Honestly, I feel like you're not giving the Hyenas enough credit. Shenzi's basically the most intelligent of them (she'd have to be, seeing as hyena societies have female leaders in real life), and don't forget, they gave Scar the idea of killing his brother (indirectly, through Banzai's sarcasm) and scared the wildebeest herd that ran over him directly.

Joanna from The Rescuers Down Under is an interesting case, as she goes all around the first three categories. At her best, she's a tenacious brute who foils Cody's escape plans (and is able to swipe McLeach's eggs). However, she tends to mess up a lot and, at the very end, she indirectly causes McLeach's demise by bumping into him and causing him to teeter off the cliff into the river (with Bernard giving the final push).

The Hyenas should have get a higher spot. Disney wiki himself said their failing of tasks are more the results of the Protagonists luck or small size than actual incompetence. I know some Disney Henchmen you guys missed. There's Doug the Ram, along with Woolter and Jesse, all from Zootopia. And then there's the Willie Brothers from Home on the Range (unless they’re means to an end just like Cinderella's Stepsisters), FoulFellow and Gideon from Pinocchio (who are, no doubt, accomplices to the Coachman), Roscoe and DeSoto from Oliver and Company, the Huns and/or Hayabusa the Falcon from Mulan, Brutus and Nero from the Rescuers, and Joanna from The Rescuers Down Under. Is there any reason they're not included on the list (not counting the fact Joanna is a sequel character)?

I wouldn't say that Malificient's goons are useless, more like their boss is the primary idiot. These are non-human creatures of fey origin, similar to her. She didn't think of informing them of all the knowledge on humans they needed, and didn't check in on updates, and updating them on anything. So it's technically her fault. It's kind of the opposite of the Horned King. He actually valued competent minions.

Diablo is actually scarily competent, if he had been set out to find Aurora from the beginning, Maleficent would have won easily. The fairies had basically nothing they could use against her, and Phillip was only barely able to squeak out a win with the magical weapons the three fairies gave him. Even in death, Diablo was able to not only warn his mistress, but, in subsequent media, even resurrect her by himself. If Diablo had found Aurora when she was a baby or a toddler, it would have been over right then and there. Growing up, I never realized just how close Phillip's victory truly was over Maleficent. Petition to nominate Diablo for the most competent and dangerous Disney minion. Imagine if there was a crossover between modern Disney heroes and princesses and classic villains in a Mortal Kombat style death match. I'd like to see modern Disney heroes and heroines take on the oldest, rawest villains in animation. Just a thought!
