Also, the Emperor made his suit as clunky and painful as possible, purposely ensuring that he'd be essentially crippled and in agony all the time. This was a punishment for his failure, as well as an attempt to force him to overcome his injuries, as the pain and frustration helped feed Darth Vader's connection to the Dark Side.

They say it was his suit I guess mistranslating it? But I think other loose material has noted that in reality Vader inside the helmet is speaking at a volume no louder than a whisper, so that means that when he did that, he was actually quiet af, due to the internal damage, vocal cords, lungs, even stomach, so how he's able to speak and whether the suit had to translate for him is up for debate as at the end of EP VI he speaks at an audible volume just fine, it could be because before the bacta treatments that Rogue one mentions about, he's not anywhere near the process of healed yet.

Well, let's not forget that even in the book it was said that when replacing his organs with non-organic ones, he was on the verge of death, because of which the Medicin droids replaced all the organs with what was at hand, in the book Darth Vader explains that his helmet is so old and not a worker that he can only see the world in ultraviolet light, and his legs and arms have been replaced with parts so heavy that he has to use his power to just move around

Fun Fact: Vader’s suit is actually very front heavy and he’s always close to face planting, is always in pain because it cuts into his burns, and has needlessly heavy prosthetics, etc etc. Palatine made it to be torture to remind him of Kenobi and to have full control of him. If he upgraded his suit most of his problems would be solved, but he’d likely die in the process so he can’t even do that

And then we are shown in canon that he can use one, though only for minor healing. Vader actually prefers his MK1 ensemble because the pain and feeling of isolation binds him closer to the Dark Side.

Fun fact: The Medical droids were perfectly capable of making Vader’s breathing silent. Palpatine insisted they make his breathing audible. This was because Palpatine wanted to test the limits of his new Apprentice. We already know the suit was designed to be painful, so that it would fuel Vader’s rage, but the fact he would constantly hear his own breathing was intended to drive him mad. I mean just imagine him trying to sleep but he constantly hears himself breathing. And it’s not like he can just stop doing something his body automatically does.

Cool facts about his breathing sound: While it is well known that his breathing is from scuba gear, it is actually made by breathing out of the respirator above water. This makes it easier to hear his different breathing sounds for in/out breaths. The damaged Vader breath sounds awfully like scuba gear underwater. Don’t know if this fact is real, but they do sound alike I think.

Vader could still talk... when he told Luke to take his helmet off to see his son through his own eyes (and not throught the helmet). When Luke took his helmet off, Vader could speak normally but a more older and weaker voice since he got electricuted by Emperor Palpatine.
