I know this is regarding a different technique, but if you have an answer I'd appreciate it... This is regarding a posterior to anterior technique on the knee, while patient is laying on their back with injured knee bent. PT puts fingers behind the knee, pulling toward themselves while patient bends knee and then straightens again. My curiosity is... I had a knee injury and when my physical therapist does this while I am on my back, it is the only way I can bend my knee deeply without pain. The pain is so intense if I try to do it on my own that I can't. The MRI failed to show why (only showed unrelated pre-existing minor things that never limited or really affected me at all, was active and fully capable). I've become quite disabled after this injury and felt the injury when it occurred. Please, if someone can tell me, are there any other damages that this technique would 'bypass' or allow the bending of other than meniscus tears? If the patella is maltracking due to a ligament tear or something, would this technique also work for that by supporting the knee or something?

If patient has hyper golexion or h ty per extension can we give rela to ivelive opposite mobilisa ty ion ( For hyper flexion of knee jt give posterior mobilisation ) If not why not ?

Why are you doing a joint mobilization in the closed pack position? Wouldn't you get more joint movement if the knee was bent, similar to the anterior drawer test position?

Great question, it ends up that there is some research to suggest that mobilizations may be a bit more effective when done at end range. I don't think open pack mobilizations are wrong (providing you follow them up with exercise, and a home program), but it is certainly food for thought ;-) Dr. B2

Aloha Brent! I would assume that this technique on someone with a knee replacement (same for hip mobilizations on hip replacement) would be a contraindication. Also, my brother has had numerous knee injuries with lots of scar tissue build up over forty years. His knee is always flexed, now giving him more pain than he'd like (not arthritic) so knee replacement wouldn't "cure" his problem. Would this help him? Thanks1

Hey Leslie, Once you have doctors clearance it may actually be recommended to do this technique. As far as your brother, he should see a PT for an evaluation. Dr. B2

@@BrookbushInstitute Mahalo nui loa, Brent! I will let him know and having had a knee replacement gone bad (from the beginning), I will give mobilizations a try. I've tried everything else to no avail. I've been diagnosed with a mild/moderate CRSP. BTW I love your teaching videos...very clear and concise and I especially love your reinforced "close ups" at the end. The only thing I might add would be a little more on contraindications for each one. I'm a medical massage therapist who always proceeds with caution on any given new technique. Thanks again!!!!

hi, what about inverting the semi round foam so the flat surface in under the calf. this would disperse your force over a larger area and avoid the top of the half circle digging into the calf. also you could get a mechanical advantage by right hand placement more distal to the knee, the edge of the foam would be proximal to the Tibia's joint line.

Hey Gilberto, As far as the foam roll, try it. I think you may get some rocking from the foam roll that you don't want, but it's certainly worth testing. As far as the hand distal to the knee... you have two vectors here - one pushing anterior to posterior along a vertical axis and another creating a torque (circular force) into extension. If your goal is to optimize glide, than you want to be as close to the joint line as possible, reducing extension/torque forces. Dr. B2

I have seen in a clinic that some clinicians medially rotate the tibia when doing the knee mobs. Why is this? How important is this?

That may preferentially lengthen the posterior lateral tissues... I guess I would need to know what the reasoning was. Dr. B2

We are geared towards both professionals and students and mobilization techniques/manual therapy techniques, etc require a lot in order to make sure the practitioner is in the right location and performing the proper technique. As an educator I want to make sure fellow practitioners know what they're doing.
