There are lots of doctors and other health care professionals jumping on board the keto bandwagon lately, but Dr. Westman has been conducting clinical research and helping patients with this way of eating for more than twenty years. So what does he think of what these newcomers are saying about something he’s specialized in for over two decades?
Check out Dr. Westman’s latest video to see him weigh in on osteopathic doctor Mikhail Varshavski (“Dr. Mike”) talking about keto.

@@Boostisablast Nothing is cheap anymore, but I can't help to notice what other people have accumulated in their shopping carts. Add up all the money they are spending on junk foods, processed crap, and even most vegetables are no longer cheap. Not to mention the time they waste shopping and preparing. I also hunt and fish for some of my food, which is almost free food and helps in self reliance.

Wait, back up. Mike is worried that on a diet that for health and weight loss, that if I do keto, I can't go with friends to eat huge amounts of ice cream and cookies. What diet is he proposing that _would_ allow you to eat with them without any restrictions? Even a moderation diet won't let you do that back to back all day long. At some point you do have to say, "no, thanks."

I think I remember seeing Thomas DeLauer telling in one of his videos about how unhealthy he had gotten at his worst when he decided to change whatever he had to to get better. When he started seeing positive results from changing his eating, drinking, and exercise habits he found that his old drinking buddies were sabotaging his efforts. He then decided that another thing he needed to change was who his friends were, so he did. It was not easy because he had known the old group for a long time and had shared many experiences with them, but his new friends shared his interest in getting and staying healthier. It is hard enough to change our relationship with food -- prioritizing good nutrition over enjoyment, entertainment, and convenience -- that having 'friends' making it even harder is what is 'not sustainable.' I had a similar experience long ago when I got out of the military and went back to school. Some old friends I had reconnected with had started using a variety of illegal drugs and encouraged me to join in. As you say, "At some point you do have to say, 'no, thanks.'" I missed them, but not worrying about becoming collateral damage in a drug raid was a big relief. (I know that cookies and ice cream are a lot different, but same idea in regard to whether someone is really a friend.)

I lost weight on keto but it wasn’t good for my health issues. I had asthma, arthritic, pain, and lower G.I. issues. Asthma I’ve had since six weeks old and I’m 64 and pretty much food makes it flare and environmental. Lower G.I. we’re talking over 20 years of bad situation and arthritis has been progressively getting worse. I started the carnivore diet in April and all three areas have improved so much that I will not go back to vegetables. In fact, when I tried them in August, my hip hurts so bad that evening I could not walk. To me vegetables are inflammatory. Fruit does not bother me but I don’t eat that much. I’m just going to stick to. It works for me and everybody needs to find their way if it’s not working try something else.

I can’t get my wife into it, because of all the false information, being spread by doctors! She asked her doctor if it was safe, she only asked if I was under a doctors care! I told my wife, that she should have said YES, that’s how I got into this situation, fake opinions, from doctors! I’m very worried about her, not me! I’m feeling better all the time, but a month isn’t enough time for a magical result! No patience, got us into all the boxes of sugary food, so we wouldn’t have to cook and clean! Diabetes in remission, lost weight without trying (80kg to 64kg), reduced blood pressure (still taking a very low dose of perindopril), all my blood work is good (LDL has increased but I've researched that and am not concerned; cardiologist recommends low dose statin even with a CAC of 0 - I'm 65 yo F), waist was 44" no 38". My ALT liver function at its worst was 45 now 13. Liver fat was moderate, now mild.

Just received your pamphlet, thank you. Been on Keto for 9 months now. Went from 95kg to 86 in about 2 moths and then seems to have stablelised at around 85 - 86 kg range for 7 months now & cruising. Mostly eating what I like anyway, meat, seafood & freshly cooked non starchy veg. I'm male 1.8 metres, aged 72. My mind is now comparable to when I was aged 45 - 50. For 7 months now, I have resumed my daily Karate exercise at a slightly less vigorous manner. I've got all my strength & flexibilities back. Blood pressure at around 127/74. All because I was so angry that I could not even get under my car to do a basic oil & Filter change - hahaha. I will download your pamphlet onto my phone as a reminder to read from to time to time. I'm from Australia, of Chinese heritage. I post the above NOT as a boast, just hoping to share encourage others. It was not hard, except for cutting down on rice to a small portion once a week. Do it for your health - it's worth it. You'll never look back. Cheers 👍👍

Lost a significant amount of weight, but most important re-gained my mobility (severe cervical and spinal nerve damage). I went off a year ago and quickly became miserable, and regained all the lost weight. My inflammation started raging out of control. I have since re-started, as I have learned my body liked keto, and I NEED it help control my nerve damaged neck and spine!

@blueckaym

Dr. Mike didn't imply that reducing carbs is bad, he just described the main difference with Keto diets. I know from experience that Keto works. I haven't been very strict with it (I guess I've done one of those dirty ketos), but even this way I lost ~27kg for 9 months (120->93), then I messed up for a while not knowing that Cashew is actually relatively high in carbs ~8% and I halted the weight loss and even bounced back 1-2kg, but when I learned why I quickly returned and dropped back down to 90 this time ... My goal was 85, but then things with my personal life went to hell so I really didn't care for weight loss anymore, stopped hiking, exercising when depressed went back to the sweetie things, so for a year I slowly went back up to 100, and then after another crash for about half a year back to ~120 (perhaps even a bit more) ... Weirdly this April I somehow naturally reduced sweetie things, and got distracted (with 3D printing) so I didn't do my bored-eating, and soon I noticed that I'm losing weight w/o doing keto or other diet. So that 4-5 months in which I lost ~8kg down to 112, and this week was my first back on keto (to finish the job this time). So this first week alone I lost 2kg, down under 110kg now. Recently climbed a mountain peak near my city (~2h hike with about 500m denivelation, and didn't even stopped! It was enough to slow my pace from time to time when I was waiting for the last people in our group to catch up). And since some friends learned that I'm back on keto few of them tried to stop me by sending me "facts" that keto is bad for you ... Funnily I've already been thru that research and half the things were wrong (few even outstandingly wrong!), and the ones that weren't were really negligible. The main thing usually is to drink more water (~3L/day is fine) to reduce the risk of sands & rocks in the kidneys (which keto actually can cause if you don't take measures), and the water loss esp. the first weeks, and with the lot of water you'll need a bit more salts (sodium chloride & potassium). So unless you already have some problem with your kidneys it's very unlikely to have problem. The other friend actually tried to convince me that tomatoes are mainly carbs ... which is you compare carbs vs proteins+fats (ignoring ~1% fiber) is technically true - carbs are ~3% and the proteins+fats ~1% together, but incl. the fiber there are ~5% of macronutrients (& micronutrients as the name suggests are important of course but really tiny amount), to imho tomatoes are mainly ~95% water :) Of course some veggies are high in carbs and should be avoided while on keto (mainly beans, starches & some underground ones, and also corn for example), but most veggies are actually ~5% carbs or even under so perfectly fine (also some have fibers counted IN the carbs content). But of course veggies (while critically important for their micronutrients) don't have much of nutritional value, so you'll have to eat other things to get that energy (parallel to burning your stored fats) - avocado is indeed great from the veggies (though technically fruit, like the tomatoes :)), but I like to complement with some cheese (mostly Bulgarian white cheese) and meat or fish (I don't go crazy when I don't eat meat :)) Keto requires a bit of learning curve (I didn't know most things nutrition value before), and the things to be careful about (drinking more water & perhaps some little bit of extra salts), but after that it's not that difficult to follow. Yes it can be difficult to find appropriate things to eat when dining out, because most things in restaurants are not keto-ready (but when I get steak with potatoes on the side I just ignore the potatoes, or give them to someone on the table ... actually after the first 9 months on keto I actually lost my taste for potatoes - only few ways of preparing them are tasty to me now ... when I'm not on keto of course!). So I really don't see why some people have problems with (usually OTHERS being on) Keto! PS. Fun fact - when you stop eating for 12 to 18 hours (because one of the arguments against Keto was that not eating for a week or more is better!), your body actually switches to ... guess what - ketosis and start burning your stored fats! But even if you do that, don't go crazy on it - too low fats are dangerous as the body needs at least a little bit in order to process some micronutrients properly! Also w/o fats it also starts breaking down muscles to eat the proteins!

Once I attained the state of Ketosis I knew I would never quit. My mind was clearer, my mood soared, and I was eating all my favorite foods in spite of not having the sweets and booze I used to crave. Eating out, I just eat normal portions of meat and low-carb veggies and am perfectly satisfied. This is actually the most sustainable "diet" - lifestyle - I can imagine. Plus it is actually cheaper than the Standard Diet because I'm not buying the High-Markup carb foods like breakfast cereals and bread/cake/snacks, so I can spend the extra dollar here and there for a grass fed steak!

I also have really enjoyed not being HUNGRY every few hours while eating three meals with snacks in between every day. Now, with two meals a day and no snacks I am still comfortable going from 16 to 20 hours between the last bite in the evening meal and breakfast the next day at noon or later. I still have trouble believing how much better that feels. When I get the urge to snack now it is out of habit or nostalgia for the feeling rather than from a real need to eat. (As Dr. Cywes says, once fat adapted someone's "snacking is an emotional event.")

Dr. Mike tried the Keto diet for a short time. Like someone else said he kind of goes where the wind blows but I thought he was overall respectful of the Keto diet thought you could tell he didn't do a deep dive into it but mostly read the "highlights". I made a comment on the video when I first saw it about how it was very much sustainable. So many people cannot wrap their heads around giving up carbs. You see this when they would rather give up meat and eat a "healthy" vegetarian or vegan diet as to make sure they don't have to give up their carbs.

No surprise to see a mainstream doctor list these cons to the keto diet without explaining at all. The red meat thing I recently found out is so unfounded. It's mainly because people cook it in seed oils which is causing more of the problem. If anything, it's the meat that partially offsets the use of seed oils. Plus, I'm sure his audience would be put to sleep if they had to learn the nuances of LDL because it's actually a pretty complex conversation

I think most people will stray from their chosen diet from time to time, even those who are pretty dedicated. It is just human nature to cave into cravings sometimes. I am disappointed when this happens but it is just one bad day out of many. If I can stick to my diet 80% or more of the time I think my results will be pretty good. I just did your 7-day keto challenge and my blood sugar decreased dramatically blood sugars are my main motivation. If I lose weight too that is good but not my main goal. So I am happy with the results so far. (by the way, I did lose 10 pounds).

A wonderfully balanced critique, doctor! One point about Dr Mike's take on "sustainability" of any keto diet, either strict or relaxed, is that overindulgence of fats and the consequent higher blood ketones produces a condition where ketones are actually wasted from the blood during respiration, according to Bikman. Unused carbs, on the other hand, are always stored -- and that creates an an intrinsic shift away from homeostasis. Which practice is actually more sustainable? What he means to say is that most people will not be able to keep to even an easy low carb diet, so why bother. That kind of perspective is equivalent to giving up on the diabetic or alcoholic, because the epidemiology doesn't support going against the established trend. I would like to see what Dr Mike would do in a river raft above the falls . . .

@@goodcooker4001 I wouldn't say proof. There's lots of anecdotal 'evidence' and some epidemiological studies, but none of this provides evidence for cause and effect. Using the scientific method we can easily disprove a hypothesis; we need merely to find one counter example. Proving a hypothesis (such as the SAD diet ISN'T good for us) require long term randomized control trials of the kind that cannot and will not ever be done.

I have been on keto and IF two meals a day since 2021, and I defeated pre diabetes. I never been overweight, but I have familial risk, and other issues that now I know contributed to this problem, and all other aches and pains that also went away. All my labs have been superb, except the LDL, which tend to be higher, but I am not worried about. I am stuck with keto and frankly, I enjoy my meals; I was never addicted to excess sugars, so that part was easy. I avoid all the keto internet hype and packed keto treats. I am mostly carnivore, but I eat small portions of veggies too. This guy does not know what he is talking about. I follow seasoned doctors like Dr Westman.

I hate the modern use of "X times LESS" because 'times' implies multiplication or more. What they really mean is 1/X of the original number. I'm also sick of being told Keto isn't sustainable, often by people who have no qualms about advocating the vegan diet. In some studies more than 90% of vegans find it unsustainable

Another thing Dr. Mike got wrong is that he said that very low carb diets induce ketosis for all people. He also defines “keto diet” as a very low carb diet. That is completely wrong. You cannot assume ketosis based on the food that is eaten. It is a mistake made by a lot of people. A keto diet is not based on food, it’s based on the production of ketones. Many people on a so-called keto diet, never enter ketosis. A lot depends on genetics and exercise. A person who runs 20 miles per day can eat a lot of carbs and be in ketosis and thus be on a keto diet. A couch potato who eats a lot of protein may have a very hard time entering ketosis. You need to measure ketones to ensure that you are in ketosis - you can’t just go by the food that you eat. Thus, the simple definition is that if you are producing ketones, then you are on a keto diet regardless of the food.

Well, Dr. Mike is young. He doesn’t realize how much those carbs are going to haunt him later in life. In my 30s, I had gut problems, low stomach acid, & dehydration. I didn’t know that at the time. My doctor was no help. She said I was getting old. Luckily I didn’t accept that answer. I began researching how to improve my health. I was never fat per se. I was just unhealthy. The most I gained was 135 which was still “healthy” in the standard model. I’m petite at 5’3.” On the keto diet, I feel better at 53 yo then I did at 35 yo. I never thought I would be fasting & eating the way I do and still feel great. Now if I just worked out more. I do walk a lot. Just need more HIT & weight training.

Excellent, reasoned content. I've been LCHF for six years. Never really tuned into the Keto lifestyle initially, as I got the impression it was a rather aggressive space, populated with angry zealots, not unlike the shouty vegans. As the good doctor states: I was probably looking at the wrong resources for guidance. Nevertheless, here I am years later, gradually moving in that direction. I don't count calories or macros. And I don't weigh the food I eat. Initially, I just stopped eating processed food along with bread, rice, potatoes and pasta. In less than 6 months I went from 100kg down to 70. I've learned a lot in that time - don't look to your doctor for nutritional advice, first and foremost. Dr Mike seemed to be painting a reasonably accurate picture up until he started talking about how difficult the lifestyle is to follow. Meat, fish, eggs and dairy. Nuts and leafy greens. I don't eat breakfast until around 1 or 2pm, and have my evening meal at around 8pm. Honestly, I've never found it difficult in the least. I suppose if you don't like meat or fish, or can't tolerate eggs or dairy, well, that's a different story. But the only way to find out, is by giving it a bash. Just experiment until you find what works for you.

Keto diet revolutionized my health. I never had anything like the "Keto flu". I noticed no difference. I am eating keto for life. I will never stop. Just maintain. The thing I don't like about these anti-keto proponents is how they treat people like such simpletons and weaklings. Like we can't manage our diet for the long term. I eat a delicious diet and don't miss anything that I have cut out. I put in a lot of work developing my menu but it isn't that difficult to manage. I spend about the same amount of time preparing meals as I used to. So to say it is "really difficult" is nonsense.

Otherwise most electrolytes to be found comes with unnecessary things like sugars). The nutrition deficiency he refers to may be vegan versions, Nadir Ali points out some problems with vegan versions, and that obtaining quality protein without it being highly processed (legumes typically have protein uptake inhibitors).

Dr. Westman, why is it so hard for us to get a CGM in the U.S.? I am not tracking ketones right now, but I am still trying to be vigilant about my glucose, I am not diabetic (surprisingly). I would love a CGM, but instead, I am constantly pricking my fingers. 🥴 It makes no sense to me, to restrict a tool to help people control their weight and sugars- How can we change this American practice , so that we all my benefit, if desired?

What you are saying makes no sense. If you don't have diabetes why on earth would you need to have a CGM (Continuous Glucose Monitor)?? I'm assuming you don't have heart problems but do you think you need a pacemaker too? The only people who need to be continually monitoring their glucose are those on insulin. Just get your A1c checked every 3-6 months and be done with it. Maybe you need psychological testing as well. Something to seriously consider.

@@terised I don't have diabetes but I've used a CGM several times (for 14-day periods). It's a great way to see how you respond to various foods. You can see how much your glucose rises, how fast it rises, how long it takes to go back to baseline, etc. I eat a mostly animal-based diet but I did make one recent change based on my CGM tests. I was eating a small amount of fruit each morning, typically 1/2 an orange or 1/2 a grapefruit. There was quite a large glucose spike from that, so I eliminated this ritual. I've also tested myself to see what happens if I eat foods that I know are problematic, such as potatoes, rice and ice cream. I rarely eat these foods but I was curious to see what happens when I do. The CGM doesn't lie and helps give you a stern reminder to avoid these foods and stay on track.

When I lost 85 lbs, reversed my t2d, got blood pressure under control using the keto/low carb diet, my doctor took me off all meds and said: " that's amazing what you did, but I don't like the keto style diet because when you quit you can gain weight back really fast and get back to where you were." I said: " yeah, but isn't that the same with any diet (with a laugh)." He responded: " well, yeah, that's true." So many doctor's are just afraid to try new things for their patients and are stuck in status quo mode. It's pretty much just use government guidelines and pump you full of medicine.

Regarding the potential harms - my 2cents (not being a doctor!): - nutritional deficiencies from limited diet - that is indeed simply WRONG. You might have to learn nutritional value of things (mainly to avoid carbs sneaking up on you, because it's true that most food, esp. the processed ones have added sugars or simply are high in carbs), but after that and when you're eating some normal amount of veggies you won't be having deficiencies (if you're worried about deficiencies you'd better run some blood tests for it, and then if needed you can eat foods richer of the deficient micronutrients - but there's no problem to find such foods on a keto diet) - Increase bad cholesterol - that is true in these extremely high (and mostly animal) fat variants of keto diets, which can increase your cholesterol, but it's not difficult to avoid that - just don't go crazy on the animal fats, eat more avocado, olives & olive oil, salmon and similar fat fishes, and your good cholesterol would manage your bad one. I've personally never liked very fat meats, and for me that's been enough to not worry about cholesterol while on keto! - Kidney issues - that can be true, but it's also really simple to prevent - just drink more water (~3L/day is good, but occasionally dropping to 2L isn't a problem /unless you're dehydrated for other reasons too - like once still 112kg recently started keto I went for a not-difficult but ~6 hours mountain hike in a clear summer day, and to compensate I drank a LOT of water - got to 7.5L that particular day/). And I never had issues with my kidney while on keto. For comparison I had to take Acetazolamide for ~8 months after a major surgery I had. And after 4 months I started to feel my kidneys being under stress (I wasn't on keto in that period), several times I had few days with signal I had to pee, but after I've just drained my bladder. After I stopped Acetazolamide my kidneys got back to normal. So for people that already have kidney problems I would recommend to consult with your doctor and if necessary take measures, but keto alone isn't really a problem. - Bone issues - that's the first I've heard of this one, but haven't heard any arguments for that problem. Anyone? I actually before starting the keto had really extreme vitamin D deficiency (4.8 ng/mL !). Under 20 ng/mL is considered deficiency (though after the Pandemic it's recommended to try to stay above 30), under 10 ng/mL is considered dangerous deficiency, and I had 4.8! So I had to take vit. D supplements for a long time before I got it back to ~30. And as most of you know vit.D is actually really important for bones, and I still had no issues with that vit.D deficiency AND Keto! - Increased cancer risk - I don't know of any arguments for this, but I know of some against it! Like that cancers and tumors like to feed on sugars & carbs (as anything really - everyone likes carbs normally), so reducing carbs as on Keto actually can limit the food for cancers. I personally had a surgery to remove a giant Vestibular Schwannoma, but that wasn't caused by the Keto, it was for most of my life (if not all of it), and has a "good" chance to be genetic in origin (I recently found the gene that can cause it, and have to test myself if that was my case). And the thing is my tumor started to grow from the Schwann cells (hence the name), which are basically like the insulation around the wire, but around your nerves (the vestibular/acoustic nerve in my case). But I found some paper (have to search for it again) that Schwann cells actually can't metabolize well ketones - they can a little bit, but not efficiently so they partially starve (more like keeping them on keto diet :)) when you're on keto. However I couldn't find other researches on that topic, and I think the paper that I found was only on research on mice, so not very conclusive for humans. So I'll appreciate if anyone can share info on that topic!

I have lost 40 pounds, just had a checkup and all my blood work has come back into normal range, I have NAFLD, all the markers were off but using just Keto that has been reversed. I can see how this can be a long term diet for me, if I stray and have carbs I find that I feel the affects almost immediately. Carbs and sugar are not worth it. The plan is to lose another 30 pounds and maintain that weight. I am open to new research, but I am not fast to make a switch from the diet without studying the research myself.

Adherence to a keto diet seems to be a major headache for the nay-sayers. Prior to going keto, I prided myself on being a good cook. I loved making and eating artisan Italian bread, curry with loads of Basmati rice, pasta, lasagna, tacos, burritos, roast pork and potatoes, bangers and mash.... I went strict keto 18 months ago and I scarcely missed a beat. I used zucchini noodles for both spaghetti and rice noodle, I fried eggplant and had it with yoghurt instead of rice with my curry, I made taco with almond flour and psyllium. I gave up alcohol not because of the carbs, but because it is a toxin even at low levels (I had been drinking half a bottle of wine a day for years) following Andrew Huberman's podcast. I have now progressed to being primarily animal based - meat or fish with a little asparagus, onion, avocado and strawberries. And I rarely even think of eating things which I used to rejoice in. I have also lost almost 25kg dropped my Triglyceride/HDL ratio significantly, reversed fatty liver, I have a low C Reactive protein and my A1c has gone from 5.3 to 4.6.

Butter is mostly fat with an animal fat profile. Sure, also some more vitamins and proteins. I think it's just the easiest and tastiest way of adding animal fat to lean meats/fish.
