Ankylosing spondylitis is an inflammatory condition that can cause the tissues that attach to the spine to swell. Over time, this can lead to bone formation and possibly fusing of the vertebrae. This type of arthritis can result in a loss of flexibility. See Doctor Jo’s blog post about this at: www.askdoctorjo...
These simple stretches and exercises can help improve range of motion and decrease pain.
The first stretch is called the pelvic tilt. You will roll back onto your back with your knees bent. You want to imagine pushing your belly button into the ground, or push your back flat onto the ground like you are squishing something. You can put your hand under the curve of your back, and try to push your back into your hand. Make sure you are breathing and not holding your breath while doing these exercises!
Next try some gentle trunk rotation stretches on your back with your knees bent in hooklying. Gently rotate your bent legs from side to side. You can hold them on each side for 3-5 seconds or you can continuously rotate them back and forth. Again, try to keep your lower back on the ground. You can start off with 10 and work your way up to 20-25.
Now you will do a bridge by pushing your bottom up off the ground, but try to do this slowly, and do one segment of your back at a time curling upward. Then slowly go down the same way, one segment at a time. You can start off with 10 of these, and then work your way up.
Still lying down on your back with your knees propped up, you are going to do a simple knee to chest stretch. Bring one knee towards your chest by grabbing underneath your knee and pulling up to you. Hold the stretch for 30 seconds. You shouldn’t feel any pain, just a stretch through your lower back. You will stretch each leg three times. You can alternate back and forth or do them all on one side before switching.
Turn over in a prone position on your stomach. You will prop up on your elbows for a prone prop. Try to keep your hips down on the ground while your stomach and chest come off the ground. If the elbows are too much, you can come down slightly to where it is comfortable. Hold this for 30 seconds and do three. Then if that becomes easy, still in the same position, now come up into a push up position on your hands. Still try to keep your hips down on the ground if possible. This one is a little more stressful on the arms, so you might want to start off with 10-15 seconds, and work your way up from there.
For the last set of stretches, turn over onto all fours, or quadruped. The cat/cow or cat/dog stretch is performed by tucking your chin in and arching your back. Then drop your back down and lift your head looking forward. You can hold this for 10-15 seconds and alternate back and forth for about a minute.

Hi Dr Jo. I loved your video and exercises. I was diagnosed 7 years ago after about 15 years of almost constant pain and sometimes excruciating pain. I’m now 47 and have never been more pain free since my mid 20’s. I very rarely feel pain or stiffness anymore due to the stretching regime I do daily. The stretches you have shown are exactly the ones I have been doing for about 5 years now and they work for me. They have been totally life changing for me to the point where I hardly even think about AS anymore. I hope this helps any other sufferers of AS out there. Take care.

Been going through this since my early 30’s until now at 51. I can go a few years no issues as I do exercise to strengthen my core best I can. But every now and then I’ll bend over to pick up a tissue or something small, twist to throw it in waste basket and then stand up straight too fast and BAM! Screwed for next couple weeks hoping it doesn’t roll into a full blown sciatica. Because that is hell. Long story to say yes, I believe stretching is just as important, if not more important than just working out with weights, etc. Took this last bout to figure it out. Even when you think you’re in good shape. Will humble you real quick. Just my little story.

Holy shnikes doc Jo. Short of the long is I was doing these exercises as I was watching the video. Amazingly enough I was able to finish and got up and was standing straight while I walked. I wasn’t even doing them all to reps, just a quick test we’ll say. So, the one exercise where you pull your knees up, the right knee no issues. The left pull up and I could feel the pain on both sides of lower back. Anything more you could add? Just keep doing them as you instructed?

My mom has had AS since she's had it her whole adult life. I am aware that's it's hereditary do to the HLA-B27 antigen which makes me twice as likely to experience its symptoms. I'm 25 and according to my research, people are diagnosed with AS when entering adulthood (17-18). To this day, I've shown no signs of the disease. Does that mean I've been exempt or do I still have a chance to contract it later in life? Thanks for your answer. Cheers!

After a Roentgen the assumption is that i got Ankylosing Spondylitis, but they say that i have to go to the nerve specialist and the doctor said that there is no problem with my back and only giving me some medicine, injection, and Flamar gel without telling me anything and now sometimes my back hurts so much...

Thanks for the video. (Cute socks BTW) Everything I've been reading about AS says that it usually starts between the late teens and around 35 y/o, but I didn't had any noticeable signs until around 46. Though I haven't been Properly diagnosed, I've has x-rays done by a chiropractor showing a large calcium build up in my lower spine, and a tilted pelvis and slightly tilted spine. Plus Every other sign of AS, except the age thing. Pretty sure ive got it though. And these excersises will help out a lot. Thanks.👍

AS gets detected mostly during your early 20s to 26. That what I got to know from my doctors, and the amount of time I spent on death bed searching about it. I bad AS, so much so that my lungs were infected, my eyes were sore, my intestines were filled with ulcers which used to bleed.

AskDoctorJo There’s a lot more to it than that. It doesn’t just develop like regular arthritis. It’s hereditary. In order to have it, you have to test positive for the gene HLA-B27. But just because you test positive for the gene, doesn’t mean you have it. A lot of factors go into it. With a positive blood test, you’ll start being monitored for it. They’ll look at your CRP level, sedimentation rate, any and all inflammatory markers, your metabolic panel, and your CBC. It’s routine blood work every time you go to the doctor because it’s a condition that will affect you for the rest of your life. I was diagnosed in July. There’s no cure and they don’t know what causes it, just that you’re born with it and, if you test positive for the gene, you better start praying. Mine started as general arthritis in my hands and knees when I was 12 that spread throughout my body over the years. I’m now 27 and it JUST spread to my back last year. Many things do not cause it. The only known cause is your genetics.
