Not to undervalue how helpful a therapist can be, but It's interesting to hear you speak of mental health professionals as though their value as therapists can be measured my their degrees and education. Having been raised in a home which was fond of psychiatry and psychology, at an early age I was given a close up view at how dysfunctional a psychiatrist can be, and I came to realize it wasn't the psychiatrist or the psychologist that helped me the most, but a local councillor who volunteered to help troubled youth, and a pastor who helped me to understand the connection between my actions and my unhappiness.

Hi Becky, a lot of this was really informative, thank you. I found as the video progressed that you seem to have quite a derrogative tone towards counsellors and some psychotherapists and described them as though anyone can call themselves such. This is the case for 'Life Coach' - there are no board accredited courses and anyone unqualified can use this term. But to class yourself as a Counsellor or Psychotherapist you can't just do a few weeks short course. You have to have completed an accredited 2-3 year course at the level of a Diploma or equivalent/higher, had at least 40 hours of personal therapy, at least 100 closely supervised client hours, completed an accredited VIVA and be registered with an accredited board (such as BACP as you said). If you trained as an Integrative Counsellor, you also don't simply 'sit and listen' (which in itself is highly beneficial to some clients), you may well use scientifically proven interventions such as CBT. I also feel you are weighing too heavily on years of study as the defining factor for a good practitioner. I appreciate this may be the case for learning the psychology behind diagnosis, and you may feel that the above examples are a pittance compared to 8 years of study. However, 8 years of lived experience (such as experiencing a long term mental health condition and/or receiving multiple rounds of high intensity CBT/NHS/scientifically proven interventions yourself) would add to ones practice, knowledge and ability to support a client effectively. It is also important to consider the client as an individual - it's all very well acknowledging that certain interventions are scientifically proven to improve mental health, but if a client has issues/difficulties accessing the approach in some way (eg. They are unable to read/ write and CBT is heavily reliant on this) then it is redundant. It's also important not to dismiss approaches because they have yet to be explored scientifically. For example - there is little/no research into the scientific benefits of Ecotherapy, specifically. However there is strong scientific research into the multiple health benefits of breathing in the phytoncides that trees release and the positive psychological impact of natural sounds and environments on stress, anxiety, exhaustion and hypervigilance for example. I feel it's really important to value all professions equally and take a holistic approach to mental health, instead of glorifying some roles and shunning others. Just because research hasn't been applied yet to a certain intervention, it doesn't mean it won't benefit a client. Everyone heals differently and academic qualifications are not the single defining factor in a good mental health practitioner.

As someone who is training to be a counsellor and has spent thousands of pounds on courses and lost all my free time to completing my Level 4 Diploma, thank you for explaining this properly. The video was full of great information but to hear counsellors dismissed as people that aren't particularly well qualified or know that much other than how to listen to someone, which anyone can do, was pretty shocking and damaging to the profession. It is far, far more than that as you outline perfectly.

You spoke very dismissively of psychotherapists and counsellors and it was incredibly disappointing to listen to. Broaden your knowledge and come back when you are prepared to praise all psychiatric professionals not just yourself. Egotistical.

Your video upset me and compelled me to speak out for the competent therapist. As Carl Rogers once said, “ Very well qualified people exist outside the fence of credentials”. With wisdom also comes humbleness. A quality also worth learning.

Interesting clip but misleading, incorrect and a whiff of being patronising. I am an integrative psychotherapist and I have been trained to diagnose everything in the DSM-V and ICD-10 and I work with psychiatrists who medicate (often unnecessarily) and do not offer psychotherapy. As a psychotherapist, I work effectively with far more than just anxiety and depression and it is also my experience that psychologists often do not practice psychotherapeutically.

In graduate school, Counseling, we learned how to make diagnoses. With further training, a counselor can indeed make good diagnoses.

The video was informative, for sure. Though the main message I heard was something on the lines of "don't go to anyone who has less training than I have, they're probably are no good, apart for being empathetic and listen well". Can't help but question this. I can't say that I fully trusted the information presented here due to the style of delivery.

"Research-proven methods" is a joke because research is always tentative and often disproven just a few years after it was published in specialized journals. She is just saying what she needs to say to make sure her professional license is safe.

This is an important topic, and useful to illustrate the differences between the psych professions. It’s not necessary however to make sweeping generalisations and denigrate the psychotherapy and counselling professions to make these points. As a practising psychotherapist with over six years of training, I’d advise watchers to research widely to get a balanced overview.

I find this video. hard to watch for how condescending it is towards counsellors. To be honest, I find that psychologists have only ever tried to fit the people I know into a diagnosis or box and lean far too heavily on their academics, and less on a human centred approach. And they have only felt relief and progress when they had this. Maybe you need to have a look at your own values and beliefs around this and why you felt it so necessary to speak down on this profession and talk about your education so much. Education and achievement does not equal connection and experience.

Perhaps speaking out her mind might compromise her professional license: It would be like this here in the US. One can lose one's license for tiny mistakes or saying the "wrong" thing.

I found this video quite uneducated in the way that she perceives counsellors as well as psychotherapists? I think it would be good for watchers to have a look on the BACP / NCS websites with how counsellors and psychotherapists are qualified as this is not fully accurate. Also, perhaps looking at research and evidence of how PCT and integrative counsellors are highly trained (at least 4 years of intensive work and placement that I would class as highly trained) and are highly beneficial to individuals in sometimes a non-directive and (one of the things Becky was right about) non-diagnostic way. To all student psychotherapists and counsellors - I hope you are not disheartened by this video. You don’t need a doctorate to be able to help clients with their emotional well-being and mental health in the exact same incredible way, but it all depends on what the clients wants and needs and the severity of their mental health concerns. Psychologists / psychiatrists can’t always do a counsellor’s/psychotherapist’s job and vice versa. I have seen positive outcomes for all my clients and students over the years and love watching them flourish. This was quite condescending and at times incorrect. It’s a real shame

this seemed pretty condescending, psychotherapists at least in canada have to get a bachelors in psychology and at least a master’s in psychotherapy, then a doctorate in psychotherapy. I have never heard of a psychotherapist thats become one in a few weeks.

I wished the US training is more like England. We are required to do a 4-year undergrad, 2-year master's, and 3-4 years to acquire a doctorate training, etc... such an overkill. I disagree with you about the definition of counselors. Here in the US (for US viewers: LPC, LMFT, MSW, etc...), counselors are clinically trained and follow evidence-based techniques. In addition to getting the certification, you are also highly encouraged to get certified in specific techniques such as EMDR, Brainspotting, etc. These certifications are out-of-pocket pay, or your practice will most likely pay, which only counts after you graduate after your master's. I'll also add it takes 2000-4000 hours to get the actual license.

This video has really good information and content of breaking down each department of psychiatry, psychology, counselors etc. yet again how this person was ranking and reading each profession I found that very unprofessional. All of these people that worked very hard to get in their career to helping people. Yet a person ranking them down going by how many years of experience and education you have gotten yes outburst. Don’t disrespect other peoples careers titles.

I'm appalled by the down grading of the profession of counsellor or psychotherapy. The training personal and professional development is rigorous as its not restricted to 6- 8 years of academic study. A psychological counsellor or psychotherapist develops as a practitioner in the field. Therefore a newly qualified counsellor at diploma level would have a years supervised clinical practice. This guidance is misleading. Counselling skills is relevant for most professionals who work with people that does not make them a professional counsellor.. The other thing not mentioned is the entry requirements for a post graduate diploma in counselling is 25 years old. Psychology training now starts at 16 years old. Counsellors and psychotherapist benefit from life experience. A qualified psychologist could be 25 years old. In my experience the mature experience counsellor psychotherapist when a therapeutic alliance is formed is just as effective in helping clients meet their goal.

I’m currently doing a undergraduate in psychology and counselling with the intention of becoming a psychologist. Prior to this I done 3 years counselling skills. I think every person who study’s psychology should have to do a level 3 in counselling skills so they can learn more about themselves in order to help people. Psychology alone is simply not enough. Counsellors deserve way more credit that this video gives. There training is so intense! As Carl Jung’s would say “beware of inflation”.

Hi, personally I have three friends who are qualified psychiatrists AND have either completed psychotherapy training or currently doing it. So, it is not necessary that unusual especially as a number of psychs are no longer content to just keep prescribing! None are EMDR people but include psychodrama, TA and Jungian.

psychologists in general do not really understand the training of psychiatrists...they often have a lot of opinion on the topic but they don"t typically spend enough time with psychiatrist or really do even basic research on what medical training means.

Your friends are the best of all psychiatrists. And, yes, Dr. Carl G. Jung was indeed a medical doctor. I had myself a Jungian analyst who had been a surgeon, I was amazed at how he interpreted my dreams right away. I can interpret dreams but not on the spot, I have to do my research.

I absolutely love Psychology, but the whole politics / qualifications debate is really tedious. A Clinical Psychologist with little or no empathy may do considerable harm, a counsellor or volunteer people helper might have a hugely positive impact on many people's lives. This video takes a very orthodox, hierachical, and simplistic approach to supporting mental health which, in practice, is disparate and complex. No mention of psychiatric nurses, for example, or the heroes that volunteer time to support disenfranchised young people.

Very inaccurate. Psychotherapists diagnose the full DSM-5, not limited to anxiety and depression. Many other errors as well. Almost every other sentence seemed inaccurate.

It appears that the entire video is, in fact, an advertisement for the CBT brand. It seems that the person behind the video projected their shortcomings onto their colleagues, criticising approaches that differ from clinical psychology and CBT. This raises concerns that clinical psychologists and CBT therapists are not required to engage in personal psychotherapy, such as the requirement for psychoanalytic or psychodynamic clinicians. This is to gain better self-awareness, acknowledge their limitations and blind spots, and manage their biases better when working with people's mental health difficulties. There tends to be a reductionist biological approach, which I think atcs as a defense to the complexities of someone's mind and mental health, which can at times disarm professionals in many ways. Additionally, it is helpful to read the primary research sources on the so-called "evidence-based" treatment approaches to understand that the RCT's that back their helpfulness are flawed, misread, misquoted, and have been funded by parties that have interests in their interventions being regarded as providing positive outcomes. Research shows that short-term (average of 4 sessions), heavily scripted and manualised therapies fall short of providing long-term improvement because they operate in a fragmented and compartmentalised way, detaching the symptom from the person as a whole, i.e., the combination of attachment insecurities, personality quirks, unconscious conflicts and psychological defences that cause most people to experience anxiety and depression.

How does a psychiatrist know if a client needs medication or what their diagnosis is, if they don't directly do talk therapy with them?

They would conduct an assessment. Talk therapy refers to structured or manualised psychotherapies (i.e., it's not the same as having a general discussion with a therapist/clinician). Generally, the clinician/therapist would conduct an assessment (and potentially make a diagnosis) to inform which treatment modality is going to be most effective.

Psychiatrists are known for making mistakes in complex cases, but under the law misdiagnoses do not constitute malpractice unless a suicide or murder ensues.

@@RicardoMartinez-oh9sq That's not true. In most cases, there is no way to unequivocally confirm a misdiagnosis. It's one professional opinion versus another professional opinion. If a client died by suicide or committed a murder, making a mis(diagnosis) would have no bearing on whether a clinician was legally culpable - they are only culpable to the extent that they failed to take reasonable steps to prevent foreseeable harm.

Psychiatrists in Australia most of them are former GPs. Being a psychiatrist is a lucrative business because it is less challenging. Easy money! They just prescribe drugs and no deep investigation in to the causes of individuals mental health and they are lacking counselling skills. These assertions are based on stories I heard from some psychiatrists and some people who were suffering from complex mental health issues.

You could have a private practice and do talk therapy, or specialize in psychoanalysis or Jungian analytical psychology. I once literally paid hundreds of dollars per session to a medical doctor who had not specialized in psychiatry but in Jungian analysis. Here in Ohio, USA once a medical doctor who was not a psychiatrist treated me for anxiety with medications.

This information is not only very biased against counselors and therapists; it is only good for UK only. Here, in the US the gradation and training is different from the UK. Hence, anyone who seeks mental health therapy can go to a licensed counselor and receive very high level of services.

Dr. Beckly Spelman would do well to provide a correction as an update to this video. Specifically concerning the information provided about "counselors". Also, I'm noting that she practices in the U. K. My comments are strictly based on my experience in the U.S. What is described as counselors in this video, equates to psychotherapists (not M.D. or doctorate level) at the masters' degree level. LPCC - Licensed Professional Clinical Counselor; LMFT - Licensed Marriage and Family Therapist, and LCSW - Licensed Clinical Social Worker. All three licenses are credentialed to diagnose and treat mental health conditions. They cannot prescribe medications. The California Board of Behavioral Sciences (BBS) has more information.

6 years or they are not going to be good enough?! A BABCP accredited CBT therapist needs a core profession or a PWP qualification, then can go on to do a CBT training programme which if done intensively can be completed in 1 more year. That’s only 4 years… for an accredited expert in their field. It doesn’t matter how much time you spend studying if you don’t even understand the basics of your field of study. Don’t make videos about things you don’t understand, it’s very misleading.

The BABCP training for CBT is one year at most universities. I was a bit confused when you say they need to have 6 year training 🙂. Also some universities (e.g., Oxford) teach a trans diagnostic approach and more complex presentations of CBT. Great video by the way. Really helpful. I have copied the link and have forwarded it to some clients. I think the terms psychotherapist also needs to be protected but I am not sure how that would happen with all the different modalities.

Counsellor typically is for short term interventions and the psychotherapist deals with long term issues. The definition is totally different a BACP approved counsellor and psychotherapist can deal with moderate to severe mental health issues. Whereas a clinical psychologist or psychiatrist deals with clinical mental health issues and diagnosis but in the Uk psychologists don’t diagnose they test or assess and collate evidence to support a psychiatrist to make a diagnosis. Psychologists don’t diagnose mental health disorders only a psychiatrist does.

Psychiatrists have a veneer of science and, at least here in the US, they believe they are above both psychologists and counselors: Untrue. In graduate school, Counseling, for instance, I learned that particularly complex diagnoses might require the use of the Venn diagrams: Tell that to a "shrink." Generally speaking, psychiatrists tend to disregard learning disabilities because they seek the "disease": They may not include them in any of the axes of the DSM V. Particularly, I find it ridiculous that within five minutes, the average psychiatrist's assessment time, psychiatrists can be so sure of their diagnoses, as they feel. They are very good at differential diagnosis and believe this diagnostic tool to be infallible, but it is not. To make sure, the law protects psychiatrists: Substandard as some of their diagnoses are, under the law a psychiatric diagnosis is merely a medical opinion. Only in extreme cases involving a murder or the like medical malpractice in psychiatry can be proven. Some diagnoses requiere, instead, differential etiology but psychiatrists do not want to hear about this. In sum, like one of my class instructors in graduate school told us the class, there is no such a thing as mental health diagnoses: "They are mere sketches that we mental health professionals use to discuss our cases," he told us. But psychiatrists would laugh at this notion.

There are some broad generalizations here that are true, but all of the information presented here is specific and narrowly true for the UK. I’m a psychologist in the US and much of this is not true here.

Correct. I know of counselors here in Ohio who partner with medical doctors to have a private practice and can be the best mental health professionals.

Hi Becky what about psychotherapist and neuroscience. I believe the mental health industry is much better in UK but Australia is shocking... I've been diagnosed with everything and over medicated etc etc bad experience. I do now see a great psychiatrist who did that extra time in talking therapies. From just listening to me for two hrs his feedback and was able to identify behaviour traits and he said I wasn't born with any clinically mental disorders. It was trauma! Which I kept saying that to all the other specialist I had seen. I really want to get into the industry I am a yoga therapist and already have study for 5;years in that area but I'm looking at psychotherapy/neuroscience. I have done course in CBT DBT etc ...and obviously with my yoga background. Do you think it worth me doing psychology degree considering the time? Or do your think what I was taking about doing the neuroscience and psychotherapy would be a good option. It accredited and three years. My difficulty with mental health is obviously every other specialist looks at the organ to then diagnose where with psychologist you dont! And for this reason I've been diagnosed wrong and it's caused my recover to be hindered. I think the brain is complex and like addiction for example we CAN NOT cookie cutter them. I know addiction is out of control in Australia and the solutions work for some but majority don't and they end up accepting being an addict and all the repercussions of that or dead from over dose or suicide. We are frighten from covid but addiction is even worse way to die and generally with this attitude or culture " they did it to themselves" if you actually asked any addict word you this life I would imagine none of them would say yes. It's very sad. Sorry for passionate about this subject being through trauma and then self medicating because I felt the mental health industry couldn't help me. It wasn't lack of trying. Thankfully I was able to find that person who could pull me out but it was most horrendous experience of myself and I hope nobody ever experiences it.

I find this video slightly confusing, in Australia you do not have to do a PHD, to obtain qualification as a Psychologist, you can of course if you wish, most at CBT orientated, not all, Psychotherapy again is also NOT all Psychologist's, I am at a loss why you said that. We Psychotherapist's in Australia do not diagnose, we are not counsellors either, we work one on one, in resonance and response to our clients, Counsellors also are an important part of mental health help, I take exception to the up and down the list, each profession has it's values.

@@Petuacapturesit In Australia to be completely honest, you need no quals to call yourself a Counsellor, or Psychotherapist, the names are not registered or protected, like Psychology or Psychiatry, Having said that, qualifications gained through education from the providers, whom meet the standards, will always enhance your skills, I did a dip couns, at age 55, then went into private practice for two years, then completed several post grad, with RPL, and 6 different sub courses in other modalities, at 62 I am still learning, and come done on the side of Gestalt Modality..

In short... Whatever you do, dont go with the Psychiatrist who will only guarantee one thing; that you will get at least one prescription after first visit.

Hi. Nice video! Creative arts psychotherapist here. Patients that I work with daily report that creative arts therapy is a more "advanced" form of psychotherapy for them over conventional talk therapy.

From one of your videos you mentioned I could easily get a doctorate without doing a masters - I want to also run a private practice I am board certified from Sydney Australia as a welfare worker and have placement hours plus work experience too. Hi Ashaab, Look into psychology doctorates to see if you meet the criteria for the course. You can do a psychology doctorate without a masters if you hold a psychology undergraduate degree, if you do not have a psychology undergraduate degree you would need to do a conversion course in psychology such as a masters before applying for the doctorate programme. Hope this helps!

Hi Dr Becky! not sure if you will see this message as I am a year late but I appreciate this video and was in need of some advice if you could offer any. I'm really interested in the counselling aspect and would love to be there to listen to peoples issues ect however I also want to learn in depth details or strategies that are in line with some of the best researches but I also don't want to prescribe people medication ect or work in that field so I'm a bit confused on what route to take.

There's no regulations of such to be honest you can actually be a counsellor and a psychotherapist in one month if you do an online course in pay for it you can do online courses in counselling and psychotherapy they give you one year to complete it but is at your own pace and you can do it all from home. You can also get insurance and practise as a psychotherapist and a counsellor you can have an endorsed qualification. And be working self-employed. That is all legal. I myself had psychotherapy and it was rubbish the therapist had no empathy and she was unprofessional and I've had counselling and I have the best experience so please don't downplay the councillor

I Becky I have a Bachelor degree in counseling and health psychology, A master degree in health psychology from a broad. Also diploma in CBT from london.and diploma in mindfulness and hypnosis. I live in London South East, I studied all the mental health illness including the symptoms and the duration...I did psychometric tests and assessment all in my country but I am really confused when I come to London about which job is suit my qualifications? You need to check with the graduate schools. Some might want you to take undergraduate pscyhology classes before you undergo a master's in counseling. But for a Psychology PhD program you would not be admitted unless you have a B.A. degree in psychology, social work or the like. A PsyD program, maybe.

Of course, here in the US your license as a psychiatrist qualifies you for psychotherapy. Find out what area of expertise you would like. You can read books by S. Freud and Carl G. Jung, for instance, to find out if you like any of the two, two amazing psychiatrists and psychotherapists. You could also specialize in humanist psychotherapy. Pick the one you would really love. Incidentally, by wanting to go into psychotherapy you are showing what most psychiatrists lack, such as deep empathy.

I hate that an American Licensed clinical social worker can't practice as a therapist in the EU/UK. It makes no sense why a person who has four years of clinical training in therapy and DSM 5 diagnosis can't do therapy and do diagnosis

Great to hear you wish to pursue psychology as a career, you can definitely work with children either as a child psychologist or an education psychologist. The most financially fruitful way would be through the clinical psychology doctorate as this is funded in the UK which means you receive a salary during your training.
