All the minions he was punching were damaging his face as normal. When he punched the Voidcaller, he went down to 5 health as it summoned Mal'ganis, rendering his opponent immune, and thus taking away Mark's lethal. He could've stopped swinging, but it's likely his opponent would've been able to win anyway (they just would've had to remove the 5/5 taunt), so he decided to finish himself off by punching Mal'ganis as well.
