Hello, what did you use for your ID or passport surname? If married name, use it and attached your married cert. Your unmarried name in diploma you'll be ask in form 2&3 of what name written.

Hi, in my opinion I think you don't need to fill in form3, no notation in the general requirements of NY BON about it.. but to be sure you can contact the NY BON directly through email or call 😇

hi! good day. question lng po. nung nag inquire kc ako sa school ko thru phonecall ang sb lng sakin is tor and rle ung irrequest. then nung nagpprocess na ko, hnd sure nung kausap ko if need ang diploma sa requiremnts kaya medyo bothered ako bka kc mag incomplete ung docs ko pag natanggap ng nysed. ask lng if need tlga ng diploma? tia po.

Hi, this is the instruction in NYSED site.. The school of nursing must return the entire form in a sealed official school envelope along with an official transcript directly to the Office of the Professions at the address at the end of this form. If the transcript is not in English, a qualified translation is also required. For information on what constitutes a qualified translation see our website.

Hi I am writing to seek some clarification while applying for registration with NYSED , Pathway 5 as a foreign registered nurse. Do I need to scan and submit all my certificates from higher school to school of nursing or just school of nursing diploma with my application. I am currently a registered nurse in UK, can I declare my registration under experience or its not required. I also want to find out can I do mandatory certificate for infection control after registration or is it needed before registration. Thanks

Hello, Before you proceed with online application Form1 it's much better if you scan your highschool diploma, nursing school diploma and other certifications including your child abuse & infection control certification, you will upload them. You may check the Ceufast site, input New York state and it will appear the need to take of these 2 course. You'll get certificate sent to your email once your done.😊
