of time to recover, 3-4 weeks minimum. so having the owner sign a contract stating that she would return for a spay by 60 days post discharge or she owes the remaining balance of the parvo treatment instead of it being discounted would be appropriate here. he kicked her out of the hospital because she said that that wasnt cool, which it wasnt.... Ive been a tech for 4 years, and no normal vet I have ever worked for thinks a spay right after a parvo infection is a good idea

Please either react to or put out a statement regarding "veterinary secrets." As a 4th year vet student who already deals with too many misguided clients, I have been deeply disturbed by what I've seen from that guy's platforms, and I'd really like to see a vet with an evidence-based mindset (like you) take him on. (I hear he's not even licensed anymore...)

Dr. Jones just posted a video addressing that. He surrendered his license because he didn't want to conform to the vet medical world. He knows there's alternatives to the harsh medicine and unnecessary procedures and he didn't want to be part of a field that takes advantage of a situation. Wth do you think people for thousands of years have done. There's obviously things you must take your pet to the vet for but not for every little thing. And prevention is the best thing. If you're not about holistic medicine and if you refuse to believe that nature is the best medicine then that would be your m.o but to discredit him is insane.

The cost of medical care to pets is sickening. Basically if you can't pay for pet insurance. You can't afford the price to fix your pet then they rather kill the pet then help. It's sickening. They deserve to make money but 10 thousand for back problem get out.

There was a time when the price of vet care was the price of a bullet. Would you like to go back to those days? A vet needs to know how to take care of multiple species, not just as is the case for human doctors. Medicines, machinery, diagnostic equipment, lab equipment, etc. is costly and not getting any cheaper. I don't disagree that things are expensive, but look at the cost of human medicine, and also how super arbitrary the costs there are. There is no magic pill, magic bullet, or easy answers to health care costs.
