Very useful information doc. My mom-in- law has PAD and undergone two (one leg each)bypass now.. in a period of one year. Now her one foot went numb last week and we went to hospital. She is high diabetic and already an open bypass heart surgery over and another angioplasty where a stent was inserted.

This is a very good video, thank you for the time and effort you put into it! But I do not think that compression with PAD is correct, it is with venous insufficiency, but when the arteries are blocked it will make symptoms worse. You might wanna add a correction note to the video :)

that is correct. compression is needed for venous disease. compression for arterial disease will only make it worse.

So be very diligent with your lifestyle...early to bed, fresh air, exercise, sunshine on your skin, fruit and vegetables and salad and nuts and seeds and whole grains and fermented foods... and good deeds.

Excellent presentation. I've bookmarked it and will watch again...and again. However, I'm not quite clear about the "compression" business. Are you talking about elastic leggings/stockings that compress the legs?

Intermittent pneumatic compression - for 2 hours daily significantly reduce resting pain, and improve healing rates, physical function, and bodily pain. Its safe, is allowed for long-term use for patients who are not candidates for endovascular or surgical procedures.

Dr. Andreas Fazakas, I think I know what you're talking about, not a passive elastic sleeve but one that is powered by a little motor.. Typically, it's sort of a sleeve-like casing that inflates. When it inflates, it produces pressure on the leg. I think I even have one, but I had forgotten about it. I don't even remember why I bought it years ago. All I can remember is that it was troublesome to put on.You can program it to turn off and on all night long if you want. I'll start using it again if I can find it. If I can't, I'll buy another one, I hadn't known how effective it can be. Thanks. ADDENDUM: Here's something I just found: "Some people with certain health conditions should not use IPC devices. For example, people with leg ulcers, burns, or peripheral vascular disease have a higher risk for problems. Older adults may be more at risk for skin breakdown. Talk with your provider about the risks that most apply to you and any concerns you may have." I'm 81, and I have PAD, so I guess leaving it on all night is NOT a good idea.

A Florida interventionist performed a procedure on my mom's blocked leg artery about 3 years ago, which I have not heard here. It involved going into the leg via her groin. The system cleared away the blockages with some kind of scraping action. It then collected the debris in a mesh, safely removing it. It opened up the flow to her feet and saved her from an amputation. What is the name of this procedure?

The medical treatment have to be optimized. Smoking should be removed. A healthy diet. Exercise. The surgical options have to be evaluated.

Just to clarify-- You're suggesting compression for PAD? Won't compression make PAD worse? Isn't compression for venous insufficiency?

Thanks for the overview of PAD treatments . . . but, if I may say so, the sole dietary recommendation of "more fruits and vegetables" indicates a degree of confidence that metabolic disorders have little or no role in the development of this disease. May I ask what is the basis for this view?

I have burning sensation in my feet and carve day and night I also use compression stockings day and night and still get the discomfort I also stand on my feet for long hours, I also suffer with muscle and nerve problems I just don’t know what to do.

I'm a GP/Family doctor myself. So theoretically a GP should be able to recocgnize the signs of peripheral artery disease with the help of a doppler device. The the GP should then forward these patients to vascular surgeons.

For over 15 years I suffered sever pains in both my legs, seen many doctors, most of them thought it was all in my head even putting it into my medical record. It was all I could do to walk 100 feet to the mail box. Not one doctor was willing to look furthet then reating me for pain. I had 1 doctor do a cat scan, there was only a small blockage at the abdominal aorta. I asked for a referral and basically had to force the referal. Went through all the stress testing. I had blockage from the navel to mid thigh. They tried to insert catheters into groin area both sides, they could not even get the catheter into the vein. Within 2 weeks I was scheduled for a full open surgery. I opted for an artifical vein bypads. Bi laterial aortic bypass. With over 170 staples, I was on bedrest for 3 weeks, had to be careful with picking up or pushing anything. At my follow up I found I only had 2% blood flow to my legs. Felt great could walk again without any pain. I was only 48 years old. Yes I smoked. Had never weighted over 130 and very seldom ill. Now 10 years later having these same pain and walking issues. They told me I had about 10 years on the bypass. I lost or had to have pulled all my teeth due to all the medications given dty mouth. Inflammatory conditions in all my body. Closest vascular doctor is 4 hours away. My question is, if I have another surgery will that graph last as long?? Im 60 now and have had hernia after hernia since my first one. I really dont want to deal with hernias as a result. My fascia it falling apart. After the surgeries I had to undergo abdominal reconstructive surgery. Thanks for posting the information. Thank you for sharing your medical story. My best recommendation is to not "save" 4 hours because your life depends on it. For me to say anything useful without knowing the whole picture is impossible. The surgeon will be able to give you advice with the help of diagnostic tools. I really wish you all the best!

The first thing all doctor's need to check is PTH, Ca, Vit D, and TSH levels. If my doctors would have checked these things when I first began having problems I would not have had to had PVD bypass. The main chemical that clogs arteries is the Calcium and when there is to much it begins to build and block the blood flow to important organs. Always have these tests ran there are ways to cure to much calcium in the blood. The PTH is what determines how much calcium is put into you blood, any thing over the normal daily amount is not good for you stop taking supplements with calcium. Taking out the Parathyroid gland/s will stop excessive calcium and then you can work on getting better.

hi doc. good video. please note that compression is indicated in venous disease only - if used in arterial disease, it will compound the problem.

You are very close to the truth. An Ankle-Brachial Index of ≤0.5 has been proposed as an absolute contraindication to compression therapy. Skin necrosis and, in a few instances, amputation have occurred as a result of inappropriate compression therapy. BUT Intermittent pneumatic compression (IPC) appears to improve walking ability and ankle-brachial indices in peripheral artery patients with claudication, with effects maintained 12 months after cessation of therapy.

Hello, I'm sorry to hear these news! The best thing to do is to talk with very good Vascular surgeon in your area where you live. The treatment of Peripheral artery disease requires long treatment period, and it is important to communicate regularly with a doctors in your town or nearby town. I'm sorry that I can not give you more info than the video itself. I need to protect myself against lawsuits, and the only thing I'm allowed to do online is to give general advise to people. Thats why I'm doing these videos, to helo people understand the diseases. You can find some info about treatment in my video. I have been taking treatment from india. actually i want to share my experience with you. i have been suffering from club foot by born, i got under a operation when i was 13 days old, but the surgery was not successful. now i am 32. i am also a diabetic patient from 27. if u give me ur email id it will be better to tell you the all what actually happening after falling the disease. i need your suggestions very badly.

I wish I could see you because the doctors around where I live dont do anything my legs scream in pain my feet and heels and my right arm muscle is worse than my left. but the pain can be severe .yes I smoked since 8 yrs old im not 57. chantex works but it also causes horrible nightmares and u lose a lot of sleep, cant get them to give me Xanax or volume. to sleep for 4 weeks. instead I just suffer.

Elaine Hamarsheh, i used Vaping (Menthol) to Quit Smoking....Ya start out with Nicotine vapes(justlikeCigs) and you transition to NON NIC Vapes and slowly Step Yourself down until your Off.....It has worked for me. 4 stents. Smoker for 30 yrs(quit) and Drinker for 25 yrs(quit) Walking 2 miles a day now and Exercising again. They wanted to do a by-pass on my leg....I dont think So! I showed them what an Asshole I can be. Go get e'm.

Giving insulin to a person with type 2 diabetes is about the worst thing you can do! That's like giving alcohol to an alcoholic! Type 2 diabetes is characterised by having too much insulin in the system in the first place.
