Some added info from a 50 year old. Lisdexamfetamine (in the video) feels quite different from Methylphenidate (ie. Ritalin, Concerta, etc.) and again from mixed amphetamine salts (ie. Adderall, etc.). While my current one (lisdexamfetamine 40) is preferred as it feels less probing into my life (I'm going to 60 in the couple of months), it can also take some getting used to as it doesn't give you that 'speedy' feeling in quite the same way. I've also had the dexamfetamine version (10), which I found hard to control right from the lack of immediate feedback. The only downside to my current long-acting is that I've had some trouble sleeping some nights, but don't know if it's medical or just life related, but I never had that happen in my years of Ritalin (10 x 5 per day, or 60 long-acting). Still think it's the right choice for me, it seems to work just right. I work in a heavily concentrated job as a computer software developer and manager.

Vyvanse has helped with my anxiety a lot as he suggested. Most of my anxiety comes from not being able to perform and executive function which the vyvanse helps a ton with. I’ve tried adderall IR, XR, and Ritalin. Vyvanse is my favorite because CBT with my therapist highlights routine, planning, and moving through your day in an effective way. Waiting 20 to 30 min for a jolt from adderall only to be followed a few hours later by a crash (for me) is not conducive to a steady focused state throughout the day. After I take vyvanse I go about my morning routine waiting for it’s effects while doing basic things. Then I get myself to my desk and start working. The onset is very smooth (takes about 40 min to an hour for me) and before I know it I’m in the groove. For me working with a therapist on organization and planning is an absolute must. I consider it just as important as the medication.

Hi, would like your opinion/advice on Vyvanse. What do you feel like when you’re in it yourself? My first couple of weeks/month i got not a high as such, but a significant bump in feeling, i felt free and happy again if that makes sense. I didnt find the need ti play computer game to fill time and block out everything else in my life. I wanted to be more sociable and general move. Now though I dint get that at all and im wondering if theres a problem, or is it supposed to be that way? Anywhere else i search ofr advice its always people talking about their first day or week etc. Sorry to randomly bother you but I’m struggling to get answers anywhere

@@Allegro_Giustoin my experience you develop a tolerance to Vyvanse fairly quickly - requiring higher doses. The only trick I found that helps is to take regular drug holidays. Stopping for 3 days or longer seems to help reset my tolerance (somewhat). Thankfully I don’t get any withdrawals, other than mild fatigue. You should also consider diet and other medications, which might be causing you to metabolise / excrete Vyvanse at an accelerated rate? Try and stay away from anything that’s acidic or causes you to pee a lot. Might be worth asking your doctor to look at doing a urine PH test?

To answer what I feel like when I’m on it, I’d say I feel like I have clarity and motivation to do things. I will definitely add a caveat that the energy to do more things is most noticeable when you first start taking or increase the dose of the medication. I’ve been taking the medication for over a year at this point and was on 40 mg until a few weeks ago when I bumped up to 50 as I needed it to last longer so I can do my class work after I finish my 9-5. That drop in “effectiveness” that you’re describing is something I’ve experienced but I’ve come to realize there are so many individual factors that go into saying why that it. Primarily it’s because your body adjusts. Perhaps you’re burning yourself out and feel less, maybe your dose isn’t high enough, but be very careful about chasing that “bump in feeling”. This is definitely a conversation to be had with a psychiatrist (not primary care Dr. IMO) bc they better understand the variables. In short, I don’t have an answer for you. I also notice that slight drop in effectiveness but I’ve learned that the medication helps me implement the planning and organization skills I work on in therapy. My best advice is to talk to a psychiatrist, talk to a therapist who specializes in adhd, explore underlying reasons you may feel less driven or focused (clarity you described), hydrate, exercise regularly, and have a healthy diet. Medication is intended to help people get to “baseline” levels of function on a population level. I’ll say with confidence this pill changed my life for the better, but it didn’t fix everything. I suggest watching Dr. K’s videos (Healthy Gamer) on stimulants. I don’t know if he directly discusses the question you asked but I feel like he provides important context to explore that topic in an informed way.

I experience that too. It's normal. It's the High that makes the fun. It's not really a decline in efficiency for me. It still hold me awake and alert. The positive Feeling, the Drive to do Stuff pendels back to more 'normal'. It stops making you high. It's one Reason why you have to wait a while until you know if your Dosage is right and if it's right for you longterm, and to speak about Changes to your Doc. The High dissappears in about 2 Days for me. When I need the Mood Boost and Drive for something very important and can relax before, I take less some Time or even Skip a Day. Than I get a little Boist again. I never take more than my prescribed Dosage! Never go above your Dayli Dosage! You only really know how it affects you after you calm down. Thats when they work as intended. When you feel the need to take more you have to speak to your Doc about it. Stimulants are exhausting for your Body and Mind. You should care about not exhausting yourself. And to get good Sleep Meds are just one Part of managing bad Adhd Traits. There is no quicksolution to snap them away sadly. Keep Care and stay well.

ADHD medication helps me a ton with my anxiety. When I don’t take it I have to use Valium but while I am on it don’t even think about it. Also makes me not do stupid things so often. I actually think before I talk.

I am recently diagnosed with ADHD at 50 y/o. Also been suffering with anxiety for the last couple years. My Dr thinks getting my ADHD under control will help with my anxiety. I start my first dose tomorrow starting slow with 10mg. Hoping it helps with my anxiety, tired of feeling like this.

Thank you! Been on 70mg since the summer. A Lifechanger. Landed a job, promotion coming up. Before that... suicide attempt (coated fellers at the hospital said at least 4 people would've died given what I did, quite a miracle I'm still here), anxiety, depression, social phobia, PTSD, epilepsy, RRMS. A neurological jackpot per se. Been on Ritalin (all kinds), Amphetamine (prescription of course, folks overseas call it Adderall), Dexamphetamine (won't mention everything else, since ADHD is the topic here). Dozen schools, always quit, never finished a book before getting diagnosed (was able to finish my first one at the age of 27 medicated), always the worst one in class, never finished a thing. Exhibit A. Secondly, with Elvanse, the potential that I've been so shy to unlock or expand or dig up, that's always been there, I've finally got a grip on it. I took control. I grew up, I became myself. I am reborn and the happiest I've ever been. No more need for the anxiety meds, depression is gone, things are in order (my own personal chaotic order where I rule). No more mood swings, spacing out, irritation, memory issues, suicidal thoughts, brain fog, million thoughts per second making my brain either burnout or get way too overly creative picturing thousands of situations and their outcomes in a millisecond or say things to people they don't want to hear, basically won't keep the mouth shout at inappropriate times, It worked for me in a way you can't imagine. 30 yrs old btw. I'm confident, I'm where I'm supposed to be and I finally know who I am and what I want. To the folks at home, meds are the answer to some, others can find a soulmate or a best mate who won't stop praising you, since the insecurity up in there is over the roof. There are many ways to combat this hell forsaken condition, which up to date still won't be taken seriously by the others, even the doctors. It's a nightmare, a horrible dark field with a terrible outcome torturing innocent people who are misunderstood, ruining their lives. To those concerned, don't give up, whether you're lonely or not, bullied or laughed at... I know it's a torture, but the best is yet to come. Stay strong, because you can!

I’m so happy for you🌸 Started taking Elvanse adult for a 2 weeks is now 30mg , going to increase to 50 mg . I have ADD and i combine with atomoxetine 40 mg( im going to try take ato away when i had elvanse for a while too see if just elvanse can be of help ) . I always been tired all my life but know i feel more motivated and alert , strange feeling cuz it’s new to me . 🍀I hope elvanse will help me grow in the spiritual path too, becoming the witch i am meant to be , helping people and finally can do something that i don’t quit after 1 week starting something new😅 going to test read a book too , i can’t read any books at all 🥲🌬️🍀 🌸Thank you for sharing your long text, all love and light to you 🌸

I have a good friend for whom vyvanse is a game changer. For myself however, it accentuates certain of my issues. The first few weeks I took it, it was amazing. I was calmer, able to focus for long stretches of time and to stick to a given task. It s been about a year since I started taking it. Now, I feel excessively tensed/antsy. I feel some sort of inflamation at the base of my head and ears (it s not caused by vyvanse, but it seems to be making this problem worse everytime I take it). I also feel disoriented because it seems like I completely lost the capacity to be located in time. Like for the life of me, I can t remember what I did 5 minutes ago or what I wanted to say by the time I spoke the first half of a sentence. The antsyness I first mentioned seems to impede the helpful effects the drug first had for me. I will hopefully be able to see a doctor and try a different medication.

Similar experience here. Definitely makes me more intense and feisty at higher doses. Also seems to make me hyperfocused a lot, which is counterproductive because I lose the ability to manage or keep track of time. If I lower the dose it’s just not effective enough to control the ADHD symptoms and I lose the ability to manage or keep track of time or regulate emotions. Bit of a catch 22.

When I took Vyvanse, I got unusually irritable. And my vision would go blury sometimes. I stopped after giving it a month. I just couldn't deal with it.

Been on Vyvanse for 10 years now. Had a lot of ups and downs with this drug. MOSTLY ups You gotta be tough and be willing to plan ahead because there is no wonder drug that will live your life for you. I break open my capsule and put it in a bottle of water every morning. I take a few sips every few hours until I finish the bottle around 3 or 4 pm. I'm ready for bed by 10pm. You gotta eat 3 times a day. (Even light meals) You gotta make sure you are sleeping. (This is so important) Also take one day every two weeks and DON'T take your Vyvanse. This helps recharge dopamine and serotonin. 10 years of experience. I've been through the pits of hell because I wasn't following these steps...

can you please elaborate or help with your comment on drug educed manic episodes and vyvance. I am a week off taking benzodiapene daily for a recent previous trauma. a few months on the medication. i feel manic and i know benzo withdrawl is one of the more horrible drugs to withdraw from. but could you please elaborate on the link between mania and vyvance and im hoping this can help me look distinguish between withdrawal symptoms and those from the tablet. id hate to think vyvance is making me feel this way when its been great, but its hard to tell a week into these withdrawls and manic states

Hi Caris, there are some patients that have more side effects on one stimulant and not on another so it may be worth trying in conjunction with your doctor. Unfortunately, there are some patients that cannot tolerate stimulant therapies altogether.

I the effects of vyvanse last about 2 or 3 hours for me. Then the crash starts to creep in along with increased irritability. Still contemplating whether the trade off is worth it.

Thank you. This was very helpful. Are there any age related prohibitions with Vyvanse? Does it have to be prescribed by a psychiatrist if a psychologist has diagnosed ADHD or can I ask my GP? I’m over 65, just diagnosed, and wondering if I s/b taking something. I have been seeing a psychiatrist for BP but she I has little experience with ADHD and, so far, is reluctant to prescribe it. I have been offered a neurological training course by the psychologist who diagnosed me but it’s very expensive. We don’t have insurance (I’m in Canada so psychiatry is covered) and are retired on one income. My apologies if this is too much for you to respond to.

Hi Moushka, it will be different depending on country, in my country the psychiatrists can prescribe it or in my case the psychiatrist can write a letter to me as the GP and I can apply for a permit to prescribe it in my country.

Hi Adriel, as usual this is general advice please see your doctor for your specific case. However, typically if the effects wear off before the end of the day, I usually add dexamfetamine as it is shorter acting and less likely to affect sleep.

This isn't advice, just my experience: I was taking 50mg as one dose when working, and I was really struggling: the come-down was really unpleasant, my appetite reduced to nothing and I couldn't do anything at all when I got home from work. My psychiatrist recommended that I split the dose so I now take 30mg when I wake up (6am) and 20mg when I get to work (9-9.30am) and I feel way better.

Sometimes higher doses can last longer than the lower doses, sometimes it just increased "intensity" without significant lengthening in time, only way for an individual to know is to try it. Check with your doctor if its appropriate for your case.

You mentioned drugs and alcohol can impact the effectiveness of the medications, does this extend to coffee? I was under the impression one should cut out coffee when on stimulant medication?

@@drkiddMD Great thanks! As a suggestion it would be cool if you could do a video covering extended release versus immediate release ADHD medications. Maybe with a small section on how dosages compare just from the pure medical perspective. How does a Dr. decide on dosage and can you relate dosages between the IR and ER forms of a drug. I am an Australian based in Germany and there is no information around to help me understand the intricacies of this stuff. It is made worse by the common notion among physicians here that ADHD (ADHS) is not a real thing in adults, and is only a behavioural problem among *very few* children

Just started Vyvance in the last month. 20mg at 5am (I start work at 7am and go to bed early). I can fall asleep shortly after 9pm. I’ve started waking up around 4:30-5am and staying awake most days. My alarm is set for 6:15am. Sleep has never been this much of a problem for me. I’m not really finding myself more tired throughout the day but it’s just way too early for my family/lifestyle. I’ve started getting a 24/7 low level headache. I’ve also noticed that my focus is waning. I’m afraid of the headaches increasing if I up the dosage. Is this a given? I miss enough work due to chronic pain and financially, I’m afraid of increasing the dosage. 😢 Update: dosage was increased to 40mg, headache stayed the same, no improvement in focus. Dr suggested a 2-week hiatus. Headache went away on day 2! 😮‍💨 I feel much better. Someone suggested that Ritalin (sp?) is easier on the body. May talk to Dr about it.

If you go to sleep at about 9pm and wake up 4:30 - 5, that 7:30 - 8 hours of sleep, which sounds perfect, tbh. Why are you taking the meds at 5 if your alarm clock is at 6:15? Sounds like it should be possible to shift everything back at least an hour.

@@mrsupertash Turns out the Vyvanse gives me a constant unmanageable headache. I also tried Ritalin… same thing. Nothing is worth that kind of pain. But yes, good idea.

There will always be a few side effects in the first two weeks, mostly they are minor and your body adjusts to the new ‘thing’, and it goes away so dont worry too much intiailly and over think. If they persist longer than that like the gentleman in this thread, then try reducing the dose and/or an alternative - it’s perfectly normal to do and discuss with your doctor/specialist.

Vyvanse can be helpful for inattentive type, hyperactive type and combined type. There probably isn't any research that I am aware of that compares Vyvanse against the different types.

Im not a doctor but i have a whole lot of experience with amphetamines. I've been addicted to amphetamine in my younger days and now im on vyanse. From my experience the feeling of restlessnes comes from either taking a too high dose, eating a lot of sugar or caffeine. Especially caffeine, it can trigger a lot of sideffects. Also, people should be extremely careful with vyanse as it have almost the same addictive properties as street amphetamine. It's all about your personality/history, most (99% of them tbh)of my friends who have been prescribed vyanse are now abusing their medication. It starts with a little extra one day to get that euphoric feeling vyanse gives when you take a little more than tolerance level, after that you will start to chase that high just like with any other drug so be careful folks.

not sure where you got info on vyvanse, but it specifically does not give you a sudden 'euphoric' feeling. In fact, if you try and take to much of it you're more likely just to get the bad side effects before euphoria.

@@capt5656 You don't know what you're talking about, vyvanse is an amphetamine and have almost the same properties. If you take dose of vyvanse higher than yout tolerance level you get euphoric just like with all other amphetamines. Some people don't like that feeling tho but thats a different subject. Higher doses does not equal more sideffects, depends on who you are.

@@donaldskurt953 🙂I can see my med bottle right here, lisdexamfetamine. This substance, Vyvanse, physically cannot affect you until it has been processed into the active dextroamphetamine. Thus, taking more Vyvanse does not magically give you a high, and yes, higher doses does give more side effects.

@@capt5656 I know how vyvanse works and i have take them daily. The release is delayed and gradual over time, but that does not mean that you can't overdose to get a high. In Sweden where i live, vyvanse (elvanse here) is sold for about 150-300 dollars on the black market for that very reason. I have many friends who have fucked up their whole life on vyvanse in the exact same way people fuck up their lives on street amphetamine.

I'm on a low dose of Vyvanse, and I am able to take it with a cup of coffee (in fact, I find WITHOUT the coffee it's harder for me to focus in the morning). Never experienced Euphoria. But, as you say, it does depend a lot on your history.

@@Highlander1432 didn't only do that it helped me figure out existence I changed Einsteins theory of relativity from a fabric in space to infinite density and that would explain the black hole 🕳️ as the weight sinks and stretches the density not to mention I figured out string theory the lot I knew how life started and what started it and when and I can prove it all too

How does this compare to adderall for ADHD? If adderall increases anxiety, is there a reason vyvanse might not?

Hi Jose, its hard to predict some people respond well to one and not another type of adhd medication while someone else might be the complete opposite.

Dear Dr. Kidd, thanks for this very helpful video. More of this would be awesome. Do you know if its common to go on a higher dosis than 70 mg? Or to combine Lisdexamfetamine and Methylphenidate? Don’t worry, i just want to know if it’s possible and then talk to my doctor about it. Thank you 🤗

Hi Leah, it is rare to go above 70mg lisdexamfetamine or be on both lisdexamfetamine and methylphenidate.

@@lenaaaahh ADHD seems to be antithetical to the stereotype of German culture and work ethic. I can understand how it has taken a long time to gain widespread understanding in Germany.

@@drkiddMD Now i know i was overdosed... a lot! I'm now on 2x10mg methylphenidate and it's working. I guess even 30mg lisdexamfetamine was to much for me. Unfortunately my doc supposed that it was not enough, so i went higher and higher, with bad side effects.

i'm diagnosed combined adhd, tried elvanse and xaggitin xl 20.30,40 and 50mg and none help with my symptoms my sleep improved on elvanse and that's about it. I'm on a break now but I don't know if stimulants is the way forward. Any advice

This is a difficult one to answer over the internet, need to assess what symptoms, whether all are contributed from ADHD or if there are lifestyle factors or co-morbid diagnoses that need to be treated etc. best to get a comprehensive assessment from your doctor!

By avoiding recreational drugs and alchohol, is it still fine to take those every once in a while say once every 3 or 4 weeks?

My advice is to minimise alcohol and recreational drugs as much as possible if taking these medications

Not really 'fine'. Any amount of alcohol or recreational drug is a risk when on this medication, but a moderate to large amount is asking for trouble in my opinion, even if you only did it once a year.

What can alcohol cause with Vyvanse? 1-3 drinks once or twice a week while on it?

The usual recommendation is that alcohol should be minimised as it can impact on the effectiveness of the medication and can also impact on the symptoms of adult ADHD.

Depending on the level of blood pressure and how well it is controlled your doctor may still consider it in your situation. Non-stimulant options for ADHD include Atomoxetine (Strattera), Clonidine and Guanfacine. As usual cannot get specifc advice over the internet but your doctor may discuss these non-stimulant options with you if your blood pressure is too high.
