My headcanon for Dr. Coomer: He was a tutorial NPC who was scripted to die from a barnacle. However, Gordon saved him from every barnacle they encountered. Well I'd say Coomer was subconsciously aware in Act 3 since he mentioned a World within Gordon's dreams, and said when Gordon goes to sleep he can feel his body torn apart, it's only just before fighting Benrey that he's fully realised the world may be fake, and he is completely aware once "Gordon" confirms the world is fake.
