I'm compelled to watch each of your videos at least two times. You cut through all the noise out there - and your calm, factual, gentlemanly (and yes I greatly appreciate that about you) manner resonates with me. I would say you're the best thing since sliced bread... but maybe I should say you're the best thing since ribeye steak 🐮 How about that? As always, you have my gratitude and admiration - thank you so much.

I stopped eating “known” added sugar, seed and vegetable oils, grains and starches on Oct. 22, 2022. While my A1c has greatly improved, it remains in the pre-Diabetic range. (This has caused several other health improvements!) I wear a CGM and have NO spikes ever, however EVERY night while sleeping my blood sugar creeps up at least 30 points and remains that level until late morning. This is the first video that I have ever heard about thalassemia affecting A1c (something that I have questioned my GP about). Dr. Westman, I have an appointment with you later this month and I can’t wait!! Thank you for all that you do!! Your information is priceless and empowering!!

I greatly appreciate the information on this channel. I would like to pose a possible benefit to regularly testing blood ketone and blood glucose levels. Despite adhering to a strict carnivore diet and consuming zero carbs, I was regularly getting higher glucose readings between 98 and 112. At the same time I was frequently seeing ketone readings of .2, indicating I was not in Ketosis. This led me to investigate possible reasons and consulting an endocrinologist. I was diagnosed as being hypothyroid, began taking Natural Desiccated Thyroid. Shortly thereafter, my glucose levels averaged 76 and my Ketone levels averaged 2.0. Had I simply assumed that I was in ketosis by adhering to a no-carb diet, I might have missed this important medical issue. Objective measures may help alert us to ways our bodies are not functioning properly.

I too am learning about ketosis. It is the other fuel when your body isn’t being fed carbohydrates which are sugar . The body will use your extra fat on the body or fats you are eating to fuel your energy needs. Look into it further , the point is you don’t need carbohydrates; they are for the animals we should be eating!

Thank you for explaining the fatty liver part. I was diagnosed a year ago with Non Alcoholic Fatty Liver Disease (NAFLD). My GP recommended avoiding red meat and trans fats…. I went low fat for 6 months and on my follow up, my NAFLD was still there and my cholesterol numbers were getting worse and my blood pressure was increasing. My weight went from 240 lbs to 260 lbs on the same 6 months. I was very frustrated that my condition and overall health was declining even though I was following medical advice. So like many people, I gave up and went on the “see food” diet, as in I see food, I eat it.. ended up at 284 lbs and feeling terrible. Started a strict keto diet and initially felt like crap, but then I was warned by you this could happen so I pushed through. Within weeks I was losing weight and actually started feeling better, better than I had in years! Will be interesting to see what my labs look like and where my NAFLD is at on my next checkup! 17:41

@DrWestman if you are severely IR despite a keto diet then 0.8g of protein per kg of body weight and the rest as butter or dripping is needed to keep BG levels reasonable. A 100g steak is 25g of protein. An egg is 6g of protein. So a 5 egg omelette is 30g of protein and add some butter and thats one meal. The second meal could be a 100g steak giving 25g of protein and 1 eggs gives 6g. Daily 2 meal total protein is 60g of protein and the rest is fat. So if you weigh 75kg you can eat 60g per day (thats 75 x 0.8) to stay in ketosis and keep BG reasonable. Note this applies only to those with severe IR as indicated by an EXAGGERATED DAWN EFFECT.

I started following Dr. Westman sometime after my bypass surgery in 2018. I did lose 80 pounds but still did not feel great. I recently had an angiogram and found my disease is still progressing. Lately I reviewed some videos from Dr. Nadir Ali and now I realize I am eating too much fat which has stalled my weight loss and it is keeping my insulin levels high. The fat circulation in my bloodstream has allowed damage to continue in my arteries. I recently cut back on the fat content and went more towards lean meat. I will follow up with blood test to see if my insulin level drops and my c-reactive reading drops. I am hoping to stop all damage to my arteries. I think Dr. Westman did mention to eat more lean but I did not understand what was going on until I heard Dr. Ali’s presentation. So if you are maintaining high insulin levels be aware.

@@tlet4769 Standard amount of fat from steak, hamburger, meats etc plus some cheese. My fasting insulin was 20.8 in January 2023 in August it was 10.3 . Now that I know what is going on I think I can get it back to normal. My A1C is 5.2 but I do not eat carbs or as few as possible. The circulation of fat in my bloodstream has caused a lot of problems for me. I am probably in a category of 20% of people that have this problem. If your BP remains high be aware. I have to date lost 90 pounds yet my BP will not budge. I think once my insulin drops I will have a chance to get my BP under control and as my weight continues to fall and the visceral fat is removed then I will be able to utilize fat storage capacity again.

I'm so thankful to Dr. Berry, Dr. Chaffee, and Dr. Westman for leading me onto the right path. ❤ I can already see and feel significant changes, but I'm still learning and continue to have questions. For example yesterday I had chicken liver, today I felt not so good. So now I wonder if there was something wrong with the liver, or if I am experiencing what people call keto flue. That's something I have to investigate. I used to eat low carb with mostly vegetables, and lean meat or fish twice a week. Now I am transitioning to almost all carnivore, and I still have some days with a little carbs, because I had bought most of my monthly groceries already before deciding to change my food habits. If I could afford it, I would like to only eat meat and fish in the future. But I live on a budget and an all carnivore diet would be pretty expensive here in Germany. So I will probably keep on eating a little leafy green vegetables together with my meat and fish. My enormous basmati rice stash will rest for times of crisis, which may be coming all too soon. The rest of the carbs I still have will be eaten within this month. I'm so looking forward to buying my new groceries on Wednesday next week. Until now, I never was excited buying groceries. 🤭 Thank you, Dr. Westman, for changing my life. 😊

As I understand it to have ketoacidosis you would have glucose levels of 250 mg/dL or more along with high ketones in the blood. But with ketosis you could still have the high ketones but your glucose would be in the 60s or 50s. Last week while I was fasting my blood ketones were 5.5 mmol/L and my glucose was only 61 mg/dL. On another day my blood ketones were 4.2 mmol/L and my glucose was only 56 mg/dL. Dr. Shawn Baker says to fast 18 hours before having blood tests done if you're eating a carnivore diet to have more normal blood test results.

Dr. Westman. I have a not so simple question regarding therapeutic ketosis. When using metabolic therapies for serious conditions such as cancer or psychosis, the doctors and researchers measure the state of ketosis with GKI which is glucose/18/ketones formula and say that a GKI of around 2 is a good target. To achieve this the person needs a ketones reading of 2 or 3 or 4 if the glucose reading is in healthy 80's. However, many carnivores I know have their ketones readings drop to a constant reading of around 0.5 , especially after a year or so of consistent low/no carbohydrates. Even fasting doesn't raise the ketones much higher. These people can never reach a GKI of 2 or better. Does this mean the adapted carnivores cannot reach therapeutic ketosis? What truly matters when aiming for a "therapeutic" state? Is the ketone production, availability in the blood, or actually usage of ketones for energy? Or is there some other mechanism ketones are engaged in that is a therapeutic intervention?

So, yes, keto works for weight loss, but I don't want to lose any more weight. I still eat cheese, drink milk, and eat plenty of meat (protein and fat) and non-starchy vegetables. I think that being on keto has been good for my metabolism by bringing down my blood glucose, but I'm getting concerned about the weight loss. Is there something I should do?

@@lauristulce3391 If you are eating any carbs, then you could be torturing yourself with fake hunger and cravings. While keto is good, better than eating garbage food, it can still put you on the hamster wheel. Since I switched to Carnivore and went OMAD, never any cravings or fake hunger signals. Many people in Keto will do 16/8 window which is more tolerable but part time OMAD under Keto is tough. Under Keto better to just fast one day a week.

It's evenings after my workout and swim that I run to my french toast and milk (albeit non sugar syrup, lots of butter and whole milk) as a nightly comfort and way to fall asleep. Too me, food is nutrition ( protein and minerals/salt ) and energy, carbs or fat. I think Dr Naiman’s idea of protein to energy level is the proper mindset. I do not agree that carbs and fat are interchangeable. I believe the body hoards what it can when we eat. It prioritizes carbs to maintain the level of sugar in circulation we need without harming us and stores what it can in muscle or fat for later, so it’s out of circulation. I believe it stores most fat right away so it can slowly draw on that energy supply when carbs are taken care of and energy can be used as needed between feeding opportunities. Fat storage is the way to deal with too many carbs and ketosis is the way to deal with too much energy from using fat. This way the body can get all the energy it needs and takes care of extra so we don’t over do it. My personal belief ( and it can always be changed as I learn more ), is that we want to run extremely efficiently and that would mean in super efficient fat burning most of the time and it that would put one in mild ketosis most of time as the excess energy is dealt with. Probably sink deeper in ketosis the longer between meals, likely to rev up the body for the labor of finding the next meal. I think carbs are limited in nature as Bill Schindler points out, which means the body only has to deal with excess carbs seasonally and for short times, so the body goes back to that mild ketosis as the rule instead of carb heavy as the exception. Personally I try to get to a protein goal every day, without worrying too much if I go over, and then watch the fat intake to keep it low enough to keep the body losing fat. Coach Bronson Dant’s approach basically. Always interesting to discuss and learn. Thx for the videos.

What is the best way to test for ketones? I used the urine test strips the first time I went on Keto but never went into ketosis. Second time around…I have lost 60 pounds but seem to have plateaued. Maybe I had started cheating too often. My inflammation in my hip has returned. I have been strict for the last few days. Hoping I will achieve ketosis and reduce the inflammation! I had a urine test recently that showed no ketones. I will have labs again in a few weeks and will have ketones to be included in the labs. My labs 6 months ago were not bad but not as good as I had hoped. I am now more carnivore than Keto. I don’t need to lose more weight but do want good labs! I’ve heard reputable people in this health space talking about keto and carnivore. That you need an insulin bump once a day in general from an adequate amount of protein in regard to kidney, thyroid, and electrolytes. Hormone and electrolyte balances.
