i don't get it which exosome's size is 1 million size of hair :( hair's diameter is 100um and exosome's size is 100nm so.. just 1 thousands i think..)

What they aren't telling you is that exosomes are a response to all kinds of stress or assaults on your body. RNA wrapped up in a package that is sent to other cells to replicate, cause the cell to burst, only for more to be released and do the same to other cells. Of course it sound self destructive, it actually is beneficial in flushing out all kinds of toxins in the body. Ladies and Gentlemen, I give you "The Flu".
