Note for people looking for guidance: take your time! In germany we do things very slowly to make sure the knee recovers perfectly and that the ACL doesnt tear again in the future. It's not about quantity it's about quality is what my doctor says. It's all about working trough everything and not pushing your traumatized knee too hard over the edge. Dont forget that having surgery is a trauma on it's own and the body needs time to figure things out. Good luck everyone!

I like how quickly they jumped to the cycling exercise calling it phase 1. I am nowhere near this 2 months of my ACL and meniscus surgery

It's much more convenient to start lying on the floor with legs placed on a large exercise ball. Just continously bring it to and away from your body evenly with both legs, like 500 times as a warmup every time you exercise. Works magic for improving flexion

Everyone recovers differently. I'm in my 5th session of physio and the bike riding is not easy but I just take it slow .

Bro, stage 1 can be divided into many other stages. Like week 1, week 2... Most of the excercises cannot be done after one week after cirugy. But that`s a golden content. Thanks for that. It will help me with my rehabilitation for sure. Cheers

5 weeks ago I had surgery for torn ACL, MCL and both menisci (unhappy triad). This video has been very helpful in performing physiotherapy at home, although some excercises I still can’t do. Great job guys and thank you so much! : D

Really informative. I had ACL reconstruction with allograft and medial meniscus repair one week ago. I am able to walk unaided and full weight bear. Your information will keep me moving in the direction I need. Understanding the process is so helpful.

Rick after 7 days I could walk with just one crutch. After 8 I could pretty much walk without them. I’m now 10 days after my op and I can walk quite well. I use my crutches just in case and I’m still in a fair bit of pain

Rick I can walk without the need for crutches... I do use them for added safety and haven’t walked long distances. It does however leave me pretty sore after walking on it. I also always use my crutches when tackling stairs. Everyone is different and recovers at a different rate. I wouldn’t rush anything. Talk to your physio about your concerns.

@No u well obviously everyone’s different. 12 months ish before I was running again. Depends on your overall fitness the extent of your injury etc. I needed a full reconstruction and mine was bad for nearly a decade. Plus my overall fitness wasn’t great. Its coming up to 2 years since mine now and I have no issues really. Abit of stiffness now and again.

She’s a totally healthy volunteer, I’m pretty sure many of the exercises shown can’t be performed in first month

All of those are early exercises which most patients can perform in our experience. Every patient is different though.

I agree. At least in my experience a vast majority of patients take a bit longer to perform these safely. Some of these seem closer to phase 2 of rehab rather than phase 1

Come to India and undergo acl , meniscus reconstruction surgery..here in 12 days u will have 90 degree extension of knee 😁. From personal experience

If the swelling it’s well controlled and early mobilization techniques applied… sure a patient in the first month can do this…

Question: * In the section talking about external-resistance allowed depending on the graft type. What type of resistance are we talking about? In general? In OKC-exercises? Or resistance to the area of which the graft has been taken from? Thanks!

Hi, The current climate on COVID-19 has meant my ACL and Meniscus tear rehab have both been delayed/postponed or as it seems completely forgotten about by the NHS, which is understandable due to the pandemic. However my Surgeon/physician told me (when i FIRST had my MRI to confirm the tears), that i may be able to return to playing football without surgery due to having strong surrounding muscles and many other soccer/football players have done so (i thought he was just telling me this so that i stayed positive but the pain i felt at the time was telling me different and in no way did i think i would be able to do the things i am doing now without surgery). As I have no physio or anyone helping me due to social distancing etc, i have kind of taken it upon my self to see what my knee is capable of... now almost 4 months post tear with no physio or any external rehab, I am able to barbell squat 120kg+KG for reps (was squatting 180KG prior) and run in a straight line without pain or sometimes both exercises cause maybe a 2/10 pain (sometimes) (i have been running 5 KM in around 22-23 minutes - so a decent pace). When i speak to others that have had the injury or at the same point in there recovery (4 months post trauma), they are unable to believe how much i can do with no physio/surgery/rehab. which is really leaving me in the dark as i just want to play football again... but also i dont want to cause anymore stress to the knee. I would'nt say my knee feels normal at all, but pain is low and stability is nowhere near perfect but its ok. Now because I have no physio due to the current situation in the UK, I have no support or help as to what i should be doing next or what i should actually be doing... Being a student i also don't have the funds to pay extortionate amounts of money to go private. I am also worried that if i tell them this info they will tell me i don't need certain surgeries etc to save money and because i have told them i am ok and i am back running. but in order to play football at the level or intensity as i did i want to do whats best for my knee. I am worried to try any multi directional work or even kick a ball. Is anyone on here able to advise or help me what i should do next? As if i can go back to playing football without having ACL reconstruction it would be a dream. If this is not the place could anyone point me in the right direction? Just to confirm I have had an MRI scan to confirm both a tear in my ACL and medial meniscus. just to note, I am a 20 year old male. Hey man, this is what happened to me, however I was told the nhs have contracted some priv hospitals during COVID. I don’t know if it’s based on your region in the uk though.

DONT go back to multi directional sport till the reconstruction surgery! I partially ruptured mine early this year and when I went to the nhs was told that I didn’t need surgery. Months went on and as I slowly got back into sports(nothing requiting explosive movement). I even started doing squats at a reduced again before the lockdown. However randomly whilst I was training this month i completely ruptured it, without being involved in intense movement. I was only doing a motion drill when the acl tore. It was much worse at that point but I was privileged enough to get private surgery. If you continue to play it will be a ticking time bomb and could be much worse. You need to wait for the nhs to get your surgery even if it means sacrificing football for a long time It’s not easy, it’s heartbreaking, but you won’t imagine what other interests and how much more time you’ll have to develop other parts of you as a side effect of what seems unfortunate now. Definitely don't do any movements that require quick change of pace and direction, as that will cause a bigger injury and it could cause some serious problems. I've done my ACL 3 weeks ago playing football and now I'm waiting to get an MRI scan, however, they already put me on the list for the reconstruction. which i have to wait 3 months. NHS is a joke when it comes to this types of injuries. Its amazing that you co do things like squatting with that much weight and not feeling too much pain. If you want to have it reconstructed then contact your gp or even go AnE and just tell them that its seriously causing you problems and that you demand a surgery. Especially if you are only 20 and you want to carry on playing sports. Then they will have to do it. It would be different story if you were much older and not being active at all.

exactly the same injury happened to me 18months ago. i have been in your position able to do certain things not able to twist the knee though for pain and fear. i think you should definitely hold off doing any more than you currently as you will inevitably make it worse. i have finally got my surgery tomorrow through private health care through work. i know you said you are a student but if you have a job maybe try and find out if they offer some kind of healthcare. it is definitely worth it as for me there was no issue treating a pre existing condition, id assume most are like that. good luck!

Hello I had acl surgery about 9 months ago. In these 9 months, I did two or three exercises that my doctor gave me. But now, after 9 months, I still feel that my leg is not the same as before. I don't have a leg like that one (I feel like I didn't recover properly) Can I make my leg the same as before and the same as my other leg by practicing properly? That I can do this by doing the exercises that you tell me

I hear allot of my KNGF guidelines in this, wich makes it even more appealing to use in practical exams . The exercise variety is a plus , but i would like explanantions with it so i can build evidance and understanding in my exams . Even though i have a good guessing , some are new to me . Yes, we're all operating in NL so we're trying to stick to the national guidelines of course. It's hard to give detailed explanations with every exercises we use as it would explode the video.

Hello! Thank you for all the detailed information, I find it very useful. I have one major doubt though: you said that open chair exercises are not recommended in the early stage and they can be performed in a specific range of motion only. Later in the video you mentioned that leg raises are allowed and safe during the early stage, aren't they open chain? My physiotherapist recommended me some exercises, which I think are in open chain: leg raises with a towel, seated leg raises with two legs on the ground... I'm not sure if I should perform them, I'm afraid my graft could extend too much or be damaged. Could you pls explain briefly which open chain exercises are allowed (such as leg raises) and why? Btw I'm 6 days after my ACL reconstrution (autograft, hamstring tendon). Thank you a lot!

Although this is probably far too late a response, but the exercises she showed were safe 'because' the leg was in a position where the ACL is not under strain. The fully straightened leg (fully extended knee) prevents the ACL from taking much of the weight when performing a straight leg raise (that's my understanding anyway).

About twenty -five years ago, I fell on the ground and received a severe internal injury in my left knee. I consulted many doctors. They prescribed me pain killers and gels but a little betterment in pain. I decided to do MRI by myself which showed fanning of the fibres of ACL of left knee, moderate sprain and partial tear of ACL. I can walk easily but I cannot get I cannot run. What will be its treatment?

I did my ACL surgery yesterday and from what I can feel- it hurts when I try bending it or trying to move. From what the doctors said- at this stage you should cautiously move it and try to function your leg. I wish you a fast rehab!!

I’m on week 1 after surgery. And I have physical therapy in 1 day from now. My leg hurts soooo bad, it’s pain in my shins, and knee and back of my hamstring. Was this normal for you guys?

I get acl reconstruction surgery at the end of the month. My doctor said he’ll be using my quadricep muscle, do these exercises help with those kinds of graphs as well? I need some help and assurance as I’m a dancer and my quadricep is what I use the most.

Hey did have a recent ACL injury and have gained full motion of knee, can climb stairs. But am 80kg of weight and wanted to know what excercises I can do on regular basis to strengthen my knee without hurting it.. concern too much weight

Are you able to terminal knee extension with a band in standing because the leg is now in close chain environment where the arthrokinematics switch so that the femur is moving on the tibia and not the other way around as in open chain. I know we want to void OC knee extension because it causes shearing at a healing ligament. How is the one where you are laying prone and extending the knee with the ankle propped against a pillow not open chain which would be contraindicated in the early rehab process? Any help is appreciated!

Terminal knee extension/the screw home mechanism happens in both open and closed chain. It doesn't matter if the tibia or the femur moves.

Hlo mam ... I got acl tear on 7 august 2019... I refused the surgry and take 3 month physiotherapy fr my knee and after that I feel recoverd and no pain so I live my life normally and also started my soccer training Bt today I feel so much pain in my knee...So do I have to start the exercise from phase one again??

Can u please make a video and a rehab protocol with evidence base with tens ultrasounds and radio frequencies for calf for example strain? Calf or other muscle or tendon injury? With evidence base? As I am a student and I had an argument with my tutors during my msk placement here in the UK as back home (Greece) HELLAS they are using a lot of ultra sounds and radio frequencies and massage techniques and also the arthro tool and accupanture but here in UK nothing. Just exercise and stretching.thankss

Tens, ultrasound and radio frequency is notmore effective than placebo for basically any condition. Muscle strains should also not be treated by stretching or massage but by progressive (eccentric) exercise.

I had an ACL Reconstruction Surgery 1 month ago using Hamstring graft. I was feeling OK, and so started walking with a gentle limp. However, once I was trying to pick up something from the floor and had a massive stretch on my hamstring. I also heard a cracking sound. My mobility is still the same as my level before this incident. However, it feels a bit strained and I observe a gentle bump and swelling behind the knee area. Could you suggest me if it's normal or may it heal naturally?

Hi, I had a Acl and medial meniscus surgery, is that enough for these 2 phases of exercise, or I need to do any other exercise for meniscus.

Depends on the post-op protocol and especially if you had a meniscectomy or meniscus repair/what you're allowed to do in terms of weight-bearing

I'm getting an ACL reconstruction next tuesday. Was just working on an ACL protocol myself to treat my own patients better. Interesting to see how yours compares to mine! Did you use any specific source to create this protocol?

That's a combination of the KNGF guideline as the very basis plus different concepts from Kirsten's master in sports physiotherapy. Her main patient population are ACL patients, so it's also personal experience and preference.

Good vid., just a little annoying music... But I've read a study, where they've measured the strain on the ACL and the most force it got with isometric tension from the quadriceps, followed by open chain movements like the straight leg raise. Therefore those exercises should be avoided in the first stage. Do you have any other sources for this topic?

I recently tore my ACL, MCL and my medial meniscus. Tomorrow I go to meet with surgeon to discuss the surgery. Biggest injury before this was an ankle sprain.. I'm scared as fuck, but so ready to get this post surgery first two days over with! Also there is a thing that they are taking out of your knee the day after surgery. Anybody know what that is? And does it hurt as bad?

Ross Fraser I had acl reconstruction yesterday. Last night was very painful. Today I can already put some pressure on my leg and the pain has decreased drastically. You got this mate!

hey! i've had acl surgery 2 years ago. after the surgery, you my have little tubes installed around the wound. no worries, they're not too bad to get out ;) as long as you get a good physical therapist, and do your therapy very seriously, you'll be alright! good luck

Hey I’m actually going through a similar recovery and they don’t show it in the video but the exercise that works best for me gaining back straightening was to lay on my stomach on a bed and let my recovering leg hang off and straighten with gravity. Not sure if I’m sending too late but i hope this helps

Hi physiotutors, thanks for the great video! at 1:00 on the bicycle, would you be strict on the seat height and hence knee flexion angles on the bicycle at this stage? If not, then what are your considerations for the bike. Looking forward to your reply, thank you

The bicycle is used to improve extension not flexion in the video. Biking can usually only done comfortably from 110 degrees of flexion on

At 3 weeks my physiotherapy made me do cable leg extensions at 2kg weight and recumbent bike.. and at 6th weeks did treadmill start at 1.5 speed, 1- 3 elevation, now at 2.5 speed at 4-6 elevation, I'm progressing but what I don't understand is that my knees still in pain until now on its 9 weeks, when I walk and stand longer is it normal that knees feel numb? Less swelling now than before.. and pain is the new normal to me, when will this new normal be gone? Are my physiotherapist did the right workouts for me? Or is it to harsh or what? I can walk but not too fast and when walking I'm trying hard to do full extension on the injured leg, btw had ACL reconstruction and meniscus repair arthroscopic surgery

I also like the workouts you've shown in this video, some of it are the workouts given to me.. but the flexion still a lot of pain than The extension.

The video series is for rehab after. Training before surgery is surely a good idea as well but can be done with higher intensity if initial complaints subside

Hi guys, 5 days ago I tore my acl for the 3rd time in 10 years. But this time I also had I low grade tear of the PCL, low grade sprain of the MCL, high grade tear of the medial patelofemoral ligament, tear of thr medial and lateral meniscus. Waiting to go and see a specialist. Am I fucked for life? I know I cant play sports anymore, but with 2 small kids I would like to be somewhat active.

Sorry to hear. Can't say anything about your particular situation but I have a friend still playing high level soccer with 4 ACL tears. Probably also very much dependent on how hard you work during rehab

Physiotutors ok so here’s an update I’ve been trying to bend my knee for the past couple days and I can bend it just about 45 degrees and I start physical therapy today. I feel like it’s getting better but still feels like there’s a lot of pressure when I try to bend my knee more then 45 degrees but when I massage it it kinda loosened up a bit.

You can start weight bearing most likely. My doctor said I could immediately after surgery but that’s mainly a question for your doctor since everyone’s surgery goes differently

Indeed, depends on the protocol. Often the use of crutches is recommend in the first 4 weeks.
