You are so clear in your teaching. I wish you had taught pharmacology when I was in pharmacy school. I am really enjoying your videos. After 30 years I've forgotten some of the specifics of MOAs of the drugs I dispense daily. It doesn't really come up when speaking to my patients, but is the stuff I geek out on.

At first, I want to thank you and all of your team for the great work and dedication to make these type of videos that is comprehensive and informative. However, I want to comment on some of the information presented: 1) The Receptor that is responsible for mediating most of the angiotensin II activity is called AT-1 receptor not AT-2 receptor and ARB act by specifically inhibiting AT-1 2) ARB class of drugs ends with the suffix SARTAN like valSARTAN , candeSARTAN etc 3) Continous and chronic elevation of Angiotension has a mitotic activity on vascular SMC and cardiac myocyte and that is why ACEI or ARB are beneficial for cardiac hypertrophy as it lower the progression of this process. 4) Renal effect of these drugs is by mainly antagonize the effect of ANG-II on efferent arteriole which will eventually leads to dilation and decrease intraglomerular pressure. By relieving the stress on glomeruli that is beneficial is case of nephrotic syndrome or kidney disease.Also that is why renal artery stenosis is contraindication as it will cause sever drop in glomerular pressure by decreasing blood flow from afferent arteriole and by vasodilatory effect of ACEI on efferent arteriole. You need to be more aggressive with your heart doctor and demand more time and better explanations as it is your life, and you are paying for his time. I'm sure he charges for way more time than he is giving you. Be pro-active. He is employed by you. Ask his office manager if he bills by the hour or the visit. Then explain that if the hour, he owes you money. If it is by the visit, the visit isn't over until you say so or you'll find a physician that will take the time to treat you properly. It's your money and your health. You have a right to get your money's worth.

Thank you for this indepth explanation of the actions of ACE inhibitors and ARBs. I suffer from Peripheral Vascular (artery) Disease (PVD/PAD) with cyanosis caused by angioedema; and Stage 4 Chronic Kidney Disease. The accepted treatment for PVD indicates administration of statins or ACE inhibitors. From this video I learned to support my original conviction, against that of my cardiologist, to NOT prescribe an ACE inhibitor for myself which could lead to an M. I. (I have already had 5 M. I.'s with accompanying open heart surgeries), and or a CVA (stroke, of which I have already had 3 of rhe ischemic variety).

At 2:42 you write AT2-R for a receptor that Ang II bind but Ang II binds to receptors called AT1 in the arteries. Ang II does bind to AT2 receptors but those are in low levels in adults and mostly play a big role in fetal tissue

Really appreciate "the anti-hypertensive agents" series The control of "ADH secretion" 1. Osmolarity (Osomoreceptor in hypothalamus ) 2. Blood pressure (Arteriral baroreceptor) The textbook usually didnt mention the effect of Ang II directly on Hypothalamus it's probably due to the situation Ang II is elevating ,it's usually the situation of Low blood pressure ( through the baroreceptor pathway)
