Great video Dr.Biernacki. My question for you is, if the taping holds down the 2nd toe, doesn't that increase the impact/loading force during "toe-off'? From an engineering standpoint, my perception would be to tape up and over the foot vs. under. I scanned back and forth over that section of the clip but there was only background music so I didn't really get how taping downwards changes the force distribution or has an arch-itectural (pun intended) effect. Thanks in advance! I'm dealing with a momentary sharp shooting pain right into that toe on getting out of bed and it's beyond frustrating.

The ligament holding your second metarsal phalangeal joint together at the bottom is called your plantar plate. There is some overlap between a plantar plate year and capsulitis. This tape job holds your toe from bending too far upward and hyperextending that joint! Good luck!

You are the first doctor to describe to a T what’s going on with my foot! Thank you very much! Mine seem to have started when I wore a new pair of (cute) shoes for the first time. And that was 15 months ago. Since I have to walk a long way just to get to my desk, I’ve resorted to wearing practical Ecco shoes with a metatarsal pad. The base of my second toe is curved downward whereas all the others are in normal position, hence the pain. It seems to me if I had something to support the base of that toe to push it UP that could remedy the situation. But I’m not a doctor! I’ve been to two ineffective podiatrist’s so I’ve resorted to finding yoga routines however there seems to be no effective ones that I have been able to locate. I have even resorted to my tens unit but that doesn’t work either. ¯\_(ツ)_/¯ I am 64 years young and don’t have time for this! I have too much I want to do in retirement to include tap dancing again and I don’t want this to sideline me! Your suggestions are greatly appreciated!!

Usually the metatarsal pad isn't enough, you need a solid orthotic with a metatarsal pad built in plus a good shoe like a Hoka may be what you need. www.michiganfootdoctors.com/recommended-products/

We would recommend a shoe like the Brooks ghost with a good pair of over the counter orthotics. Generally if there is too much pressure on top, the shoe is too tight. Potentially go up half a size to a size.

Michigan Foot Doctors I’m watching more! My Morton’s toe is turning into a claw in part because of osteoarthritis not helped by my years of dancing and running). I’m trying to avoid surgery. This in particular is a big help. There’s a study that found that consistently taping can make a permanent change of a millimeter or two. I’d rather try that than have surgery! Looking for more videos. Keep up the good work!

I went on a quest to grow my calves. I destroyed them every week (reps to failure). After about 4-5 months I started having some pain in ball of foot. I think I have capsulitis. I hope this works for me.

This actually made a difference for me. Do you just keep it on when walking or sitting, but take it off at night when you sleep? Also, would the toe separator big toe / 2nd toe spacer thingy be a good substitute? Or maybe one of the others you have listed in the links? I'm currently working through this after seeing my podiatrist and it's exactly the same as you talk about. I have the strong sturdy shoes, the orthotics that raise my arches and also put a small lump under my affected foot base and also the tape. It does make a difference and I'm also stretching my calves etc plus doing self foot massage. Great video!

Please - any recommendations for a shoe - OTHER than a running shoe with a stiff bottom and a rocker movement- that will be able to be worn when weightlifting. A training type show that will offer stability too. It seems I keep coming across (even podiatrists I have gone to) shoes like Hoka that I feel like I am on a platform (not good for training). And secondly, any recommendation if I wanted to purchase a custom orthotic with the pad in the front like the one you show? To alleviate this issue I would spend more money for a custom othotic. Thank you in advance.

Realistically I like to perform a biomechanical exam for specific recommendations on the shoe. But yes in order for athlete's my favorite shoes are generally in this order Brooks, Asics, Saucony. For performance running Nike, Brooks are the tops for me, but for toher people you can have great On, Mizuno etc

Hoka for me is great for pain and recovery, custom insoles can be made in person and modified with the pad. Generally the only order version aren't really custom to be honest in my opinion. Thanks!

I had capsulitis for over a year and it improved by using a Metatarsal pad. Unfortunately, I am now developing a hammertoe on the second toe caused by the metatarsal pad. My Podiatrist gave me a crest pad and recommended I continue to tape my second toe, but I don't see any improvement and she says there's really nothing else that can be done. The joint in my toe is changing color, at times it's purple. I'm really desperate for help here in California.

Dam that was very very informative and Excellent presentation. I just started having the problem when the Corvid started in the spring as I wanted to get off my butt and loose weight. I started walking like 3 miles a day 4, 5 KM daily so by June I was in pain daily. I went to the chiropractor, acupuncture, now the the foot doctor next week. Ice at night Ibuprofen, booze, pot. Nothing is helping. I thought It was a bunion but I see it's not, calve problem to tight. Now it all makes sense. I'm only 58 this can't be happening all ready..lol Thank you so much for sharing. Thank you for this video! And, your others. Way too many plyometrics led to capsulitis (or perhaps my bunion?) three months ago. Trying to get rid of it with taping and icing. What else? Ordering the inserts and will try the Hokas. Meantime, is it possible for the pain to radiate up the shin after (light) exercise or moving around the house doing chores?

With a foot that doesn't load properly a lot of muscles can for sure get agitated and this can also lead to nerve agitation. Radiating pain can usually mean pressure against a shoe or a tight muscle aggravating nerves. But get checked out if worried!

Loved your video and your patient kind manner! I think this is what I have - not sure - I had bunion surgery a few years ago and it left my toes very stiff (especially big toe) with a very limited range of motion. I will try the orthotics I had made a couple of years ago (but rarely use) and the taping..I was already doing the stretching. Will let you know if this helps but really appreciate your videos especially since I cannot got out to see a podiatrist now. Thank you !

Unfortunately it is impossible to be specific unless the condition is examined and known, the severity and the surrounding life factors are known. Always get evaluated in person if having a problem.

Just saw your video. Woke up with an excruciating stiff second toe yesterday. I work in a foundry on concrete all day and then usually 4 to 6 hours in the garden, pasture and around my place getting things done... so im on my feet approximately 17-18 hours a day. After i watched your video, I immediately grabbed the duct tape and followed your instructions... my foot and toe is still sore, but not even close to the pain i was experiencing! You were terrific!!! I looove your tutorial! But yessss I do hv 2nd toe capsulitis in joint no.67 I believe acc to your numbers on the foot u had in your hand. And thanks to u I’ve learned something I never ever knew! If I could hug u through my iPad I would🤭🥰but my second toe DOES NOT GET LOST UNDER MY BIG TOE! But I got swelling in that bone joint n it’s hard meaning the bone is truly inflamed n yes I am very very heavy w/controlled diabetes 6.3 but I am seriously on a diet bc I no toes can’t afford to be cut, lanced or anything! So that tape thing u did doesn’t wk for me nor fix my problem so please keep in mind that every body’s second toe with inflammation does not hide under the big toe. Mines however is slightly bent n so r the other two toes on my left foot.🤦🏾♀️So if u can plz help me as I do not hv 45.00 copay as a 65 yr old senior to see the foot doc. But may hv to go anyway. thanks, I have had this problem for a couple years, I have been to the foot dr twice, given a shot, and told to tape my toe..but never like that, great explanation, and my calf muscles are really tight..which could be some of the issue..I didn't realize that these were all connected... So, what are your thoughts on barefoot/minimalist shoes? Been running in them for around 7 yrs now. Had capsulitis in my right foot at the beginning of my transition to barefoot (I would alternate between traditional and barefoot shoes at the beginning and helped resolve it by having only barefoot style shoes). Now I am almost positive I have capsulitis in my left foot now, excruciating pain if I try to bend my 2nd toe up. I run at least 3 days a week. How long should I take off of running? I am doing stretches, taping it, using toe spreaders when not in shoes.

Hellooo doc! Thanks for uploading all these helpful videos. I fractured my 5th metatarsal three months ago. It has now healed, but I'm having problems with my toes. My big toe now seems to be much further away from the other toes than it was before (the opposite of a bunion), and my first toe pains after I go out for walks. My calf atrophied due to use of the boot, and I'm trying to strengthen the muscle again. As the fracture is now healed, are the problems with my toes related to a weakness elsewhere caused by the lack of activity when earing in the moonboot? I would be extremely grateful for any advice.

The ball of my left foot flared up after doing a three mile walk on hard sand at the beach, I bruised it pretty good, thought it would go away but didn't. I've seen a couple of podiatrist; one cut a pad then taped it, I did this for a couple weeks but wasn't worth the hassle, so I went back and received a cortisone shot that the doctor injected from the top of my foot down thru all of the tendons and nerves and I think it caused nerve damage; second podiatrist prescribed arthritis medicine and gave me a shot directly into the ball of my foot, the shot alleviated it some and I never took the medicine. Both doctors say this happens as we get older, I said nah your lying. lol So, I started wearing a silicone cap over the second and third toe, as the third toe is bent now too, both have formed calluses from the rubbing. I have flare ups every now and then but it's just toes, it could be a leg or something then where would I be. I believe it's capsulitis, bursitis, tendonitis, all of this combined with swelling and bruising isn't going to go away unfortunately. I would love for my toes to straighten back out but will the bruising and swelling ever go away for that to happen?

We would love to be able to give you a more specific answer, but unfortunately we cannot do so. These are very complex medical problems that do require specific evaluation in both physical and imaging modalities. Please see a podiatrist near you for further evaluation. We would be doing you a disservice with general advice for such an important issue. Your podiatrist or primary care doctor can likely help much more than we can in this instance. We really do appreciate you as a fan and we hope to bring you more detailed videos that help more in the future!

thank you for this information. You are awesome. I am an avid tennis player trying to avoid surgery for a torn ligament that was caused by my bunion. My doctor has me using a small device that slips on my 2nd and 3rd toe to help relieve the pressure on the 2nd toe, a sports orthotic and NB sneakers. You reference a small gel device to stop the big toe from going under the 2nd toe. The link to that product now references a product to protect the bunion instead. Holding down the nuckle with prevent the flexible hammer toe from developing. But more so this needs to be combined with addressing the underlying root causes.

Why the recommendation of overengineered shoes? Those types of shoes (arch support + high drop + narrow toe box + pronation control) cause foot/ankle/calf atrophy and structural inefficiencies leading to pain in the foot, knee, hip, and back. In other words, those shoes cause collapsed arches, lack of ankle flexion, hammer toes, anterior pelvic tilt which all have the potential to irritate an MTP joint. In addition to the recommended increased ankle flexion mobility, shouldn't MTP joint patients be strengthening their foot/ankle/calf/tip/fib by incorporating a minimalist shoe that allows for the engagement of muscles responsible for proper alignment of the ankle and foot? Otherwise the engineering of those shoes does the work for your foot and because there's no longer a neurological stimulus to those muscles, they become weak making the foot/ankle unstable, weak, and prone to injury.

In short because they work extremely, extremely and undeniable well and there is no strong evidence saying they don't. There are some pro barefoot articles that are small in number, lots of people sell barefoot shoes and promote barefoot stuff but the bottom line is these supportive shoes work extremely well and fix the pain. It's cool to be barefoot but the ugly shoes will help your pain in the vast majority of these cases.

Great video! Never knew this was a thing until I decided to switch speed work shoes and now I have this! It’s very frustrating. I see rest is a way to heal this…. How much rest are we talking? I’m an avid distance runner and am in training. I took 3 days out in a row and that felt long.. and nothing improved haha! ;)

There are usually better fixes such as orthotics and really good shoes. But taping can be of help for some people.
