My wife was helped by PT who performed an A to P Talus from a stabilized Tibia Joint mobilization last week. I now understand the details. I am a health professional who is just now understanding the importance of PT assessment, strength, balance, confidence and return to normal function. wow what a discipline.

No because all physios who use manual therapy are time wasting farces "meakins st al" - I just think the 'no more manual therapy crowd' are too lazy to get their hands dirty.

Great video and explanation of the technique! I am a student physical therapist and have had trouble with techniques like this with the set up and feeling for the arthrokinematic motion of the talocrural joint. My only question is I noticed the knee moving into flexion and the posterior aspect of the lower leg going into the cushion of the table as you apply downward pressure so how do you know you're actually mobilizing the joint versus just moving the knee and leg into the table? Always felt like I was just moving the leg so how do you prevent this from happening? Thanks!

Hi sir you technique really give good inspired to me... I have a patient who is having talus # she did surgery... Her dorsiflexion is very stiff ... I am applying Manual therapy technique. It give good improvement.

I have anterior impingement of both ankles. I had surgery in 10/2020 on my right ankle. Doctor did an arthroscopic surgery on the instep to clear any debris. Also, inserted plastic reinforcement to stabilize the ankle ligament. This surgery did not help with the impingement. I have custom orthotics, another doctor gave me steroid injections, which only helped for about 12 hours. I have been to physical therapy and my doctor will be fitting me for Arizona boots. Also, he will give me a amniotic fluid injections in both ankles next week. I do stretching exercises with a elastic strap that I purchased from my physical therapist. Walking is painful with me having the impingement, which is caused by me having pes cavus feet. I am at a loss for what I need to do next. Thanks.

Hii..i m from India...i went through talus ORIF surgery on 06-03-21..my physiotherapist didn't perform any of these excercises on me..m still struggling hard for full weight bearing.when will I be to walk independently.? I m very tensed

I have a question ..?? I had a spring ankle 6 months ago I have one month already on therapy because when sit for to take a break or when I'm on bed and wanna go somewhere , my first 3 steps I do hurts with my weigh .. hurts in the are of the tendons inside medium malioles .. I do all exercises to make my ankle strong and gain dorsiflexion but in front my ankle hurts and dont know what it is .. exercises seems it's not working .

I had an ocd fracture in my right ankle. Ever since I have had extremely terrible dorsiflexion. When I try to self mobilize ( due to my financial situation) it hurts. Is this to be expected?

I would try some release (Gastrocnemius, Soleus, Peroneals) and maybe some light stretching (Calf) first, and then try to mobilize again. Pain while you mobilize is not great... generally it lead to guarding (increase in muscle activity) which is going to make getting a result tough. Dr. B2

Brent, i took cipro over a year ago and now my dorsiflexion and great toe extension is crap, if i do a reverse lunge and go into toe extension it hurts so bad, i can barely get my knee over my toes during a soleus stretch!

Have you seen a physical therapist? There maybe manual release, joint mobilization and strengthening techniques that can help. Dr. B2

Avacado Papaya im a pta, ive tried to a/p talus mobs, mwm,, alot of calf/soleus stretching, graston , active release, ...it sucks

Brent Brookbush my initial symptoms all pretty much happening at the same time, on my first and 2nd toe, post tib, and heel cord. At one point i couldnt curl my toes , which im still limited in, nor could I do a heel raise, which i can do now no problem. So as of now, my arom toe flexors and extensors are limited, as well as ankle dorsiflexion.

isn't the knee bending when doing the posterior mobilization of the TCJ a compensation? (doing it too much/hard??)

Hey Rach, I could see your point and maybe if I wasn't stabilizing the tibia with my other hand it may be something to watch for. Since I am stabilizing the tibia with my hand and opposing the force created by the mobilization, the knee angle itself does not matter for the mobilization to be effective. Dr. B2

Why don't physical therapists use grade 4 mobilization? I noticed you use it in at least some of your videos when you for example do the whipping motion with the cuboid bone in the foot and I wondering whether or not you are physical therapist or chiropractor or either one thank you

Great video. Can you do something like this with your foot on a chair and a resistance band around the front of the ankle and then do small presses forward with your knee. I have tried this and it seems to help. But why does the ankle dorsiflex with more ROM and easily but then the next day it is stiff and feels like there is some gunk at the ankle crease preventing good, easy gliding movement without feeling like there is swelling or some kind of very uncomfortable pressure at the ankle joint.

The technique is wrong. You should stabilize tib-fib with your left hand and mobilize talus A-P direction for DF improvement.

No... watch the video again. I am moving the talus and calcaneus as one unit, while the tibia is stabilized by the table. This is a Tibiotalor (Talocrural) mobilization. Dr. B2

Hello sir!Your physio technique is vry help for us..Actually my spine L3 level got fractured and I'm facing walk, my problem is. knee,toes n ankle mobilisation and strength is very weak so I didn't walk continue without support..dr.saysbmy nerves is damaged but improvement is going on but very slow. How to recovery this.

I agree, stabilize the tib-fib, mobilize the talus, and use your leg to change the level of DF- you have to stabilize proximally
