Dr. Falchuk explains the different symptoms that diverticulitis will cause a patient, including location of abdominal pain, signs of infection, and treatment options.
Kenneth Falchuk, MD: Abdominal pain is a broad subject, but if someone is concerned about a specific disease such as diverticulitis, the pain usually is acute. More often than not it is a pain centered to the lower abdomen, more commonly or not, the left side of the abdomen. We call it the left lower abdominal quadrant. That is an area where most of diverticula usually occur.
It is an area where the bowel is somewhat narrow and a little curved and when diverticulitis occurs, develops, that means inflammation within the diverticula, and the pain will be or can be quite severe and can also be mild. Patients may develop a change in the bowel pattern, they may become constipated, and they cannot eliminate the stools as well they may even feel bloated. Sometimes the pain is quite intense when the patient is doubled over.
Sometimes the patient can have a fever with chills, a sign of an infection. What diverticulitis really is, the definition, is perforation that means a little hole that could be small, does not have to be large, in one of the diverticula or in several diverticula. So in the space, the area of the colon where these are located, things become narrow, swollen, we call it edema. Pus can collect and that is called abscess and therefore patients can be quite ill, they need to be seen promptly.
Many cases of diverticulitis tend to be mild, mild in the sense that patient can be seen by a physician as an outpatient, evaluated, diagnosis established and be treated with antibiotics. The specific way to make the diagnosis is not only the clinical presentation. The clinical finding is the need to do an x-ray, a CT scan with contrast of the abdomen focussing in the area where the pain is present.
Now that does not mean that you cannot have diverticulitis in other segments of the colon. It could be at the right side, it could be at the mid-portion of the transverse colon.
As I said previously, the left side of the colon, the sigmoid colon, is the predominant site where diverticulitis develops. Abdominal pain is a broad symptom. It is caused by many conditions so it is important for the physician to know how to proceed and differentiate, what we call established differential diagnosis or in another conditions, it could be a catastrophe of something is leaking into the abdomen. There could be another abscess. There could be a perforated appendix and so and so, that is why prompt evaluation, proper x-ray studies and laboratory studies as well and then proceed with a treatment and followup.

One night, not long after I broke my back, I started having contraction level pain in my entire abdomen. I thought it was from my broken back because my back was hurting as well. I went 6 days not going to the ER. I started running a fever and the next day I went to the ER. Not knowing what was going on. My BP was 60/43. It was so low they couldn't draw blood. The pain was the first and probably only time I'll ever say was level 10. I couldn't even form a sentence, I wanted to die. It was not something I'll ever forget. I was in the final stages of septic shock. At my follow up with my GP she said if I had went to sleep that night, I wouldn't have woken up. So now I take abdominal pain very seriously. They ended up dxing diverticulitis with a severe intestinal infection in my right ascending colon with numerous pockets. A few weeks before I had went to my gyno because I was having bad pelvic pain. She noted fluid in my abdomen but didn't tell me about it. Anyway, my ex husband has Crohn's disease. It began in his ascending colon. That's one reason they are going to go in and biopsy for Crohn's due to the location of the pain and the pockets. Now, the pain i have stays in my lower left side abdomen. I can barely eat without some type of pain, bloating, nausea, or having to run to the bathroom. I don't know what to do. I can no longer work due to not just the broken back but now this stomach pain. Pain meds are not really helping so I don't take them anymore. Today I am going back on a liquid diet for the rest of this week to see if that helps. I need digestive rest.

+jaelyn May I thought I had appendicitis from days of appendicular pain(pain about 1 inch below the belly button and 2 inches toward the right of the belly button). I went to the ER and had a CT which the ER dr. and the ER radiologist said was diverticulitis, put me on TWO antibiotics. Then a few days later, the dr. called me in panic that the final CT report did not show appendicitis or diverticulitis. But just to make sure, I'm going to see a gastro to make sure.

My problem started with a mild pain in my right kidney area . The pain then moved lower to near my groin then started moving towards my back and upwards ,then moved around to the front of my rib cage all within two days. The pain was strong but bearable but if I coughed or stood up the pain was so severe it frightened me to do either. Went to the doctor who immediately said probably Diverticulitis and prescribed Amclavox DUO. It has been 4 days now and the pain has just about gone except when I cough and even then the pain is not as severe. I still have 6 days of tablets and I'm confident I'll be cured by then.

I had full blood counts and an ultrasound that all come back normal, however I've been in extreme pain.
