It's 4 years of college/premed. 4 years of medical school. Then onto postgraduate programs, masters, internships, residencies and/or fellowships. If the parents are lucky, they finish school. But there are quite a few that keep going. I know an MD that also got his DMD, then became an oral surgeon.

Where I'm from, surgeons are traditionally addressed as Mr (or Ms/Mrs/Miss) although that is being phased out because of all the identity stuff. They leave the title Doctor behind when they get to slice & dice.
