Ball of the foot pain can include Morton's neuroma, capsulitis, plantar plate tears, stress fractures of the metatarsals, and ankle joint tightness. This is most common with running and walking. It can even happen in the morning after sleeping at night.

I agree! I saw a podiatrist and his first suggestion for me was a cortisone shot, he was shocked when I said no. I went for a 2nd opinion with another doctor, who gave me suggestions to reverse my issues!

@@DrTomBiernacki Hi Tom. I am so pleased by how much support, knowledge and information that you share on KZbin. I am 70 years old and live in the UK. I have suffered "Ball of Foot" pain for 10 years, including 2 sesamoid bones removed and have a metal plate fitted along the big toe (left foot). Many cortizone injections etc. I am taking onboard the contents of video "Metatarsalgia Treatment. I just wondered what your thoughts are about TENS machines. I have just started using one to give massages and improve blood circulation. I have also considered "supplements" such as Devils Claw and Montmorency Cherry Tablets. BTW great videos and advice

So I have been literally limping around with foot pain for weeks! I watched this video a couple of days ago and did what he said. I did the ninety second stretches and made pads for my shoes out of duct tape And shoe inserts. Omg today I am WALKING! It still hurts on the side of my foot but it is soooo much better. I can hardly believe the relief I feel. Thank you thank you thank you!!!

Been dealing with this pain in my right foot for months now. It comes and goes though. I do have really tight ham strings and a ton of lower back pain. I'm not on my feet a ton. I do travel for work so alot of miles driven. I will be losing some weight, couldn't hurt eh?

Finally someone that understands the feet. I had this come on all of the sudden and couldn't understand why. I came to find massive trigger points in in my posterior tibialis and lateral superior part of the gastrocnemius. They were only on the leg with the ball of foot pain. Thanks man!

This is such a great video! I’ve been suffering with metatarsalgia and plantar fasciitis all summer, I work in a nursery and walk about 20,000 steps a day. Going to take this advice to heart!

This video is great! I’ve been dealing with this issue for 20 years now, the only thing that allowed me to walk was custom old school orthotics. No doctor ever told me about stretching out my Achilles’ tendon. My orthotics maker has retired and now I’m on my own with creating custom insoles, I do most of what he talks about already. The one thing he didn’t address is wearing shoes that are wide enough! When your foot sits on the metatarsal pad, it spreads the foot out.

You truly have made my life so much better. You gave me back my quality of life. After suffering with foot pain for over four years, your video reversed all the pain. Doctors and podiatrists tested me for neuropathy, Morton’s neuroma, vascular problems, etc. Your simple instruction for using a metatarsal pad under an insert has stopped all my pain. This video inspired me SOOO much. I was thrilled when I picked up my newly purchased NB sneaker that set me back more than $100 and realized it has everything the doctor said. I also was mis-placing my pad. I literally cannot wait to walk tomorrow. I had a bunion removed almost a year ago and did not have therapy, subsequently "froze" my big toe in an upwards angle. I have gained 20lbs and have been miserable because it feels as though I am walking on a marble, so I walk less. But NOW! Thanks Dr. Tom.

Thank you! I found you by accident and am so happy I did. I am scheduled for an ultrasound as my Podiatrist feels that possibly there is a ganglion cyst in the bottom of my foot around the area that you explained (metatarsal). I was a runner and for the past 5 years have replaced running with walking (I am 65) which I have enjoyed until this pain started. Your video really explained things well and I definitely need more stretching and really hope that this will help.

I thought I had plantar fasciitis but my heel doesn’t hurt. Then I found out about this diagnosis and it’s exactly what I have. Been trying to walk for exercise to lose weight and my feet would just go numb. This is the best video I’ve seen on the subject. Thanks for all the great tips!

Thank you! This was amazingly helpful. I spend 10 hrs a day on my feet on hard floors and I was suffering. I learned to pick a quality shoe and the right, quality insole....foot pain is now gone.

@@juicegrape1494 Yes, if I actually would do them. I use good shoes, and try to stretch all the time now. I tried the pad at the bottom but had trouble finding one that would stay put and I'm not really sure I know where to place it. But what I tried seemed to give relief. My feet do feel better since I posted. I have not kept up with it, and really should to keep my feet happy. Your post actually is a good reminder to me that I wrote that post! (I was going to lose weight! I did not do that.) But it's all good information.

I've been to two Podiatrists over the last 2 years. Saw a little relief from the first pair or orthotics, then they got messed up due to some really bad adjustments the doctor made on the fly. Got a second pair of orthotics from a new Podistrist and I still can't even wear them because my Metatarsalgia hurts so bad.... there's just not enough support to offload. Injections haven't helped either (and boy were those painful). I'm at the point, again, where I'm making my own adjustments to an old pair of orthotics and taking it a day at a time. Just started trying stretches and I'm hoping that will help. This is a great video. I knew most of what was said, but there were some really helpful pieces of info that I'm going to consider and add to my routine. I have 4 kids and this has destroyed a lot of my ability to be a good and active father, as many days I can hardly walk. I need to see some healing and I'm hoping it comes some day soo becasue right now... it's miserable. Thanks for the great video.

Unfortunately there are rules preventing an answer online, and this could be dangerous to your without having specific information. A problem of this nature really does require a proper history and physical. By getting non-specific advice online, it really can make your problem worse. For a problem this complex and with so many variables, you really would benefit most from seeing a podiatrist in person. Thanks, Tom for your awesome advice. I am a 40 year old type 1 diabetic that is active and love the gym. I started getting this blister on the bottom of my foot near the pinky, and then all of a sudden it burst...looking like a gun shot wound. I have extreme pain. My foot got swollen. the doctors in the hospital did X--ray scans, ultra sound scans not showing infection. Now I am waiting on my podiatrist to get me an MRI appointment to see the full detail of my foot to see whats really going on. when I see my podiatrist all he does is remove all the callus and apply triple antibiotic on my foot and it burns with a band-aid. The foot doctor says its "plantar Fasiatist" but I don't think so! I am going to try all these options you talked about in your video. thanks for your great content!

By far the best information I have received with this condition. The pain is excruciating when hubby presses it or even squeezes it .Have recently been advised to take anti inflammatory meds .10 days later , the pain is still there .Losing weight was also advised but through walking , thankfully the punching in this video makes soo much sense..thank you doc , for such am amazing informative and beneficial video to so many ppl ..Having this condition affects all activities, especially if you sit down for a period of time and decide to walk ..omw, mine is on the right foot ..its dreadful .. Thank you again doc ..God bless you..

Oh my gosh! This is exactly what is going on with me. I had surgery to remove a Morton's neuroma and have been in pain ever since! Hopefully your suggestions will work!

I’ve had Morton’s Neuromas in both feet for years. Walking kills me now and the orthotics aren’t working. I’m told the operation may only last a year before the problem returns. My feet are the bane of my life.

My partner is 82 ,very fit. He’s always walked, not overweight but worked on a market all his life now he’s in agony. The people who gave him the insoles through the g.p. Put them in the wrong shoes and various other problems. This has been the most useful advice I’ve found for him, thank you. ❤🇬🇧

love your approach, I'm a registered nurse and I do 12 hours shifts, i'm 53 and I play basketball on a regular basis, it's basketball that is killing my left metatarsal; i'm getting older that is for sure but i'm not too heavy 198 at 6 feet tall. Ill try to loose more weight. I also lift weights about 5 times per week. I don't stretch and I guess I jump to much. Ill try to loose pounds and jump less thank you!

Y'all! I'm pretty sure I had this after several days of hiking. After a few days of continued walking, it felt like I was walking on a rock. I tried icing, frozen water bottle rolls, cbd oil...then, on a lark, I tried those crazy yoga toes things. Holy cow, I experienced instant relief. After one day, my foot is still sore, but 80% better!

I’ve watched other videos and most will only talk mostly about wearing orthopedics as a solution and don’t emphasize the importance of stretching and weight. I got out of the shower one evening and I had this pain, it’s been ongoing and hurts so bad. I think these days with so many having hard floors in their homes, it’s adding to the foot issues. Carpet adds as a shock absorber the hard wood and tile don’t. We don’t wear shoes in our home and I stay home, so my issues are worse. I need to stretch.

Update: I just used some gorilla tape to stick a foot pad underneath my shoe insole. The pain is nearly gone when I walk! Thanks again!

I’ve been diagnosed with PF and have watched many videos on PF. I am seen by Podiatrist. Have been doing stretches, boot, ice, etc. Then developed wicked pain in front pad. The pain radiates up my legs and the pain increases my BP. Wearing orthotic but now wearing New Balance sneaker 2 days. I plan to use this recent additional information. I feel I have stumbled on the right word to describe the condition. Thank you Dr. B.

I literally diagnosed myself last night. I officially have a tailor's bunion 😔 thankfully it is not painful, BUT what is painful is the ball of my foot which I now know the term (metatarsal). I literally went to a foot specialist today for a consult and he didn't say much. He did explain a few things but you explain WAAAYYY more and go into detail. The doctor I went to just said it could be hereditary and explained how my bone is crooked etc. and gave me no other remedy than surgery... which I knew it's not true because I've been on KZbin this whole time looking for ways to fix it. I'm only 24 years old and this is something I definitely was counting on. I've been pretty sad thinking I won't be able to wear heels anymore or any other nice shoes. So thank you so much for all your free knowledge and help!! I truly appreciate it.

Thanks for the video! 23 year old ultra runner here. I've had metatarsalgia in my 3rd head since November and have been resting/going to PT since December. An MT pad and stretching has helped reduce the pain significantly, but the swelling and structure of the heads still does not seem to be right. I'm wondering what other options I might have. I have heard some talk of spinal adjustments helping and was curious about your opinion. From what I have read a 4-5 month (could be more) recovery is longer than average.

The tough part is some people heal almost immediately and some never heal because they can't correct enough. People that take 1,000s of steps usually are the most unhappy as they get older and less flexible combined with less muscle mass unfortunately. Every athlete and runner eventually loses that pop and ability to bounce back and do massive miles as frequently. Sometimes extreme solutions can be replaced with cross training. Maybe triathlons :)

I teach a lot of Dance specifically Tap and Jazz and I also ran for several years using the barefoot running forefoot striking technique to save my knees. For the past several years I’ve experience shooting pain in the ball of my foot specifically at the second and third toe area. I have pretty flat feet and my second toe is longer than my big toe. I’ve watched this and another video of yours and they’ve been very helpful. Based on them I believe I have Metatarsalgia and possibly the tear you describe and also the plate issue you describe as well. I obviously cannot atop teaching dance but did stop running mainly to save my knees. Do you have any suggestions for me?

We would love to help you with your foot and ankle problem, but a complex problem like this really does require in depth evaluation and imaging to make a proper diagnosis. We wish we could do more for you! We would be doing you a disservice with general advice for such an important issue. I'm approaching 40 and i'm in the best shape of my life after a about a year of serious effort. Haven't had my weight this low since high school. But after being rid of my metatarsalgia for about 7 years now all of a sudden it's back. Been working at a desk for 20 years now, so that's not it. I've been playing basketball since age 15 so i know that's not helping. It's hard to swallow though. I'd likely get depressed if i have to stop playing now. The rest of my body feels like there's so much left in it

I’m halfway through my marathon training plan and unfortunately my 5th metatarsal is still aching after a 15 mile run last week 😭. That insert-underneath-insole tip was clever, I’ll definitely try that thanks! (Also when you were holding up that shoe at 5:05 and said it was a saucony, I’m pretty sure that was a Mizuno-Both good brands though!!)

I run at least twice a weak but I only feel the pain once I reach a distance > 12km. Have tried the stretching and all. I start feeling a burning session on the foot thereafter Is that something I should be worried about and what could I do to make it better

It's tough to say sepcifically and an in person evaluation may be needed for specifics, especially with such long distances, we try to be as specific as possible in the video otherwise. Hope you feel better soon!

This video is so helpful! After I started developing pain in the ball of my foot and toes, I needed answers; you definitely gave me that. Thank you. Question: I’m in good shape and already really enjoy HIIT exercises, but I am particularly fond of exercises that involve jumping (box jumps, squat jumps, jumping rope, etc). I know this is probably terrible considering jumping is even more pressure on the metatarsals, but do you think with the right shoes and orthotics, it would be okay to incorpore jumping in small increments? Maybe only 1-2 a week?

So unfortunately we can't be specific to you, but in general with good shoes, modified orthotics, we roughly see 50% improvement per month in chronic cases until it goes away. If someone were to catch it early, it could go away quicker!

That being said, if there is more issues like a Morton neuroma or stress fracture or deeper underlying issue, it could take even longer.

I’m glad you’re feeling better. I never had this problem until I started wearing Vapor max sneakers I’ll be going bk to ASICS or trying new balance. My one foot the ball omg be killing me, a nurse said to use a tennis ball and roll my foot on it, that almost killed me so much pain.

Thank you for this video. At 62 walking is my main form of exercise and dancing is just in my dna. So this pain in my foot is really taking enjoyment out of my life. Also I have a lot of stairs to climb at home and I stand all day long at work. So I am glad for this information and also you have a good sense of humor about it! The bike thing....not sure about that

Dr. Tom, Thank you very much. Great video! Very informative and well done. I developed metatarsalgia hiking a through hike on a very rough rocky trail with a backpack. (The Israel National Trail.) I’m 63 years old. After the trail, I had to stop my daily walks and took up swimming. I’ve gotten good shoes and really good insoles and now after more than half a year later, I can say that my foot is healing! Losing weight is the biggest challenge!

Absolutely, it's always a battle between as much support as possible, increasing flexibility and taking as much pressure off as possible. Enough of that will solve any metarsalgia problem!

Thanks Tom for putting that video together. It basically summarized the countless hours of research I have been doing on metatarsalgia. That is awesome.... Mine is caused by overuse (excessive surfing) and the thought of walking on sand just makes me cringe. I have the stretching down and the pads under the inserts and also a pair of NB shoes that I need to replace. My questions is how can you tell what type of Asic, Hoka etc have the stiff back, minimal toe flex and sturdy middle section? That's the last piece of the puzzle for me. Thanks again a million times for the clear, non-medical jargon video :)
