MRI takes advantage of the fact that the hydrogen atoms in water are in different chemical environments all over the brain, not just in fluid-filled areas. Anywhere there is a water molecule, that area can be imaged with MRI. It is not confined to fluid-filled areas, as there are water molecules all over the brain. The differing environment of the water molecules in the brain gives them different relaxation times in the static magnetic field when the radiofrequency field is turned on and then turned off, and those differences in relaxation times are what is mapped to the grayscale image.
