Thanks for your question. It usually comes down to preference (whichever approach the physician is most comfortable with). In some patients with advanced patellofemoral osteoarthritis, it may be quite difficult to palpate the medial retropatellar space, so I would usually go with the lateral approach for aspiration in those patients. I was at the orthopedic doctor office and they said they were going to do this injection. She made it sound really scary but this video helped! When she actually did the injection they used a cold spray just before they inserted the needle and I didn’t feel a thing :)
