Thank you, most interesting. I have been waiting for a Dexa Scan for nearly 18 months, I was put Risdronate which gave me side effects, took myself off them.. Now the idea is to put me on Denusomab (Prolia). Still have not had a face to face with anybody ref this. But this is in the UK where all health things are in a bad way. I do watch your videos and I am learnng a lot, on how to improve my diet etc. so I thank you for them

I am 61-62 years old female. Recent DEXA T-scores are not so good (-2.2/-1.6/-1.1, spine/femoral/total), in the osteopenia range, but FRAX calculation shows risk as 3.8%/0.4%. I think I fall into the 4th guideline. I am determined not to go down the pharmaceutical route. As a scientist in life science, I am not convinced that increasing bone density (as a readout on DEXA) can be translated into bone function. The information you put up is very helpful in various aspects and is much appreciated. As far as exercise is concerned, I dance (ballet) 2-3 times a week. In many aspects I believe it is high intensity resistance training but I don’t know if it is enough for bone health. As for the diet, I definitely don’t get enough protein (micronutrients are fine), barely 30 gm/day, based on the calculation (1 gm protein for 1kg body weight). Is there any protein supplement (like drinks) that I can take easily to make up the gap? I weigh about 50 kg with a BMI

Hi Grace, Thank you for sharing your story! Your right that your DEXA scores are low but they are not terrible. This is a good starting point. I have many talks coming on bone drugs and BMD. You are correct in that BMD is not always strongly associated with bone strength and fracture prevention. We are more than our bone density. That said, density does matter to some extent. We can't ignore it but there is more to it. I bet you didn't know I was a professional ballet dancer :) I know first hand what it takes. That said I think it mostly fills the first requirement and minimally the second. It requires strength but not to external load. It also depends on the intensity in which one is doing it. For most I bet it's not enough. Protein needs we aim for are 1g/POUND, sorry for the silly American/English units. 2.2g/Kg. So, yes it sounds as if you are deficient by these standards. I like Whey for those that con tolerate it or Beef isolate powders. If you are vegan pea protein is the most complete protein powder out there. Thanks! For protein quantity calculation, I am giving myself some leeway. There is no way I can consume 110 gr of protein. 😭 Typically it is 0.8 gr/kg but for older people the ratio goes up a lot. For the exercise, I agree with you but that is the only exercise I have been doing. 🩰 Definitely will try to join the master class to gain more insights.

I’m a 62 YOM who has always been active in sports, soccer, skiing, hiking, etc. due to a malady of my youth I had to have bilateral hip replacements in 2011 but stayed very active in sports. I had a routine bone density scan and was shocked to learn I had osteoporosis with a DEXA T score do -2.9 in my spins and wrist. Huh what?!. My physician says bisphosphinates haven’t proved to be effective in preventing spine fractures but will put me on Fosamax if I wish. That’s where we left it. ??????? I have read a bunch of contradictory and cautionary but supportive stuff, a lot of squeeze for a little juice, and other article saying I must get on the medication straight away, all on the internet . I don’t really know when to go at this point.

I think more needs to be said about the fact that most drugs once started must be continued for life. Also the blood tests need to be regular screening tools for women in peri-menopausal and post menopausal. American RN 30 plus years wellness focus.

Hi Simone, Completely agree but would add one additional issue. Most of the drugs cannot be taken for life. Prolia is the only drug that has a long track record at this point but I question if we just haven't seen the effects of poor bone metabolism with long term use yet. Drug therapy simply doesn't have the long game in mind. They are tools that can help prevent fracture in the short term and I believe there are cases where that makes sense but ONLY after a comprehensive plan is in place. Most of my patients do great without drugs and when we do recommend them (rarely) it is for a brief period of time with Healthspan in mind. Love your Podcasts, Dr Lucas and have learnt so much. I am 67 year old female and have a T score -3.1 at lumbar sine and - 2.9 in hips. I had a L2 fracture a few years ago. I didn’t know till I x-Ray. Should I start taking bone meds or can I be redeem through lifestyle changes😢?

Interesting that according to the tool, previous mid/long term prednisone use basically doubles the risk. Pushes me to 14%/8.5%.

Thank You Great info! I never knew this tool existed Assuming we can the info from a DEXA Scan I scan periodically Then use the T-Score to enter in this FRAX tool? Other means or tools to obtain the T-score would be from an MRI? another question then🙂 DEXA Scan.. Would this be ok and " reliable "? I understand per a previous video..it was not the best

Hello, DEXA has limitations, yes, but it is the easiest and most available way to get T-scores. I have a video on imaging for DEXA you could check out! Another modality would be REMS/Echolite. It's an ultrasound study that gives T-score and fragility score. It's great but not abundantly available. You can use a T-score from either to input your FRAX data and see risk. You can also get a risk percentage without a T-score.

Not sure what you are referring to here but the FRAX calculator does the math automatically.

Yea I know that it automatically does it but in your example you placed 125 pounds for conversion it should have given 56 kg automatically But somehow it gave 86kg which would affect the frax score I believe
