great vid as always. quick question, the last exercise is actually a lateral raises most do for delts. any advatdages holding the dumbell like "hammer" rather than regular grip (pronated) being done in most lateral raises exercises?

Yes, many people experience clicking and popping when their rotator cuff is irritated. As that irritation reduces, you should notice a reduction in joint noises.

Hi doc I just have a quick question I was diagnosed with mild supraspinatus tendonitis 10months ago My doctor says its not that serious So no reason to worry But i am experiencing pain in the whole upper body (even tho the tendonitis is in my right shoulder)specially after waking up I have tried physiotherapy too Now i dont know what else to do What do u think i should do now? These can cause some neck stiffness in people when their supraspinatus is injured as it is common to compensate with other shoulder girdle muscles (upper trapezius especially) that connect to the neck and skull. Sir, can you tell a difference between supraspinatus tendonitis and pain in shoulder & posterior head of triceps ? Latter is usually described as cervical radiculopathy. May painful zone goes trough posterior deltoid to triceps yet virtually all therapists describe pain in frontal & lateral deltoideus. I've got my pain from repetitive movements ( pushups and dips).

Hello, i have supraspinatus inflamation when i overdo overhead work. How can i know if i should strenghten or stretch the supraspinatus. Or does it need any stretching at all, can that be a problem? Great video, gonna try Full can exercise for sure to see if it helps.

Stretching is usually not necessary for this muscle. Because it is the most commonly torn rotator cuff muscle, it is most important to strengthen the muscle and tendon with exercises like shown in this video. Hello, ive got a question if you wouldnt mind answering please. I apparently tore my rotatir cuff approximately 10 years ago. Not sure which part and had physio on it. In the last few years I've been powerlifting and my shoulder hurts when i bench press or overhead press. I know its impossible to duagnose this from my comment, but theres a weird thing i have that happens. The exercise you use in this video with the band, well when i do this, firstly my shoulder cracks (though its more like a "clunk" sound) and when i get just before vertical my shoulder rotates itself and repositions. I cant do anything about it, it's automatic. Also if i lay on the floor on my back and put my arms over my head straight, my good arm lays flat on the floor, my bad arm hovers above it. I can push it down but get some pain doing so and even if im fully relaxed, it still springs back up to hovering just off the floor. Have you got any ideas whats going on please as im stumped. Thank you in advance

Many people with long-standing rotator cuff issues develop tightness in the joint and changes in their mechanics. I would be willing to bet that you would benefit greatly from specific shoulder joint mobilizations. These often restore normal joint range of motion, reduce pain and help correct irregular movements. The key is to find a physio who specializes in orthopedic manual therapy.

These exercises often help with subacromial bursitis as well. The key is to only perform exercises that can be done with mild or less discomfort. These exercises only for very weak old patients. What about younger stronger patients that have good strength and mobility but can't pick heavy weights because of tendinitis.

@sujalsingh514 I have carried out one method. Say you can bench120kgs for 1 rep max. Don't bench it but keep benching 80kgs for 10 to 12 reps keeping your pain threshold to about max 4 out of 10. Gradually increase the weight after about a week or two by around 5kgs per side. You will find your pain Gradually reducing. Never stop doing the exercises, cause if you do you will never get back. Keep at it with lower weights and Gradually increase. Don't be in too much of a hurry to increase the weight. Approach it Gradually. I have done this and I am much better. Almost back to my original form. Also keep your diet and supplements on. Don't stop them. Believe me the body adapts. Tendons become stronger, recovery period shortens. Best of luck.

l had such a strong inflammation that it was unbelievable. I trained like a mule, and in the end I stopped training and lost 35 kg. That was my biggest mistake. After much struggle I managed to regain my old strength almost without pain. Only progressive weight gain. FORGET ABOUT EGO AND ALL GOALS! Reduce to the limit of pain (it almost always hurt at least a little in rehabilitation) and eventually you will be able to return. My advice is to do up to 5 repetitions in the bench, plus one or two hypertrophy exercises. And a slow and gradual increase, just no rush please.

@@sujalsingh514try the last exercise with a 10lb weight. If there's still pain picking up heavy stuff you need to rest 2+ weeks. I was in this position and it got worse because it didn't fully heal even tho it felt strong

Hi, I'm looking for some advice on this topic I get since months a painful click, mostly in the front of my shoulder, like in connection with the bicep where the pain also first started. But sometimes it also seems to click on the side, behind or maybe just around the shoulder elsewhere. Also many times it feels as though the ache came from the very inside/center of the shoulder, where massage I believe can't reach, and also I sometimes just feel muscle tightness or soreness around the area This appeared just a day or two after doing some chest push ups, and the following day working on biceps and many variants of shoulder with an elastic, a pretty mid weight one. At first I just felt a little chest pain, just like the normal common ache you feel sometimes after exercise, mostly when not having exercised in a while and then going back to it. But then and still now, the clicking and pain has been mostly around the shoulder/bicep So since it's continued for so long without success, I'm reaching out to see if I can get some answers/advice Also I've tried: massage, stretching, mobilizing, strengthening.. And I also know of others that I tried, am trying or considering to try like ice, resting, using an arm holder.. Sometimes I think whether I may have done just too much self massage or stretch to the point it may've been counter productive.. like irritating instead of helping recover Other than that, the only other thing that I'm trying to see if works, is trying to have my shoulders more to the back (posture). This one I heard many times so I thought to give it a try. Just a few days trying it, seems to be doing some effect...but the painful clicking is still there...we will see I heard the explanation is that shoulders to the front posture can irritate or inflame the area of the shoulder and produce or worsen this kind of thing sometimes But, honestly, so far I feel like I've tried many things as you can see, and none have worked...so I would just like to listen to suggestions on what might be the key point to understand why it is still clicking, and how to solve it. So far the rough idea or notion I can say to have about this is that it's basically just an inflammation that causes the clicking. Would like to confirm this, or at least have some more explanation to understand how this works/confirm this would be why my shoulder clicks, or what else could be. Btw, the clicking happens in different movements, like circular movements or just perhaps any movement...reaching the arm to the front, etc, not necessarily upwards Hope I can get some info/advice, I'd really appreciate it. It’s difficult for me to give really specific recommendations without assessing your shoulder, but I would still recommend performing the exercises in this video since you have mild pain with those two items. Cortisone (steroid) injections can definitely help reduce pain. We try to avoid them if we can as they weaken connective tissue structures, including tendons.

It pains alot As per medical advice, I've been advised to refrain from engaging in overhead exercises. To strengthen supraspintanus shall I still do mentiond exercises to strengthen the supraspinatus muscles while adhering to these restrictions?

Okay, saved the book to my Amazon wish list, same with the loop band. You're like the Doctor Mike of rehab. lol Those overhead exercises you mentioned maybe eliminating altogether, like tennis, you mean during rehab, not forever, right? My goal is after the rehab to get back into everything. It's incredibly hard to not just do stuff anyway. Like my head is saying, "You can handle it". I probably can't, or if I could it would probably feel much worse after. I know when I move my injured arm (shoulder) it feels better for a little bit. When I don't move it, it hurts when I try. It's a weird feeling when you make it to 50 and have never broken a bone, and the worst injury ever are sprains (which I walk on, probably shouldn't, but they go away eventually). And it's not like I've gone easy on my body, my friends and I used to tear around the woods, I once fell out of a second story window playing in a construction house, got hit by a motorcycle head-on as a teen riding a moped, I've been into full-contact sports most of my younger life, and semi-high-impact things like mountain biking, although, tbh I avoid the single track most of the time. It's just not fun any more. My wife is 36 and her sister who's 24 lives with us and she has us doing all sorts of stuff, Last winter I went ice skating, my first time in hockey skates. My parents made me do figure skating as a kid. And ballroom dancing. And gymnastics. But not karate, so I got beat up a LOT. lol. My wife wouldn't skate because she's like "I'm 36, what if I fall?" To which I said, "You get up and keep going." I fell twice. hockey skates are harder to use. But I kept going. I don't want to not do stuff any more, fully-intend to get back into tennis, back into riding, and hopefully do some skiing this year, or maybe try snowboarding. I never have, but how hard could it be? It looks like fun. Rollerblading, too, I want to try that. I know how to roller skate and ice skate, so it has to be in the middle there. I will take it easy while I do the rehab, though. One of my other long-term goals is to try to get a job with UPS hopefully as a driver, which is incredibly competitive to get hired permanently as, but great pay and benefits. So I need to be in peak shape if that can ever happen. Thanks for the videos!

You can definitely go back to tennis and other activities once your tissues have healed have you have strengthened them.

Its laundry and house cleaning. I have autoimmune disease causes inflammation in all organs and muscles. I can not take n.s.a.i.d.s anymore due to kidney and liver disease caused by the inflammation. A tear occurred when I carried a bag of groceries. Its 2 years since and my shoulder blade, cuff and under arm pit swelling. My neck is in agony, biceps as well. I have appointments with rheumatologist to get a cortisol shot try control inflammation there. It is very hard to control inflammation as I am limited in medication I can take. The tear was small but not healing. I do what I can to stretch and movment without excessively causing flare ups but its difficult. When day to day normal movements causes injury, what can be done?

I'm soon turning 80 years old and I'm dealing with a full tear in the supra. I'm going to therapy right now, last month and a half. I feel I can get by without surgery, what do you think considering my age? Not much strenght in that arm but it's my left and I'm right handed.

Yes, I would encourage you to avoid surgery if you can. If we look at the research, approximately 80% of individuals who are 80 years old have a supraspinatus tear.

If you feel more than mild pain with that exercise, I would suggest only moving through a partial range of motion, or eliminating it from your program and retesting in a few weeks.

Hey Doc, So i’ve been diagnosed with minor calcific supraspinatus, but i have pain somewhere around ac joint when working out. It’s minor pain but still bothers me. And i also have painless clicking behind my shoulder when i rotate my arm. X-ray showed my ac joint is fine so my question is.. can supraspinatus really hurt at that spot around ac joint or it could be something else? And why do i have clicking in the back. Yes, it is common for supraspinatus to hurt near the AC joint since it runs directly underneath the acromion. The acromion is part of the AC joint.

You are not obligated to even do a video, so let me start by saying thank you. I have supraspinatus and infraspinatus severe tendonosis and I am only 49. I’m just here to give you a tip in return for your tips. I went to a personal trainer who noticed that I was rotating my shoulder forward during my exercises and , also, pushing my butt out. Maybe also mention, first, before you demonstrate arm exercises that one should hold shoulder back using lats and rotate pelvis forward. Someone who naturally does this might not know that is ladies rotate out shoulders forward because of our heavy breasts and push our butts up because of the fat rolls underneath our butt. I was just hurting myself worse with my incorrect core posture. Thank you for reading.

When doing these and other stretches, my shoulder is popping and clicking. Is that ok? If I continue doing these exercises, as the shoulder gets stronger, will those noises go away?

Those types of noises are very common when rehabbing from this issue. As long as they don’t cause more than mild pain, it is OK to keep doing exercises. In many cases, when the area heals, clicking and popping sensations will lessen.

Yes, shoulder impingement pain is typically due to supraspinatus tendon dysfunction.

I would drop the weight and potentially start with arm weight only. If more than mild pain still occurs, then I would remove that exercise for now.

@@TheTaei Usually the scapula is mostly fixed in place mainly so that it doesn't elevate during the exercise. However, it does need to rotate some, so don't hold it in place too firmly.
