Thanks for the comment! I'm glad these videos have been helpful. Regarding the videotaped patient encounters...ugh. They were filmed and produced with a small team of us using a grant from my educational institution. Despite a preexisting understanding of how these videos would be used (e.g. posted on this KZbin channel +/- a standalone website on clinical reasoning), after they were filmed (but before most were edited) there was a disagreement on that point. I did not want to pour another 100-200+ hrs into these materials only for their distribution to be severely curtailed, so I left the project, after which the whole thing fell apart. A minority of the material had already been finished and is now used by our preclerkship courses (behind the university firewall), but all of the unused footage has been sitting on my hard drive ever since. I've considered offering to purchase the intellectual property rights from the institution by reimbursing the original grant. That way I could use the videos how we originally envisioned them. But it's frankly too much money that I would never recoup. The whole episode was extremely frustrating and an epic waste of time. I'm very sorry it never came to fruition.

I am genuinely impressed by how broad your medical knowledge is. Do these topics come up in your Internal Medicine practice?? Or do you research them for the purpose of making these videos?

Thanks! It's a bit of a mix. This particular video and the accompanying one on female infertility do not come up in my personal practice as a hospitalist, but I'm part of our med school's core teaching faculty and we cover sessions on these topics with our preclinical students. This kind of foundational knowledge helps a lot. In fact, the motivation for the infertility videos came after I realized I had made a false statement in one of the sessions, and that I didn't know the topic as well as I needed to.

Hi have a question can a person over vaccinated from covid 19 vaccine?

Assuming you mean, "can a person receive too many COVID vaccines or boosters?" I don't think there is any significant harm to receiving a few more boosters than recommended. The risk from each shot is roughly the same; you're not going to burn out immune system or anything like that. But the question is whether there is any benefit from piling boosters on one another, and there is diminishing returns from additional identical boosters since your body can only make so many antibodies. However, if a new vaccine were to come out that targets a different variant (e.g. Omicron), there would be additional benefit in receiving that. The bottom line is that I would not personally recommend receiving any more than 1 "booster" for now, until a new version of the vaccine has been tested and made available to the public.
