I would think that normal t3/t4 values are important to consider here. Those with imbalances with T3/T4 and TSH (true hypothyroidism) respond better to Levothyroxine (especially if they are older/above 50 years old) If T3/T4 values are normal and TSH is off then this is why we have the dx of Subclincial Hypothyroidism. This is more common in those under the age of 50 y/o and therefore Levothyroxine is seen to have too many side effects (which is why they don't treat). In many of these cases diet (not enough carbs/healthy fats and proteins - emphasis on *healthy* fats and carbs) and lack of exercise are often the etiology. Thyroid hormone is all about getting oxygen into the cell to increase metabolism. You have to move your body and eat right to make this happen.

Thank you so much! This is validating my experience so much! 4 years ago I got checked, TSH at 3,9. I was so tired all the time. My doctors said everything normal. I should take D3 and this would help. Now, 4 years (and 25 pounds) later TSH at 7,1 and free T4 at 12. Get new bloodwork in 1 week, with same result we start treatment. (It’s the standard here in Denmark, start treatment with tsh over 4) I am beyond happy my doctor checked everything, listened to me- I am tired of being tired all the time..!

Very informative video. My doctor diagnosed me with hypothyroidism with a TSH of 3.5 a few months ago. I had a persistent pain in my elbow joint which he said was due to my thyroid as my uric acid level was at 6.2. Other symptoms I had included weight loss resistance, dandruff, dry skin, fatigue and trouble focusing . I've been on treatment and my TSH levels have been improving.

About ten years ago I was feeling very poorly, got diagnosed with a burn-out. I had my blood examined then, but really the worst was already passed by then. My TSH was 5.6 and my Free T4 was 13.2. Later on when I was feeling much better my TSH measured 2.4 and my Free T4 measured 12. Now I'm having some of the same symptoms I had back then. Mainly, extremely cold hands and feet. Tiredness, moodiness. And strangely, for a little while, the feeling that something was stuck at the back of my throat. I'm not waiting this time, I'm having my blood checked tomorrow.

My GP won't treat me, despite the fact that I'm extremely symptomatic, I can't get through a day without needing a nap, my hair is falling out, I've gained weight, my skin is dry and painful, my tongue is swollen and I'm so exhausted and low. NHS guidelines are stupidly out of date, I've been told I need to wait until my FT4 levels drop before they treat me and I feel like I'm drowning. I'm at a loss.

How are you doing today? I’m so sorry your dr didn’t take you seriously and your health issues progressed. I just had an ultrasound on a nodule and it came back “moderate to high suspicious”. My dr couldn’t care less and told me to come back in a year to check it again. So I said screw this and made an appt at a specialist’s office to see their recommendations. I have a TSH of 5.33 and my dr didn’t bat an eye at it. He was just like “you’re fine. We will check everything again in a year.” Meanwhile my quality of life admits all of the low thyroid symptoms I have plus now the worry of having a suspicious nodule in my neck has drastically declined. I left my drs appt in tears today. I feel completely brushed off and unheard. He made me feel like absolute shit.

I have been told I don't need treatment at the moment unless I want to get pregnant. My mum is also considered to be subclinical, but I believe my symptoms started slowly showing up after having covid or the vaccination. I am concerned about finding a way to make this better, the doctor told me there was nothing I could do but my quality of life has reduced somewhat since all this started. I initally thought I had PMDD and went down the path to look into that. It was only the white spot on my skin that prompted me to go to the dermatologist who sent me for further tests.

TSH should always come with reflex to T4. Assuming T4 levels from only TSH is widely frowned upon. Usually, Sublinical hypothyroidism is Asymptomatic elevated TSH with normal T4. I agree though that regardless of TSH or T4 levels if symptoms are clinically present and obvious in the exam then Thyroxine is definitely needed. TSH + T4 should be checked every 6 weeks to 3 months and watch for symptoms of exogenous hyperthyroidism.

Thank you for this vid Dr. Westin. I am not in the US and it resonated with me when you said about most cases you hear of Dr's not treating because of 'normal' TSH are outside of the US. I have visited endo's in Singapore and Thailand and have been met with the same, told I am subclinical Hashimoto's and to wait until my numbers get worse until being able to receive treatment. I have asked them (4 different endo's) whether there is anything I can eat or not eat during the flare-up episodes (I faint and remain spaced out for hours at a time, bp drops), and ALL of them have told me that there is no evidence to support nutritional solutions! This is from the best hospitals in both countries! So I am left with experiencing hair loss, dry skin, weight gain, flare-up episodes and recently vitiligo has begun to appear. TSH 2.8, FT4 0.8, FT3 2.1. ( I also have a pituitary issue which may artificially keep TSH lower, Dr isn't sure whether the fainting/low bp episodes are hypothyroid or hypopituitary). Anyway, thank you fo rthe vid and teaching what 'subclinical' actually means.

I prescribed improvement in sleep hygiene, vitamin D supplementation, CBT-I for a stressful new job, and will check his total T. My first symptoms from an 0.02 TSH score were insomnia followed by leaky gut, and then high blood pressure and high heart rate and lots of anxiety for no reason! I then was prescribed Carbimazole for which I took for a bit too long and then swung the other way into Hypothyroidism with a dangerous TSH of 32.0 and my entire front thyroid has expanded squishing my front neck making it difficult to breath at night along with sleep aponea that you can hear even upon when I’m awake, also swallowing difficulties ! I’ve literally stopped taking the Carbimazole and introduced Potassium Iodide, along with high doses of Vit C, Turneric and Ginger, plus Goitogenic foods.. Do you think I can shrink this thyroid back down or will I need meds?

Had symptoms of weight gain, problems on the front of cognitive capabilities, tad elevations in cholesterol, energy sapping .But since i was a regular gym goer back then ,and active they prescribed me the meds whilst maintaining not to touch them unless i feel a real need to .And i only had it for a period of 2-3 months during this decade .No dietary restrictions self imposed.During the pandemic my levels dipped below 4 on their own .It did not progress to overt hypothyroidism but instead reversed to euthyroid. However muscle fatigue, intolerance to extreme weather remains and im on the verge of considering taking atleast small dosage to combat this .

In subclinical hypothyroidism the TSH is elevated but the thyroid hormones are somewhat normal. The elevated TSH is causing problems, like blocking LH and FSH release because of the elevated prolactin levels. If your T4 or T3 is low, then that is just old school hypothyroidism.

At 14 years of age in 2013, the reference range were ft4 (12 to22), my daughter was 12.6 and her results showed sub clinical, told by the doctor who knew maternal and paternal members of her family had been diagnosed with hypothyroidism, “even though your abnormal, I’m not going to treat you”. As soon as I found out she was pregnant in 2015 reference range ft4 (9 to 19 ) she was diagnosed at ft4 (9). So with this diagnosis she was even more sub clinical than my diagnosis in 2011 ft4(12 to 22), and her sub clinical diagnosis in 2013. It makes a real mockery of the doctors who put their names to this serious condition, doctors who saw symptoms and probably treated accordingly.

I've watched my TSH and TSH Reflex to T4 drop below normal and rise above high normal. In addition, over 4 years, my free T4 continued dropping. My most recent CBC last week, in which I was able to diagnose myself, showed a 4 year WBC Increase from 7, 9, 11.9, and 12.5 with a comparable rise in lymphocytes. My TSH was 7 last week. I'm symptomatic and have been for quite some time, and had I not studied medicine independently, I believe illness would be the only constant in my foreseeable future. With my last bloodwork, I was classified as clinically and biochemically hypothyroid. What exactly does this mean? I am on cholesterol and blood pressure meds. My tsh was 7.67 and was prescribed Synthroid 75mcg. I have not felt good since taking it. Does it take awhile for it to start working? I go see dr. In Jan.2025. Any advise would be appreciated.

I've just been told that I'm subclinical hypothyroid. I'm 29 my TSH is 5.9, my free t4 was "normal", I'm neg for thyroid antibodies, I've gained 17lbs in the last 2yrs and I'm exhausted all the time, my nails split like crazy and have high cholesterol. I'm also low in DHEA so my doctor recommended supplementing that and come back in 3 months and see how I feel. Should I try to find a new doctor or wait and see what the dhea does?

THe elevated TSH is very suppressive to your gonadotropin productions. You are essentially suffering from a deficiency in sex hormones. Thats what it does. You need levothyroxine/t3 or both to reduce the TSH to around 1 mIU/L. Enjoy!

TSH- 9.4, T3- 1.4, T4-6. LDL is elevated. Doctor says no action needed as it's all within range. Check labs again in 6 months. 😢

My doctor said said that I have borderline hypothyroidism because my TSH is elavated and my T4 is low normal but hasn't given me anything. I have gained alot of weight and my mood and energy levels are very low. I can only sleep with a sleeping pill. My CRP is elavated so he put me on low dose prednisone and my liver enzymes are still elavated as well as my cholesterol. I am so frustrated. It's as though I have no say in my own treatment. I see him today so hopefully I get some answers. I also get muscle weakness.

My TSH is 2.77. T4 is 1.2 ng/dl. T3 is 2.69 pg/ml. RT3 is 17 ng/dl. I dont have hashmoto. My weight is good, and my titamins too. But I have symptons like too much anxiety, some tiredness, dry skin, 47 bpm, 96 temperature when waking up, high LDL, some constipation. Should I take thyroid med like NDT? I would love to get some feedback. Thank you all.

Hello, I had my labs done five days ago and my TSH is 18.75. my daughter told me that I have sub clinical hypothyroidism and I see you say that if it’s above 10, it is hypothyroidism. I really don’t want to take the pills and I would like to try to do it with some natural approach. do you think I should seek pills as a must or I can try to reverse it more naturally?

It’s been 3 months. Test it again. If it’s still high take the medication & get your TSH under 2.5 over the next few months. My TSH was 29 when I first had a thyroid storm, it dropped to 7 & stayed there for 3 months & then I started medication & it makes me feel better.

i had 8.0 and took another test 2 days later with 9.3 tsh, the doctor told me it could just be an inflammation and that i should come back in 4 weeks, ive been feeling like shit for 4 months

Yeah I was in that boat. I’d take the medication if I were you. I waited 3 months thinking it’s resolve with lifestyle change & gluten free diet, it didn’t. Now on full replacement & feel myself again.

What about a diagnosis of subclinical Hyperthyroidism...which the opposite of what you are talking about...My TSH was tested at .005 but my T4 and T3 are in the "normal" range so I have been told. I also have cold nodules and a goiter but have symptoms of Hypothyroidism...i.e. weight gain, fatigue, and high cholesterol....Not being treated because "the system is working" so says my previous doctor...?

Hi Dr Childs. Wondering which of your products might assist me and if you ship to Australia. TSH hovering around 2.3 to 3.1 with a low T4 and low T3 (all “in range”) and high RT3 (top end of broad range) and high Leptin - I suspect from extended caloric restriction through fasting - now how most of the typical low thyroid symptoms.

Hi doctor, Mine was 15 and diagn as subclinical..t4 was within range on the lower side…that s what they were testing.. is it possble that at some point thyroid is sluggish but at some point starts working well again, how would i ever know that if I m taking levothyroxine every day?My problem started same time as perimenopause. Is it poss to cure this condition? I dont have ptoblem with sleep, never had, no problem with constipation, never had. My only problem was falling hair and hot flushes, as well as rapid weight gain, so didnt know if that was menopause or thyroid. My diet now is no gluten, and rather ketogenic diet. How do i get rid if levithyroxine, diagnosed 4 yrs ago…

My score was 7.32 with Triglycerides 248, HDL 23 LDL 151 but cholesterol is 174. I suffer from fatigue, prediabetes, sarcoidosis which is inactive & having sleep problems which they prescribed Vistaril which doesn't help. I have a fatty liver with some fibrosis. My Physician just wants to monitor the level. My free t4 is 1.02 Should I be on medication to help with the LDL etc. I also take Ozempic for my diabetes and fatty liver which has helped reduce the levels from diabetic to prediabetic and and had normal AST & ALT levels for the first time in years. I don't drink and I'm not over weight. Should I be on Medication

Maybe it's normal for your body it's not an exact science, all this tsh must be below 2.5 or even worse below 1 for people without thyroid disease is nonsense, everybody's body is different the most important thing is how you feel If you don't have any symptoms of hypothyroidism like weight gain hair loss high cholesterol ect then its nothing to worry about.

Hi Dr. Childs. Thank you for this video. I am sort of at a loss right now and now sure how to proceed. I had thyroid US a few days ago that found 1.1cm TIRAD 4 nodule. My GP says to just check it in a year. My blood work this week shows that my TSH is 5.33 but my T3 and T4 are normal. I have some bordering on severe hypothyroid symptoms (At least that’s what I thought was the cause of them) and I expected a 5.33 TSH to validate those symptoms. But my GP said he’s not going to treat it and to just redo my blood work in 3-6 months. Honestly, he seemed like he couldn’t care less and I could tell from the moment the appt began that he was set on not doing anything about it. I realize in the grand scheme of thing hypothyroidism is the least of his concerns. But if a patient is experiencing almost ALL of the HT symptoms and has a 5.33 TSH, would it hurt in the long run to just try and treat it??? I’m guessing this is going to be a matter of finding a different doctor, eh? This comment/question was way too long and I don’t blame you if you never reply to it. I’m annoyed with myself already.

Hi, my TSH levels have been fluctuating since the past 7 years. It can go up to 7.8 and fall back within normal range in subsequent blood tests a few months later. I am at a healthy weight, no depression strong nails, thick healthy hair, no goiter etc. Should I worry about my fluctuating TSH levels?

@@shawnpeters2196 I'm still the same. Going on 10 years now with fluctuating TSH levels, but zero symptoms of hypothyroidism. I see an endocrinologist regularly. I have gotten my blood work in the morning. For the last 6 months it has been in the normal range. The blood work prior to that was very high. My doctor says 3 things could happen; the TSH could stabilize, could continue to go up and down or permanently elevate, resulting in hypothyroidism. I'm hoping it won't be the latter.
