Well done. I’m a joint replacement surgeon in Tacoma, Washington and have been doing both minimally invasive anterior for ten years and modified minimally invasive posterior hip replacements for over 20. With that experience I select the approaches based on the patient’s anatomy and activity situation as well as their spine arthritis status. When an experienced surgeon selects the best approach for the overall scenario the results between the two are identical. Thank you for putting together an unbiased and accurate assessment of the current two techniques. Wel done!!

@,robertmurian I went for a consult with a Dr here in PA. He said I have arthritis and reccomended replacement. But I was uncomfortable with his reaction when I asked him a question about my x-ray. I asked if the replacement would take care of my labrum tears,he seemed angry and abruptly replied that is nothing. He then left, what am I to do? Should I accept the replacement under these uncertain conditions

Posterior right total hip replacement done six weeks ago. Home the same day, took meds one day just because I was "supposed" to. Two days on a walker, three days on a cane and i was walking on my own with very minimal pain just took some Tylenol. For me the tried and true posterior was my choice, and I recovered so fast I'm glad I did it. Once I got the metal sutures out any lingering pain in the incision are was greatly minimized. I walk two miles every day effortlessly. Best thing I ever did my right leg was in a lot of pain beforehand. DON'T BE AFRAID.....modern surgery is so advanced. You'll never look back.

I had posterior on one side 5 years ago and anterior on the other last month. Anterior is better in every way. Cutting through the muscle is barbaric. The recovery time was much longer and more painful. I'm 5 weeks out from the anterior and I'm walking 100% normally and feel very strong. I stopped needing a cane at 3 weeks. The posterior one had me on a cane for many many weeks. I was in the hospital for 3 horrible days vs going home same day. Even 5 years on, I have weird pain and tightness in my quad muscle. I've had it looked at and it's from having my muscle cut through. It wasn't done exactly right. Yes, I don't have arthritis pain anymore but the result is still sub optimal and the recovery period was pretty awful. I made a mistake the first time in not doing more research and finding a more modern surgeon.

I had a THR 4 yrs ago. Posterior approach. I was born w/one leg shorter than the other so my hip was bone on bone. I was in the hospital one night and was in rehab for 16 days, which I enjoyed, and was given both PT & OT everyday. I have had no problems at all and what a joy to walk w/out pain. My other hip & both knees have full cartilage, and no significant arthritis at all.

I had a posterior hip replacement with a trusted surgeon. I have had no problems since recovery was over except I decided to wear a heel lift of 7 mm even though the surgeon did not recommend it despite the fact that he said there was a 7 mm leg length difference. I went without for several months and got tired of limping so I added the heel lift. Now all is good. Even though my surgeon was great, there was no preparation prior to the surgery, no rehab after, we had no idea how to prepare for recovery at home. In addition, the first 2 weeks were brutal. For some reason, the burning pain I had in my buttocks made any movement very difficult. I don't know why that is. But now, I need a hip replacement on the other side and I am very interested in an anterior approach because I have spoken to several other people who had that and said the recovery was so much easier.

I'd find a new surgeon if I were you. They did you dirty with all the things not mentioned and having your hip that much shorter. My doctor, (who did anterior) and hospital had multiple lessons and pre op things and exercises to do and then gave me in home rehab for a month until I went to outpatient. He had done about 25,000 surgeries over his multi-decade career as an orthopedic surgeon. The surgeon is the most important part of this process for success. They are not all created equal.

I'm one of the exceptions--I had anterior a few weeks ago. Three days later my hip went out of socket. ER put it back in. 10 days later it happened again in my sleep. I'm having a second replacement in a couple of days. The incision healed well and was relatively pain free. I believe my surgeon is very capable--I'm looking forward to the revision. The pain of a dislocation is intense.

Thank you so much. No one told me about the possible complications of anterior hip surgery - involving more blood loss and nerve pain. I had both and needed to spend 3 nights in the hospital to recover via blood transfusions and pain meds. All is better now - 4 months post op. I'm not sure why more information is not openly shared with patients prior to or after surgery. I told the PA at my last visit, "I'm not a car, I'm a human, talk to me."

Had both hips done. 1, 3 years ago, the other almost 2 months ago. I used the same surgeon, he does the posterior version. I don't understand why some people do not have restrictions with this approach, some enlightenment would be nice, thank you in advance :). For me I had the same restrictions for both. No crossing the legs, no bending over past 90 degrees and no twisting the leg inward. I think the next statement is important here, I am relatively young, 48, and I am not over weight. I played hockey, sports in general so I am pretty athletic and strong overall. IMO the restrictions aren't that big of a deal, do not let them scare you, if anyone feels that way. THE NUMBER 1 by far most important piece of information i can give someone, is, make sure you use a good surgeon. This is your body, interview them. Ask them if they have any previous patients that they can talk to. Do your research. I could have very easily gone with a new doctor and done the anterior approach but NO WAY, my first replacement went so perfectly I didn't even think about using another doctor.

I had anterior approach surgery 2.5 weeks ago. Came home same day not needing a walker or cane. I had no precautions or restrictions. It was my left leg and not using a narcotic I drove the next day. By day 6 I was back to walking my 10,000 steps a day. As well as doing the required exercises. I had 3 PT appointments and after that have continued the exercises on my own. After 2.5 weeks my range of motion is back close to 100 percent. While my left thigh strength is 80 percent back to its pre surgery level. I highly recommend the anterior approach for anyone who is considering hip replacement surgery.

After discussing both approaches with my surgeon, the head of orthopedic surgery at a major hospital, there was no way I wanted the anterior approach. Success is dictated by ur body and the skill of the surgeon so it’s futile relying on someone else’s experience. My only issue is that I did not ask enough questions about sex. My surgeon recommended standing which I hate and the only peer reviewed studies were done in the 70s so surgeons cannot give u definitive information. The overall advice I was given by my surgeon was to chill and rest for the first 8 weeks. No PT until he clears me. Walk as much as I want. Drive when u are comfortable. Work out at the gym but avoid any hip exercises. No swimming until he clears me at 8 weeks. So everyone is different. Listen to ur body and surgeon. What sucked were the opioids. I got off them on day 3. They made me anxious, disconnected, and constipated. I’m a side sleeper so sleeping is still crappy. What I love is all the pre opt pain is gone including back pain. Absolutely worth while. Do not be an idiot like me and put off ur THR for years. The new technology is amazing

Believe me I feel a million times better 6 weeks after the surgery. My team was amazing and they knew going in I was a train wreck of fear. U can do this! I know u can because it’s basically an unfounded belief that is holding I back. There’s a YT on stoicism u should look up. A guy does a bunch of shorts about stoic thought. His take on fear really helped me self analyze why I felt the way I do U and I deserve to be the best we can be. I was in so much pain for 7 years I missed out on so many family activities. Thanks so much for putting out such a balanced and informative review of the two THR methods. I’m a very active 59 year old but have been persevering with increasing levels of pain and restricted ranges of movement over the last 10 years, so the time has finally come to replace the hip. Living in Singapore, approx 90% of surgeons seem to opt for the posterior method, and given my goal is to get back to an active state, the number of articles advising against the posterior approach due to risks around dislocation and slower recovery have been quite concerning. Your review has really calmed my nerves because your comments about the relative outcomes and risks associated with modern posterior approach v’s the anterior method, balanced against the experience and preference of the chosen surgeon is exactly what my referring physician suggested is most relevant. So, looks like I’ll be proceeding with a Makoplasty (robotic option) posterior approach - Much appreciated!

Having the other one replaced in Jan with an Anterior approach. Outpatient surgery. After both of the posterior surgeries, about 3-4 weeks after, muscles “Snapped” back into place, which is quite painful for a day or two. From my understanding, the anterior approach goes between the muscles that will eliminate that issue. For those of you that have never gone through this before. Just follow your DR instructions. I am 70 and had a posterior THR on 11 December 2023. I had pre-op tests (including blood and ECG) and was given an epidural and a sedative for the operation. The surgeon said all went well and that I didn't lose much blood during the operation but I had an internal hemorrhage post-op and my hemoglobin count dropped from 15 to 7 so they put a couple of bags of blood into me. I was also on iron and folic acid medication for a couple of months post-op. Due to the hemorrhage and transfusion, I ended up staying in hospital for a week and had difficulty sleeping as I'm not used to sleeping on my back and had to share a ward with two loud snorers (!), but once I got home, things improved greatly. My main concern is the slowness of my recovery. My surgeon said physio would not be necessary and recommended some exercises and just walking. I moved fairly quickly from a walker to crutches and by the time I had my first revision (which included an X-ray) on 26 January 2024, my surgeon recommended that I move to one crutch. However, it is now nearly 5 months since my operation and I still have a lot of stiffness in the joint and cannot walk unaided without a limp. Is this normal?

I am struggling with a huge wound 33 clips later and still suffering. This is not the way forward posterior is much more acceptable. and the least bloody painful. I lost over a lite of blood, none in the previous procedure. I have a scar on my right hip 4 Inches long... great recovery. My recent hip replacement has been torture, different surgeon different approach. My scar is like I have been in a fight with a great white. I do not understand why we cannot receive the best and least invasive of treatment. Oh I forgot surgeons have their own way of doing.

I dislocated my right hip way back in 1980. Now, 41 years later, I really need a hip replacement. My only concern is who will perform the surgery. My only choice on the matter is it must be performed by the V.A. Hospital located in Little Rock Arkansas. I have used this Hospital since 2002, when I retiired after 20 years from the U.S. Air Force. As it stands right now, I just wish I had some more information on this procedure performed at this hospital. I welcome any and all comments. Thank you all.

Your concern really is valid. Frankly I would look for recommendations within the community and ask your local docs and PT folks WHO THEY WOULD USE if they needed a new hip. I played ice hockey for most of my life. At 57 it was time for my right hip to be replaced. I actually chose a surgeon that I skated with for a few years. He really had a great resume', trained in a practice with a physician that is always number 1/2 in the state of Florida annually as to number procedures performed. Then I spoke to a fellow player who was back on the ice after 6 months with fabulous results. All in all I am happy with my choice 17 days post op. The incision however is a lateral incision from what I see. Not anterior and not posterior. About 5" in length. No magic rehab to report yet. All going well but still pain down the leg through the knee and tibia sometimes throughout the day but slowly improving. As a vet... I would want to know the experience of the VA surgeon. 100 hips a years seems like a nice place to start to make sure they are very well experienced.

@@teleguy5699 Thank you for your input. I do feel I should provide you with an update. I had the Total Hip Replacement performed on 31 May 2022. It was performed at the John L. McClellan Memorial Veterans Hospital located in Little Rock Arkansas. My surgeon was a Dr. Jagadell. The operation and follow-up care were excellent! No problems at all and now I can walk properly again. Granted, it has taken me longer than expected to regain my strength. I lost a lot of muscle in my legs prior to surgery. I was on a cane or walker, and it was either hip replacement or wheelchair. My life is much better now. I thank all the staff at the hospital for all your skills and care. I guess Little Rock has a really good staff. I would rate them A+.

I am very happy with the results and at the end of the day if the operation is successful your not going to quibble about having a bigger scar.

Going in for total replacement on my left hip in 11 days. Posterior approach. I've been working with this ortho DR/surgeon for almost two years on my arthritis, and I finally made the decision to do surgery. He told me a year ago that even though he can't force me to decide to have surgery, that he would "Schedule it next week" based on the X-Rays. There are days I cannot walk now due to pain and the fact that it floats around in the socket, and I don't always have control of it. I can't imagine any amount of post op pain could compete with how it is currently. I work in the restaurant biz and am on my feet for 8-12 hours a day even with my femoral head so flat the x-ray looks like you could set a drink on it... My surgeon has 35 years experience doing joint replacements, so I am going to trust him on this and not seek one who does the anterior method. I hope that is the right call.

I'd say I was on 2 crutches for 3 weeks then 1 crutch up to 6 weeks. I dread it if my other hip gets worse and I have to have that done. I was freaking out about only a spinal too. Best decision I made afterwards. They give you, relaxing meds, then the needle, put you out with some sleepy meds, and when you come to, no post anesthesia fog for days. I was pretty alert and relatively pain free. When the spinal wore off, they gave me opioids, Tylenol, and Celebrex. As I have had over a dozen general anesthesia surgeries, I highly recommend this route.

I had posterior lateral hip replacement 2weeks ago. 8” incision. It was rough. 3 nights hospital stay. Lots of pain. Leg swollen and numb. Just now I am feeling better but need a walker. I have to get the other side done. Should I be okay with the same approach or should I ask for more minimal invasive? I looked up briefly the different approaches to surgery before, and everything I read seemed to say that it was pretty to recover from either way. But did I choose a surgeon who is ‘old school’? Should I have been in hospital for three nights when everyone else seems to go home the next day?
@Aviation_Professional

Seems excessive. Just did mine and due to C19 they asked if it was ok for me to go home same day. I accepted and completed the surgery. Lateral incision 5" long. Surgeon used the Zimmer Biomet G7 implant. While I am by no means PAIN FREE at day 17, I am improving slowly but still use a walker and ice. I think these guys and gals riding bikes and returning to normal activities after 3-4 weeks is simply very RARE. Pick your surgeon wisely for sure. There are some folks we all need to stay away from out there. Ask your local docs and PTs who they would use.

I had my 1st surgery when i was 35 ,that lasted 2 years while i was still in pain ,walker ,i told the surgeons they said everything looked good ,pat me on the back i go on like this for 2 years ,the surgeons decide to do another surgery after 2 years,si onto my second surgery, same results,so at some point they broke off a drill bit in my hip, after surgery i and my husband was told they left it in there because it would do more harm than good ,i didnt know any different, after 2 years of that hip replacement i switched dr and he said the problem was that they needed to customise the piece to fit my body,i didnt know this was a thing but it worked fabulous .i got my life back and felt so good after 14 years of that replacement i had begun to have issues ,back to my surgeon he said its time for another hip replacement so i go ,had that hip put in and 2 years later ,working as a cashier i would go home and could barely make it out of the store with so much pain,,limping,i get home and couldnt wait to get in my recliner, food wasnt really an option nor was anything else ,i continued like this working and then my hip after months started to lock up so i decided then that something was going on ,after emergency room visit ,dr said there is something going on we are going to refer you to an orthopedic and spine dr ,ok i went they said i was way more than they could handle, referred me to a dr in little rock ,he right away said you need surgery,i am in tears a month later i had the surgery, after surgery the dr told me when he got in there he said it look like a bomb went off in there,he said i had so much metallosis in my hip,he cleaned it out ,also said the surgeon before that should have did the same approach because i had metallosis,i wasnt told about that, anyway to make things worse ,this dr in little rock said he cleaned out all of the metallosis, that there was dead tissue ,bone damage ,surgeon put in a new liner and antibiotic spacers ,said you will need to come back for another surgery to put the foreign parts in ,no telling when that will be ,its been 8 months and i still have pain ,cane assisted ,i go back soon to see my surgeon to see what is next so if he says surgery that will be my 6th surgery posterior .i am not thrilled ,i am older now snd i dont recover fast ,i need to get my life back ,oh i was also told i have part or parts that were recalled ,.i have depression and anxiety from all this .dont be discouraged because if you get the right hip replacement ,its the best thing i just happen to have gone on a rollercoaster of events ...sorry my phone isnt automatically putting punctuation marks where they should be .before my last hip following up to what would be my next hip i just thought i was doing alot at work as we stay really busy at work so thats was just part of it why i was in pain ,exhausted ,limping ,so just pay attention to

There are way more variables than just slightly quicker recovery which is debatable. The bottom line is whatever one you get, FIND A GOOD DOCTOR FIRST!!!! By your logic you would pass up a Dr who did 10,000 old school replacements and pick the Dr who did 1 of the modern technique. Good Luck with that logic.

Thanks for your info buddy but you could have said it in half the time. You lose your audience if you drone on with long intros etc. It took you 4 minutes to get to the point. Local anaesthetic for a hip replacement? Er, no thanks. Some orthopaedic surgeons disagree with what you are saying re nerve damage and dislocation - most notably those who specialise in the posterior approach. They say the anterior approach has a greater risk of dislocation and ill-fitting from the outset. They agree with the possibility of front skin nerve damage with the anterior approach. The risk of blood loss and transfusion they didn't mention at all.
