It was hard to follow the comprehensive (detailed) slides, however I did not see where as a second step after observing an elevated PSA is to get a fractionated PSA. This differentiates between bound and unbound PSA that can increase the specificity for malignancy. What is the current state of this additional PSA test? Concerning treatment and long term survival rates, is radical prostatectomy still superior to radiation based on the fact that post radical surgery, Gleason scores increase 25% since even 12 core Bx results do not give a complete picture of the disease?
