Why didn't you put screws in the mount extension plate? Are the holes on the mount extension 16" apart from each other so I can mount on wall studs?

The extensions do allow it to fit to studs 16" apart. The ceiling in this video was solid, so the installer ended up using the central mounting holes for solid ceilings instead.

How much space do you need from the wall to the bracket?

This will depend on the size of the TV, but for an 85", we would recommend placing it with at least 22" between the back edge of the ceiling mount and wall.
