I think it was a pre planned and choreographed move that was meant to go along with the lyrics and yes, shock. When it wasn't well received, all involved claimed it was a "wardrobe malfunction," which I just don't buy. The fact that Janet Jackson was blamed and just assigned the brunt of the scandal, and that Justin Timberlake came out unscathed, speaks to the misogynistic structure of our society.
